BACKGROUND: Trapeziectomy with suspension arthroplasty is a reliable treatment for thumb carpometacarpal (CMC) osteoarthritis. We modified the suture suspension technique to add further support to maintain the arthroplasty space by interposition of the dermal allograft. Our aim was to investigate the surgical outcomes of our technique of suspension arthroplasty with dermal allograft interposition (SADI) compared with those of traditional ligament reconstruction with tendon interposition (LRTI) using a half-sling of the flexor carpi radialis (FCR) tendon.
METHODS: This retrospective study enro

UNLABELLED: The poly-L/D-lactide joint scaffold (RegJoint™) has recently been associated with adverse tissue reactions and osteolysis after partial trapeziectomy for trapeziometacarpal osteoarthritis. Twenty-two of 23 patients previously operated on with this scaffold were re-examined at a mean follow-up of 3.3 years (range 36-53 months). Overall, the results showed an unacceptably high rate of adverse tissue reactions related to the degradation process of the implant, resulting in a revision procedure in three patients. At final follow-up, at which point the implant had completely degraded, t

PURPOSE: To investigate the presence or absence of union of the new trapezium to the retained metacarpal base after pollicization and to relate this to stability of the new trapezium and the new carpometacarpal joint.
METHODS: Thirty-six patients (46 pollicizations) were assessed at clinical review. Mean time from surgery to review was 96 months (range, 9-260 mo). Clinical assessment measured range of motion (ROM) at the carpometacarpal joint, stability of the carpometacarpal joint, and extrinsic and intrinsic strength of both hands. Radiological review evaluated 3 parameters: bony union betw

Rhizarthrosis is the most common degenerative joint disease of the hand, affecting about 10% of the population. We report our results with trapezium replacement using a pyrocarbon spacer. Between January 2005 and April 2010, 70 patients underwent trapeziectomy with interposition of a pyrocarbon spacer. Sixty patients were examined at an average follow-up of 23.6 (5-64) months after the operation. Six (8.6%) of the 70 implanted pyrocarbon spacers dislocated. Based on the assessment scale devised by Buck-Gramcko, 19 patients achieved a very good outcome (31.6%), 31 patients (51.6%) had a good ou

We describe an alternative method for treating chronic trapeziometacarpal (TM) joint instability after acute injury or chronic repetitive use of the thumb by performing a dorsoradial capsulodesis procedure. The procedure is done by imbricating the redundant TM joint dorsoradial ligament and capsule after reducing the joint by pronating the thumb. The dorsoradial capsulodesis is a reasonable reconstructive option for chronic TM joint instability and subluxation.

PURPOSE: Pantrapezial osteoarthritis affects the trapeziometacarpal (TMC) and scaphotrapezial-trapezoid (STT) joints, causing pain, functional loss, and sometimes necessitating surgery. Among the various reconstructive options available for pantrapezial osteoarthritis, trapeziectomy and ligament reconstruction with tendon interposition have consistently yielded favorable long-term outcomes in many series. However, implant arthroplasty remains a valuable alternative for selected patients. This study evaluated a dual-prosthesis approach designed to preserve carpal stability and optimize thumb fu

PURPOSE: To compare in trapeziometacarpal (TMC) osteoarthritis the effects of trapeziectomy with tendon interposition and ligament reconstruction (LRTI) with or without a bone tunnel after a mean follow-up of 5 years.
METHODS: We randomized 79 women (aged 40 years or older) with stage IV TMC osteoarthritis to either trapeziectomy with LRTI using a bone tunnel (Burton-Pellegrini) or a tendon sling arthroplasty (Weilby). Before surgery and at 3 months and 1 year after surgery, patients were evaluated for pain, function, strength, satisfaction, and complications. Of these patients, 72% were eval

PURPOSE: Dorsal subluxation of the base of the first metacarpal is a typical finding in trapezial-metacarpal (TM) osteoarthritis. The purpose of this study was to clarify the correlation between dorsal subluxation and articular tilt of the metacarpal facet in TM osteoarthritis.
METHODS: We investigated 50 subjects between 25 and 70 years of age (mean, 45 ± 11 y) with normal thumbs (100 hands) and 98 subjects between 43 and 89 years of age (mean, 61 ± 11 y) with TM osteoarthritis (132 hands). We established 3 groups: normal (normal thumbs), mild arthritis (40 hands, Eaton stage 1 or 2), and se

The authors report a series of six cases of acute traumatic dislocation of the trapeziometacarpal joint of the thumb. Displacement was always dorso-radial. There were associated osseous lesions in one patient: avulsion of the palmar tubercle of the base of the first metacarpal with a fracture of the trapezoid. All the patients were treated immediately by percutaneous pinning according to the technique of Wiggins. The late results, evaluated at an average of 19 months postinjury (range: 8-29 months), were considered to be satisfactory in the majority of the patients. Treatment by percutaneous p

Efficiency of surgical treatment in Kienböck's disease has never been proven in the long term. We retrospectively reviewed the charts of the 104 patients treated by various techniques for Kienböck's disease from 1981 to 1999 in our unit. A comparison was made between 19 cases treated conservatively (amongst 59) and 11 cases (amongst 25) treated by scaphotrapeziotrapezoid (STT) arthrodesis with a mean follow-up of 13 years. The two groups were statistically comparable in stage, age, sex ratio, number of manual workers. STT arthrodesis was responsible for an increased loss of mobility, an increa

PURPOSE: We aimed to review the effects of carpal tunnel ligament sectioning on the thumb carpometacarpal (CMC) joint with the hypothesis that carpal tunnel release (CTR) may result in biomechanical changes at the thumb CMC joint predisposing an individual to pain and or accelerated articular wear.
METHODS: This review was performed using the Pubmed and Medline databases from January 1, 1970 to January 7, 2023. Articles that examine the thumb CMC joint after CTR were categorized as cadaveric, clinical, or computational studies. The anatomy, pathophysiology, and treatment of both CMC arthritis

The carpal tunnel is a fibro-osseous canal at the wrist that is formed by the retinaculum and bony structures. The superficial border is composed of the flexor retinaculum, the floor of the tunnel is formed by the carpal bones, the medial (ulnar) border by the hamate, and the lateral (radial) border by the trapezium. Contents that originate in the forearm and run distally through the carpal tunnel include the finger and thumb flexors and the median nerve. Carpal tunnel syndrome, when the median nerve becomes compressed or entrapped, occurs due to various issues that will be discussed in this p

We dissected 393 wrists to evaluate the incidence and distribution of anatomic features, arthrosis, chondromalacia, and soft tissue lesions. The data were then analyzed for any statistically significant associations among the different variables. The most common (73%) lunate morphology had a separate medial facet on its distal surface for the hamate. The capitate had a separate facet for the fourth metacarpal in 86% of the wrists. Fourth metacarpals with a dorsal radial facet, either separate from or connected to the rest of the fourth metacarpal base, were the most common types of fourth meta

OBJECTIVES: Total joint replacement as treatment of osteoarthritis of the trapeziometacarpal joint may lead to excellent short-term results, but also with a high risk of failure of the trapezium component. The aim of this study was to compare revision using trapeziectomy with a revision into a cemented trapezium cup.
METHODS: Thirty-four patients with a mean follow-up time of 47 months were included in a case control study with 17 hands revised with a cemented cup and 21 trapeziectomy.
RESULTS: At follow-up, no significant difference was found in self-reported outcome, with pain at rest or a

BackgroundTrapeziometacarpal osteoarthritis (TMO) is a prevalent degenerative condition. While conservative treatments such as physiotherapy, drugs, and corticosteroid or hyaluronic acid injections offer symptomatic relief, their long-term efficacy remains debated. A recent study has explored collagen-based fillers as an alternative, but long-term clinical outcomes are still under investigation.MethodsThis study enrolled 64 patients diagnosed with TMO, stratified into 2 groups based on the Eaton-Littler classification (grade 1-2: group A; grade 3-4: group B). All patients received a percutaneo

Metal hypersensitivity is a rare and often underestimated complication, as shown by the small number of published studies, particularly concerning the upper limbs and the hand. However, the increase in the annual number of trapeziometacarpal arthroplasties underlines the importance of better understanding of this problem. We performed a study based on data from the revision of trapeziometacarpal prostheses to assess the incidence of this complication and to define a sequence of radiological changes to detect metal hypersensitivity. This single-center retrospective study included 37 patients op

BACKGROUND: Resection arthroplasty of the carpometacarpal joint of the thumb is considered to be the most frequently used surgical treatment for osteoarthritis of the trapeziometacarpal joint. Although simple trapeziectomy and fixation of the capsular tissue have been found to be an easy, successful treatment, the ligament reconstruction and tendon interposition in different techniques is still widely used. We evaluate the results of our patients after simple trapeziectomy.
PATIENTS AND METHODS: Thirty-four thumbs were treated by simple trapeziectomy and fixation of the dorsal capsular tissue

PURPOSE: Although recent studies have emphasized the importance of the adductor pollicis (AdP) and first dorsal interosseous (FDI) muscles in maintaining trapeziometacarpal (TMC) joint function, the viscoelastic properties of these muscles in the presence of TMC osteoarthritis (OA) remain unclear. This study aimed to investigate differences in the elasticity of the AdP and FDI muscles in patients with TMC OA, thereby contributing to the understanding of muscular changes associated with the disease.
METHODS: Eighteen patients with TMC OA (29 thumbs) and 13 age- and sex-matched asymptomatic con
