Do abused young children feel less pain?

The objective of this study was to investigate whether acute pain in abused children was under recognized by doctors and nurses compared to children evaluated for accidental injuries. We hypothesize that an abused child's reaction to physical pain could be an additional symptom of this challenging diagnosis. For the observational prospective case control study in an emergency department, children were eligible when: younger than six years old, the reported trauma occurred within the previous seven days, the trauma comprised a bone injury or burn, and the child was able to express his or her pa

Pediatric trauma: preparation and management.

1. Trauma causes more than 50% of the deaths in children. The leading cause of pediatric injury is motor vehicle accidents involving children as passengers, pedestrians, or bicycle riders. 2. Trauma is always unexpected; therefore, the most vital aspect of any trauma system is that the system itself never be caught unprepared. Anticipation and preparation are the best first-line defenses against traumatic injury. 3. Because of a child's smaller size, vital organs are in close proximity to one another; multiple organ injuries are common. 4. The team who responds to a pediatric trauma should inc

Emergency nurses' perceived knowledge and comfort levels regarding pediatric patients.

OBJECTIVE: To assess emergency nurses' levels of preparation and education regarding pediatric patients, their levels of knowledge and comfort with specific medical conditions of these patients, and the need they perceive for special policies for pediatric patients. METHODS: Surveys were mailed to nurse managers and emergency nurses in six diverse counties, including Los Angeles and vicinity. Fifty-one nurse managers and 362 emergency nurses from 51 hospitals responded to the questionnaires. RESULTS: More than half of the emergency nurses had 6 or more years of experience; many had certifica

Lost information during the handover of critically injured trauma patients: a mixed-methods study.

BACKGROUND: Clinical information may be lost during the transfer of critically injured trauma patients from the emergency department (ED) to the intensive care unit (ICU). The aim of this study was to investigate the causes and frequency of information discrepancies with handover and to explore solutions to improving information transfer. METHODS: A mixed-methods research approach was used at our level I trauma centre. Information discrepancies between the ED and the ICU were measured using chart audits. Descriptive, parametric and non-parametric statistics were applied, as appropriate. Six f

Autonomic dysreflexia--a nurse's challenge.

Autonomic Dysreflexia (AD) is a potentially life-threatening complication for spinal cord injured individuals. It is one of the most serious emergencies that can develop post trauma. AD provides a challenge to nurses. A basic understanding of the patho-physiology involved in AD is recommended and encouraged for those people who care for the spinal cord injured. The purpose of this paper is to provide a tool for nurses to identify, understand and manage an AD crisis using a step-by-step approach.

Multidisciplinary Perioperative Care of a One-Month-Old Infant With Severe Traumatic Brain Injury and Brain Herniation: A Case Report.

Severe traumatic brain injury (sTBI) is a major cause of trauma-related mortality and disability in children. Infants face unique challenges due to physiological immaturity and scarce evidence-based protocols. We report the perioperative nursing management of a one-month-old infant with sTBI complicated by brain herniation, guided by a multidisciplinary team (MDT) through a structured care pathway adapted from the Third Edition of the Guidelines for the Management of Pediatric Severe Traumatic Brain Injury with infant-specific modifications. The patient presented with respiratory and circulato

Posttraumatic stress disorder symptoms and food addiction in women by timing and type of trauma exposure.

IMPORTANCE: Posttraumatic stress disorder (PTSD) appears to increase obesity risk but the pathways by which PTSD leads to weight gain are not known. Identification of the links between PTSD and obesogenic eating behaviors is necessary to clarify this pathway and inform development of obesity prevention strategies in PTSD-affected populations. OBJECTIVE: To determine whether women with PTSD symptoms are more likely to report food addiction, a measure of perceived dependence on food, than women without PTSD symptoms. Also, to determine whether age at PTSD symptom onset and type of trauma influe

Allen's test: a tool for diagnosing ulnar artery trauma.

Trauma to the ulnar artery was first reported by Von Rosen in 1934. In more recent years this condition has been termed ulnar-artery thrombosis, hypothenar hammer syndrome or post-traumatic digital ischemia. Trauma to the ulnar artery is caused by the repeated use of the hand as a hammer in order to apply force to an object. Any person who presents with a history of trauma to the hypothenar eminence of the hand should be screened for ulnar artery thrombosis. This screening can easily and quickly be performed by the nurse utilizing the Allen test. Early detection of vascular insufficiency coupl

The Role of Nurses as Allies Against Racism and Discrimination: An Analysis of Key Resistance Movements of Our Time.

The remnants of colonialism manifesting as structural violence, racism, and oppression continue to plague our society as evidenced by the persistence of health inequities, particularly for minority populations in the United States. As a profession bound by moral and ethical mandates, nursing must resist and deconstruct oppression in all its forms. Nurses, informed by critical race theory, intersectionality, and historical trauma, can become formidable allies with marginalized populations in the fight for social justice and health equity.

Thyroid storm during pregnancy. A medical emergency.

Thyroid storm is the major risk to pregnant women with thyrotoxicosis. This life-threatening condition is more likely to occur with another precipitating factor such as labor and delivery, surgical delivery, infection, or trauma. Thyroid storm most often occurs in patients with undertreated or undiagnosed hyperthyroidism. As many as 20% to 30% of cases can end in maternal and fetal mortality. Therefore, critical care nurses must be able to recognize and initiate proper medical and nursing interventions promptly.

Thoracic trauma.

Thoracic trauma causes one of every four trauma deaths in this country, and many of these deaths could have been prevented with prompt diagnosis and treatment. Injuries to the thorax range from a superficial wound such as contusions and abrasions to life-threatening tension pneumothorax. Being aware of the mechanism of injury and pathophysiology of the chest cavity will alert the nurse to suspected injuries. A brief description of common injuries is discussed in this article.

Reflections by licensed practical nurses and nursing assistants on the violent behaviours of individuals receiving mental health care in an adult acute psychiatric ward: a qualitative study.

BACKGROUND: Violence in adult acute psychiatric wards remains a significant occupational hazard for nursing staff, particularly Enrolled Nurses (equivalent to Licensed Practical Nurses) and nursing assistants who often provide direct patient care without specialized psychiatric training. Exposure to patient-perpetrated violence has serious emotional and professional consequences. This study explored and described the reflections of Licensed Practical Nurses and Nursing Assistants on the violent behaviours of individuals receiving mental health care in adult acute psychiatric wards. METHODS: A

Traumatic Injury of Major Cerebral Venous Sinuses Associated with Traumatic Brain Injury or Head and Neck Trauma: Analysis of National Trauma Data Bank.

BACKGROUND: The natural history and epidemiological aspects of traumatic injury of major cerebral venous sinuses are not fully understood. We determined the prevalence of traumatic injury of major cerebral venous sinuses and impact on the outcome of patients with traumatic brain injury, and/or head and neck trauma. METHODS: All the patients who were admitted with traumatic brain injury or head and neck trauma were identified by ICD-9-CM codes from the National Trauma Data Bank (NTDB), using data files from 2009 to 2010. NTDB represents one of the largest trauma databases and contains data fro

Metabolic complications associated with severe diffuse brain injury.

Diffuse axonal injury (DAI), the severest form of diffuse brain injury, causes extensive damage throughout the cerebrum, diencephalon and brainstem. The effects of this injury, however, are not isolated to the brain. This paper discusses diffuse axonal injury, the metabolic complications to be anticipated from the systems' response to cerebral trauma in general, and the nursing implications.

An Integrative Model of In-Hospital and Out-of-Hospital Nursing Care for Non-Suicidal Self-Injury: A Narrative Review.

BACKGROUND: Non-suicidal self-injury (NSSI) refers to the intentional and repeated physical trauma of an individual without explicit suicidal intent, which has negative effects on the physical and mental well-being of an individual, especially for adolescents. Timely and accessible nursing care may play an important role in the survival and rehabilitation process of NSSI. METHODS: In this review, we systematically discuss the nursing care of NSSI behavior and provide recommendations based on an integrated nursing model for NSSI management. RESULTS: As reported in previous studies, a variety

Caring for patients with limb amputation.

This article provides an overview of the care of patients undergoing limb amputation. Absence of a limb can be congenital or the result of trauma or complications of chronic diseases. While the economic burden of limb amputation is significant, nurses have an important role in limiting other losses attributable to limb loss, such as long-term disability leading to loss of employment and delayed return to work or school. Comprehensive nursing assessments and appropriate interventions, pre and post-operatively, as well as early discharge planning and community reintegration can help avoid some o

Who keeps watch? Structural safeguarding against epistemic erosion in integrated nursing curricula.

AIM: To examine how integrated nursing curricula protect vulnerable knowledge domains once those domains are distributed across a program and to propose a conceptual framework for understanding and responding to this challenge. BACKGROUND: Integrated curriculum design is widely valued in nursing education for its capacity to reduce fragmentation and support learning across complex practice contexts. However, integration alone does not protect essential knowledge domains from dilution, containment, or invisibility over time. DESIGN: Discussion paper. METHODS: The paper undertakes a critical
