HER2 intratumoral heterogeneity predicts response to neoadjuvant therapy in HER2-positive breast cancer: impact and interplay with HER3 expression.

Intratumoral heterogeneity (ITH) of HER2 expression and HER3 upregulation have been associated with resistance to HER2-targeted therapies. However, their predictive role in the neoadjuvant setting remains controversial. We retrospectively analyzed 59 patients with HER2-positive invasive breast carcinoma treated with neoadjuvant chemotherapy and anti-HER2 agents at the Agostino Gemelli University Hospital (2018-2020). HER2 ITH was assessed on pre-treatment biopsies and residual tumors (when applicable) by immunohistochemistry and SISH. HER3 expression was also evaluated using IHC and categorize

Contributors to the heterogeneity of response to CFTR modulators.

Cystic fibrosis transmembrane conductance regulator (CFTR) modulators have transformed the management of cystic fibrosis; however, substantial interindividual variability in clinical response persists, even among people with identical CFTR genotypes. This variability challenges clinicians, regulators, and researchers in defining treatment success and optimising long-term outcomes. We synthesise evidence from clinical trials, real-world cohorts, and mechanistic studies to examine the contributors to heterogeneity in response to CFTR modulators, with a focus on triple therapy (elexacaftor-tezaca

Symptom profiles at baseline are associated with remission in major depression: a confirmatory analysis of the PANDORA trial.

OBJECTIVE: To test whether baseline depressive symptom profiles are associated with remission in Major Depressive Disorder (MDD) and to replicate previous findings from GSRD and STAR∗D in the PANDORA trial. METHODS: We performed a secondary analysis of 215 adults with MDD enrolled in PANDORA and starting a new antidepressant. Baseline severity was assessed with the 17-item Hamilton Depression Rating Scale (HAM-D). Remission at week 12 was defined as HAM-D ≤ 7. Logistic regression models examined associations between baseline HAM-D total and item scores and remission overall and by sex, adjust

Identifying treatment response classes to transcranial direct current stimulation from daily ecological momentary assessment patterns in patients with depression.

IMPORTANCE: Transcranial direct current stimulation (tDCS) is known to be promising for depression, but heterogeneity across studies highlights the need for strategies to optimize treatment effectiveness. Identifying distinct response patterns based on ecological momentary assessment (EMA) may enhance therapeutic outcomes and support the development of personalized and precision psychiatry. OBJECTIVE: To identify distinct EMA-derived response profiles to tDCS in patients with depression and examine how the identified subtypes differentially predict treatment responses and symptom return follo

Functional connectivity during positive mood and anxiety treatment response in children.

Self-reported low positive mood is common in youth anxiety disorders and has been associated with poorer treatment response. Functioning of neural affective systems may have distinct associations with treatment relative to self-reported measures and be useful for understanding biological processes related to treatment. This study examined associations between self-reported mood and pre-treatment functional connectivity network patterns during positive mood and treatment response in 73 youth (mean age = 11.1) diagnosed with anxiety disorders. Participants engaged in an fMRI autobiographical mem

Responder discordance between migraine frequency and patient-reported outcomes after CGRP-pathway monoclonal antibody treatment: a secondary analysis of public patient-level data.

BACKGROUND: Monoclonal antibodies targeting the calcitonin gene-related peptide (CGRP) pathway are established preventive treatments for migraine. Although treatment response is often summarized by reduction in monthly migraine days (MMD), patients and clinicians also judge benefit by changes in disability and headache impact. We examined how often frequency-based and patient-reported outcome measure (PROM) responder definitions classify the same individuals after CGRP-pathway monoclonal antibody treatment and characterized the direction of discordant response patterns. METHODS: We performed

Differential effects of transcranial direct current stimulation on depression symptom clusters.

Depression is a heterogeneous condition. While transcranial direct current stimulation (tDCS) shows promise, its effects remain inconsistent. Understanding factors underlying this variability could help improve outcomes. Therefore, this study aimed to investigate the differential effects of tDCS versus sham across distinct symptom clusters of depression, using an agglomerative hierarchical clustering approach. Data from a previous IPD meta-analysis of randomized controlled trials (RCTs) involving patients with acute depression who received at least five sessions of tDCS were used. Only RCTs th

Organ Involvement and Treatment Response across Cutaneous Sarcoidosis Subtypes.

Cutaneous sarcoidosis presents with diverse morphologies, yet comparative data on systemic involvement, disease course and treatment response across subtypes remain limited. We conducted a 2-centre cross-sectional study including patients with histologically confirmed sarcoidosis identified through ICD codes, stratified into patients with and without cutaneous involvement and further by cutaneous subtype. Demographics, extracutaneous organ involvement, treatment response, clinical course and the organ leading to initial diagnosis were analysed. Among 125 patients (median age 53 years; 57% fema

Sex differences in efficacy and toxicity of first-line treatment of metastatic colorectal cancer: An analysis of 18,041 patients in the ARCAD CRC database.

BACKGROUND: Greater toxicity of adjuvant chemotherapy in females has been reported previously, but the impact of patients' sex on chemotherapy (CT)+/-targeted therapies for non-resectable, metastatic colorectal cancer (mCRC) has remained unclear. We compared efficacy and toxicity of first-line chemotherapy, with or without targeted therapies, between male and female mCRC patients. METHODS: Twenty-seven randomized trials from the ARCAD CRC database were included. Primary endpoint was overall survival (OS); secondary endpoints were progression-free survival (PFS), overall response rate, toxicit

Mixed Response to Nivolumab Among Different Metastatic Sites in Patients With Metastatic Renal Cell Carcinoma: Incidence and Clinical Implications.

OBJECTIVE: Discordant responses to immune checkpoint inhibition (ICIs) may occur at different sites in metastatic renal cell carcinoma (mRCC). This study investigated the incidence and prognostic significance of mixed response in mRCC patients treated with nivolumab. METHODS: We retrospectively analyzed 116 mRCC patients who underwent nivolumab monotherapy without prior ICIs. The best overall response (BOR) was evaluated for each patient using RECIST version 1.1, and organ-specific responses were assessed for each metastatic site. Patients were classified into three groups: responders (lesion

Predictive value of the heterogeneity index in assessing treatment response using 18F-FDG PET/CT in locally advanced and advanced lung cancers.

OBJECTIVE: Intratumoral metabolic heterogeneity may influence treatment response in lung cancer, and the heterogeneity index (HI), derived from threshold-based metabolic tumor volume (MTV) measurements, has been proposed as a simple fluorine-18-fluorodeoxyglucose (18F-FDG) positron emission tomography/computed tomography (PET/CT)-based parameter to estimate this variability. This retrospective single-center study evaluated the predictive value of HI for treatment response in patients with locally advanced and advanced lung cancer undergoing chemoradiotherapy and compared its performance with c

Racial outcomes in patients with diabetic cardiomyopathy treated with an aldose reductase inhibitor: the ARISE-HF trial.

BACKGROUND: Racial and ethnic differences in diabetic cardiomyopathy (DbCM) exist, with black and Hispanic participants showing poorer health status. It remains unclear whether these differences affect the natural progression of the disease. We aimed to evaluate disease progression in individuals with DbCM, as well as racial differences in the response to AT-001. METHODS: A total of 625 participants with DbCM were randomised to either placebo or AT-001 and followed for 15 months. The primary outcome was change in peak oxygen uptake (peak VO2) and secondary outcomes included Kansas City Cardio

Electrographic flow (EGF) mapping reveals sex-based differences in EGF patterns with women concentrated in phenotypes that benefit from EGF-guided ablation.

BACKGROUND: Electrographic flow (EGF) mapping allows for the near real-time visualization of atrial wavefront propagation. Two factors have been found to be associated with higher rates of atrial fibrillation (AF) recurrence: presence of active extra-pulmonary vein (PV) sources and low electrographic flow consistency (EGFC) representing chaotic flow and abnormal atrial substrate. Based on these characteristics, EGF phenotypes have been identified: Type I patients have no sources + high EGFC, Type II have sources + high EGFC, Type III have sources + low EGFC, and Type IV have no sources + low E

A Longitudinal Study on Predicting Treatment Response in Depression Using Self-Reported Symptoms.

BACKGROUND: Depression causes serious disabilities, and response to treatment is difficult to predict. This study aimed to examine whether self-reported initial symptoms can predict treatment response in depression. METHODS: One hundred fifty-seven patients who were diagnosed with mood disorders (major depressive disorder or bipolar disorder) and in a state of moderate or severe depression were recruited from six psychiatric hospitals between 2018 and 2022. Participants were classified into treatment responders (n = 89) and non-responders (n = 68) based on the Montgomery-Åsberg Depression Rat

scRADAR: Dissecting intratumoral drug response heterogeneity at single-cell resolution via mechanism-guided prototype routing.

Precision oncology requires resolving intratumoral heterogeneity to identify drug-resistant cell states associated with treatment failure and relapse. Although single-cell RNA sequencing enables characterization of heterogeneous resistance-associated states, single-cell drug-response phenotype prediction remains challenging because of sparsity, noise, class imbalance, and limited mechanistic interpretability. Here, we present scRADAR (Response Analysis via Drug-Aware Routing), a mechanism-guided prototype routing framework for predicting and interpreting drug-response phenotypes at single-cell

Effects of pulsed radiofrequency combined with ozone on zoster-associated pain: a systematic review and meta-analysis.

Pulsed radiofrequency combined with ozone therapy has been a newly proposed treatment method for zoster-associated pain in recent years. To explore the effectiveness of a regimen of pulsed radiofrequency combined with ozone for the treatment of zoster-associated pain. We searched commonly used English databases (the Cochrane Library, PubMed, CINAHL, Embase, and Web of Science) and Chinese databases (the CNKI, WanFang, CBM, and VIP databases) for randomized controlled trials. Two researchers independently screened the literature on the basis of the set conditions and extracted data from the inc

CFTR Functional Rescue and Heterogeneous Clinical Outcomes Following Elexacaftor/Tezacaftor/Ivacaftor Therapy in People with Cystic Fibrosis Carrying the W1282R Variant.

The clinical response to elexacaftor/tezacaftor/ivacaftor (ETI) in people with cystic fibrosis carrying the rare cystic fibrosis transmembrane conductance regulator (CFTR) variant W1282R has not been well characterized. We retrospectively analyzed 12 people with cystic fibrosis treated with ETI for 3-36 months in addition to ongoing standard CF therapy. Baseline lung function was generally preserved in patients with available spirometry (median percent predicted forced expiratory volume in 1 s (ppFEV1), 110%; interquartile range (IQR), 93-123), limiting the potential for further improvement in
