Trichophyton indotineae, a new species of dermatophytes, has become a significant concern in treating dermatophytosis due to the high level of terbinafine resistance reported in this organism. This is the first report of Trichophyton indotineae infection in Central Vietnam. Antifungal susceptibility testing showed that this isolate was susceptible to itraconazole, voriconazole, and terbinafine. Therefore, the patient was successfully treated with oral itraconazole and ketoconazole topical cream.

Tinea corporis and congenital ichthyoses are common dermatological diseases. The association of the two disorders is plausible due to the immunological and barrier defects of ichthyoses; however, relatively limited literature is available in this field. Since superficial fungal infections possess atypical morphology in keratinization disorders, and could imitate other dermatological conditions, the correct diagnosis can be challenging. We present the case of a 54-year-old woman with ichthyosis, who was initially treated for drug-hypersensitivity reaction.

A 26-year-old female homemaker presented with an approximately 2-month history of an erythematous lesion with agminated seropapules in the right popliteal fossa associated with scales and crusts. The lesion was initially treated as contact dermatitis, but there was no improvement. KOH examination revealed filamentous fungi. A 25-year-old man was examined for tinea corporis. Microscopic examination of direct 10% KOH mount of skin scraping revealed the presence of hyaline, septate, branching hyphae. Trichophyton verrucosum infection, which is acquired from the hide of dairy cattle, is the cause of tinea barbae, a severe pustular eruption on the face and neck. We present the cases of five patients, three of whom had severe pustular tinea barbae and two of whom had eruptions of the forearms. All five patients were dairy farmers. An 18-year-old male agricultural worker with recessive X-linked ichthyosis was found to have ringworm infection with Trichophyton verrucosum, which resulted in well-demarcated clearing of the ichthyotic scaling in affected areas. However, rapid recurrence of the abnormal cornification was observed after treatment of the fungal infection. Some possible mechanisms regarding the effect of fungi on hyperkeratinization are discussed.
