BACKGROUND: Scrub typhus (ST) is a life-threatening infectious disease caused by Orientia tsutsugamushi. Early prediction of whether the disease will progress to a severe state is crucial for clinicians to provide targeted medical care in advance.
METHODS: This study retrospectively collected severe and mild ST cases in two hospitals in Fujian Province, China from 2011 to 2022. Eighteen objective clinical and laboratory features collected at admission were screened using various feature selection algorithms, and used to construct models based on six machine learning algorithms.
RESULTS: The …

BACKGROUND: Scrub typhus is an acute febrile illness caused by the rickettsial bacterium Orientia tsutsugamushi, transmitted to humans through infected mite bites. Patients typically present with sudden onset high fever, headache, myalgia, and eschar formation at the site of the bites. Respiratory symptoms, such as cough and shortness of breath, can also occur, especially in severe cases that lead to pneumonia or acute respiratory distress syndrome (ARDS). This report presents a case of scrub typhus pneumonia, diagnosed by metagenomic next-generation sequencing (mNGS) on bronchoalveolar lavage…

Scrub typhus is a tick-borne disease caused by the intracellular organism Orientia tsutsugamushi. It typically presents with the cardinal "triad" of fever, rash, and eschar, along with other nonspecific symptoms. We report a case of scrub typhus in a 74-year-old man who did not exhibit the typical rash. He presented to the emergency room with a one-week history of generalized symptoms, including fever, throat pain, and myalgia, and was admitted due to suspected cholangitis based on elevated liver enzymes. However, computed tomography (CT) and magnetic resonance cholangiopancreatography (MRCP) …

This case series highlights four pediatric patients with coinfection of dengue fever and scrub typhus, presenting at Patan Hospital. Dengue fever is caused by flavivirus transmitted by Aedes mosquitoes, and scrub typhus is caused by Orientia tsutsugamushi transmitted by Leptotrombidium mites, which share overlapping symptoms such as fever, headache, and abdominal pain, complicating the diagnosis. The cases were from different geographical regions of Nepal with varied presentation: from febrile illness to severe manifestations like meningoencephalitis and raised intracranial pressure. Two patie…

Polymyositis (PM), a rare autoimmune inflammatory myopathy, typically presents with symmetric proximal muscle weakness and elevated muscle enzymes. A muscle biopsy is confirmatory for the diagnosis. We report a case of a woman in her 30s with scrub typhus and anti-nuclear matrix protein 2 (NXP2) positive PM. The patient presented with fever, proximal muscle weakness, dysphagia and serologically confirmed scrub typhus a month prior. Laboratory investigations revealed a positive ELISA IgM for scrub typhus, elevated creatine kinase and elevated transaminases, leading to a provisional diagnosis of

BACKGROUND: Scrub typhus and other doxycycline-treatable infections are significant contributors of acute encephalitis syndrome (AES) in India. Limited surveillance in South India has hindered their recognition and the inclusion of doxycycline in treatment protocols. We aimed to systematically investigate infectious aetiologies of AES in children from Karnataka, India, and develop clinical prediction models for diagnosing scrub typhus and guiding clinical decisions for doxycycline therapy.
METHODS: This multicentre, prospective study enrolled children aged >28 days to 18 years with AES presen

Acute pancreatitis is an exceptionally rare complication of scrub typhus. We report 2 cases of scrub typhus-associated acute pancreatitis from a tertiary care centre in Kolkata, India. The first case was a 27-year-old male patient, who presented in 2023, with severe epigastric pain a few days after the onset of fever. Laboratory investigations confirmed acute pancreatitis associated with scrub typhus. He had no additional organ involvement and recovered fully with intravenous doxycycline. The second case, reported in 2024, was a 42-year-old female patient who presented with fever, abdominal pa

Scrub typhus (ST), caused by Orientia tsutsugamushi, is typically transmitted through mite bites, but vertical transmission from mother to infant remains poorly understood. This case presents a 13-day-old female neonate diagnosed with congenital ST. The infant developed severe systemic involvement, including fever, jaundice, respiratory distress, and signs of hemophagocytic lymphohistiocytosis (HLH). Despite broad-spectrum antibiotic therapy, her condition worsened. PCR and serological tests confirmed the diagnosis of ST in both the neonate and the mother, with IgM positivity indicating congen…

Scrub typhus is increasingly recognised as a common cause of acute febrile illness in South and Southeast Asia. A lack of clinical suspicion and delayed diagnosis contribute to a mortality rate of ∼10%. A clinical diagnosis based on seasonal epidemiology, presence of eschar or rash, thrombocytopenia, hepatic or splenic involvement, and multiorgan dysfunction warrants prompt empirical treatment with doxycycline or azithromycin. IgM enzyme-linked immunosorbent assay remains the diagnostic mainstay. Treatment is stratified by severity; dual therapy is recommended in severe, confirmed cases. Most

BACKGROUND: Scrub typhus, once known as tsutsugamushi fever and attributed to Rickettsia, has transformed into a growing public health concern. Despite its increasing incidence in India, comprehensive spatiotemporal analyses of scrub typhus have been lacking.
METHODS: This study examines the prevalence of scrub typhus cases reported from 2003 to 2023, using available literature to provide a breakdown of cases by year and state in India, aiming to elucidate the disease's spatiotemporal dynamics. The aetiological association of Orientia tsutsugamushi and scrub typhus is based on geographical, i…

BACKGROUND: Acute encephalitis syndrome (AES) has high morbidity and mortality in children. Empirical treatment of AES often consists of third-generation cephalosporins, with vancomycin and acyclovir, and frequently excludes the use of azithromycin, targeted at scrub typhus. In a resource-constrained setting, testing for scrub typhus becomes challenging. Considering the lacunae in the existing literature, there is a need for robust evidence to determine the role of additional azithromycin use in children with AES. Objective: To evaluate the efficacy and safety of adjunctive intravenous azithr
