[A child case of Down's syndrome with intestinal tuberculosis and tuberculous pleuritis].

A 13 year-old girl with Down's syndrome was admitted to our hospital with a very positive reaction to PPD 5TU, abnormal shadow on the chest X-ray films and diarrhea. She suffered from acute enterocolitis one year ago, and then has been complaining of abdominal pain, appetite loss, and weight loss for a year. After admission, she was diagnosed as tuberculous pleuritis and suspected intestinal tuberculosis by laboratory examination. She recovered without sequelae by the combination therapy of SM, INH, and RFP, and was discharged after 5 months. The diagnosis of intestinal tuberculosis was confir

The risk of tuberculosis in patients with psoriasis treated with anti-tumor necrosis factor agents.

BACKGROUND: Tumor necrosis factor-alpha (TNF-α) antagonist treatment is associated with 1.6 to 27 times higher risk of tuberculosis (TB). OBJECTIVE: To find TB incidence of psoriasis patients treated with TNF- α antagonists and define risk factors related with this condition in a country with moderately high risk of TB. METHODS: Three hundred seventy psoriasis patients treated by anti-TNF agents in four referral centers were included. The data on the characteristics of the patients, TB history, tuberculosis skin test results, anti-TNF agent type and exposure time, localization of TB, and iso

Perforated acute appendicitis on computed tomography revealing underlying tuberculosis.

Tuberculous appendicitis is an extremely rare manifestation of gastrointestinal tuberculosis and is often clinically and radiologically indistinguishable from conventional acute appendicitis. We report the case of a 27-year-old man who presented with fever and pain in the right lower quadrant. Contrast-enhanced computed tomography revealed an enlarged appendix with an appendicolith, a distal wall defect, and periappendiceal abscess formation, consistent with perforated acute appendicitis. Laparoscopic appendectomy was performed. Histopathological examination revealed acute gangrenous appendici

[Revealing of tuberculosis in an infectious diseases hospital of a megalopolis].

The advisory work of the phthisiatrician in an infectious diseases hospital was analyzed; the analysis revealed that in 2005 tuberculose changes of various degrees of activity had been revealed in 42.5% of examined patients, and 32.1% of them were subjects in whom tuberculosis of diferent localizations had been revealed for the first time. In 43.2% of the latter subjects, the reasons for hospitalization were "clinical masks" of tuberculose process (influenza, acute respiratory viral disease), while 48.6% were hospitalized for gastrointestinal infections and viral hepatitis. In 20.7% of cases t

Tuberculosis in end-stage renal disease patients on hemodialysis.

Recent studies have shown that there is an increase in the incidence of mycobacterium tuberculosis (MBT). This is more prevalent among immune compromised patients (those on dialysis) and recipients of organ transplants. Furthermore, extra-pulmonary presentation appears to be more common and difficult to diagnose. We aimed in this study to assess and evaluate the presentation of MBT in a retrospective study conducted among 256 hemodialysis (HD) patients where 18 of them were diagnosed and managed for tuberculosis over a 10-year period between 1990 and 2000. The mean age of the patients was 38 y

Tuberculosis screening in adult patients with inborn errors of immunity: a single-center cohort study.

BACKGROUND: Data on tuberculosis, a leading cause of infectious disease mortality, in patients with inborn errors of immunity (IEI) are limited. OBJECTIVE: This study aimed to evaluate the burden of tuberculosis and the diagnostic utility of the tuberculin skin test (TST) in patients with IEI and to propose a tuberculosis screening strategy. METHODS: Evaluation comprised a comprehensive exposure history, symptom assessment, TST (which was repeated after 1-4 weeks if the initial result was 0-4 mm), and chest imaging including radiography and computed tomography where available. Patients under

Jejunal multiple perforations for combined abdominal typhoid fever and miliary peritoneal tuberculosis.

Typhoid fever and tuberculosis, considered rare diseases in western countries, is still considered a notable problem of health issue in developing countries. The gastrointestinal manifestations of typhoid fever are the most common and the typhoid intestinal perforation (TIP) is considered the most dangerous complication. Abdominal localization of tuberculosis is the 6th most frequent site for extra pulmonary involvement, it can involve any part of the digestive system, including peritoneum, causing miliary peritoneal tuberculosis (MPT). This is the case report of a 4 years old girl with multip

Gastrointestinal tract in the immunocompromised host: opportunistic infections and other complications.

In the decade since the early 1980s, the increasing use of immunosuppressive therapy for cancer and autoimmune disease, as well as for organ transplantation, has combined with the acquired immunodeficiency syndrome epidemic to increase greatly the incidence of opportunistic infections and other complications of the gastrointestinal tract. Consequently, barium fluoroscopic and cross-sectional imaging studies tailored to address these problems are no longer uncommon. Although overlap exists, there are radiographic patterns that can direct the diagnosis to an opportunistic infection and sometimes

Duodenal tuberculosis; uncommon cause of gastric outlet obstruction.

Gastrointestinal tuberculosis manifesting as isolated duodenal involvement is uncommon. We present a case of 52 years old man with post-prandial abdominal pain, early satiety and weight loss. Upper endoscopy showed circumferential duodenal stricture adjacent to the ampulla with nodularity, erythema, and ulcerations of the overlying mucosa causing gastric outlet obstruction. Biopsy of duodenal stricture revealed chronic duodenitis and the patient was treated with acid suppression therapy. Endoscopic balloon dilation was also performed with no response. Abdominal computer tomography scan showed

[Esophageal tuberculosis: case report and review of the literature].

The esophageal tuberculosis is the rarest form of this infection in the gastrointestinal tract, corresponding to 0.15% of the cases. This pathology is unusual even in countries with high prevalence of tuberculosis. Its clinical presentation could be easily confused with the one of esophageal carcinoma. The diagnosis is reached by demonstrating in a sample of the mucosa the presence of caseating granulomas or by finding the Mycobacterium tuberculosis in a sample of tissue. In the present case report 73-year-old male with dysphagia, weight loss and fever is presented. At the endoscopy an ulcerat

A clinico-radiological reappraisal of intestinal tuberculosis--changing profile?

Intestinal tuberculosis is still common in developing countries. In 186 patients with intestinal tuberculosis, clinical features, radiological findings and complications were carefully recorded and compared with those from earlier studies with a view to study any possible changes after the liberal use of antitubercular drugs. Sixty two percent of the patients in the present series had had prior exposure to antitubercular drugs. The incidence of systemic symptoms like fever and anorexia, alternating diarrhoea and constipation, peritoneal and lymph node involvements and associated pulmonary lesi

Cancer incidence attributable to tuberculosis in 2015: global, regional, and national estimates.

BackgroundTuberculosis is associated with increased risk of cancer. However, the impact of tuberculosis on global cancer burden is unknown.MethodsWe performed random-effects meta-analyses and meta-regressions of studies reporting the association between tuberculosis and cancer risks by searching PubMed, Web of Science, Embase, Cochrane library, and CINAHL from inception to 1 June 2019. Population attributable fractions (PAFs) of cancer incidence attributable to tuberculosis were calculated using relative risks from our meta-analyses and tuberculosis prevalence data from Global Health Data Exch

Anal fistula: a review of 82 cases.

Eighty-two cases of anal fistula have been retrospectively reviewed, and their clinical presentations, pathology, and management presented. Most of the fistulas were primary and, except for the four cases due to tuberculosis, there does not seem to have been any underlying systemic or gastrointestinal causative factor. A preoperative fistulogram is essential in the management of this condition.

The diagnostic challenges in a child with intestinal tuberculosis.

INTRODUCTION: Romania is one of the European countries with a significant burden of tuberculosis (TB). Although pulmonary TB is still highly prevalent, intestinal TB is very rare and remains a diagnosis of exclusion, especially in children. The authors aimed to raise the awareness on this pathology by discussing the challenges faced in the management of one difficult case. CASE PRESENTATION: A 3-year-old boy was hospitalized in the Pediatrics Department of Grigore Alexandrescu Emergency Children's Hospital, Bucharest, Romania, for abdominal pain and melena. On clinical examination, he was mal

[Symptomatic diseases in pneumology].

The diagnosis and treatment of granulomatous lung diseases is challenging. This article describes two of these entities: (1) eosinophilic vasculitis with polyangiitis which is clinically characterized as a combination of bronchial asthma and eosinophilic granulomatous vasculitis. Antoneutrophil cytoplasmic antibodies are present in approximately 40 % of patients. Treatment with steroids is sufficient in patients with isolated pulmonary manifestation but extrapulmonary manifestations, e.g. heart, central nervous system (CNS), kidneys and gastrointestinal tract warrant combined immunosuppression

Asymptomatic gastric tuberculosis in the gastric body mimicking an isolated microscopic erosion: A rare case report.

INTRODUCTION: Gastric tuberculosis is rarely seen in clinical practice, which occurs mostly secondary to lung tuberculosis, intestinal tuberculosis, and other common tuberculosis. Gastric tuberculosis rarely presents as a single microscopic superficial erosion. We recently diagnosed such a case, hence reporting it herein. PATIENT CONCERNS: A 40-year-old female patient was admitted with a chief complaint of painful enlarged cervical lymph nodes. She had no other symptoms or any previous history of remarkable diseases. DIAGNOSIS: Physical examination found multiple enlarged cervical lymph node
