Flushing and pruritus secondary to prescription fish oil ingestion in a patient with allergy to fish.

BACKGROUND: A brand of fish oil capsules contains omega-3 fatty acids obtained from several fish sources. Although the manufacturer calls for caution in patients with fish hypersensitivity, insufficient data is available to make a definitive recommendation regarding its use in this population. CASE PRESENTATION: A patient with documented seafood allergy presented to the emergency department 4 days after the initiation of prescription brand name fish oil capsules complaining of chest tightness, shortness of breath, tingling of upper extremities, flushing, and pruritus that was minimally reliev

Heavy metals in commercial fish in New Jersey.

Levels of contaminants in fish are of particular interest because of the potential risk to humans who consume them. While attention has focu … few significant correlations (Kendall tau) among metals for the three most numerous fish (yellowfin tuna, bluefish, and flounder), the correlations were generally low (below 0.40), and many correlations were negative. Only manganese and lead positively were correlated for tuna, bluefish, and flounder. The levels of most metals were below those known to …

Management of an outbreak of botulism with benign clinical presentation.

Botulism is a life-threatening presynaptic disorder of the neuromuscular transmission produced by the neurotoxin elaborated by the botulinum neurotoxin-producing clostridia. We describe the management of a case series of 14 patients, members of 5 different families that were exposed to home-canned tuna and developed symptoms compatible with a mild clinical presentation of foodborne botulism. The electrophysiological study of the index case represented a reliable diagnostic test as it demonstrated a slight presynaptic dysfunction of the neuromuscular junction. Definite diagnosis was later confi…

Foreign body ingestions in a schizophrenic patient.

The topic of foreign body ingestion has received extensive coverage in the areas of surgery, emergency medicine and pediatrics. A subset of this topic, the intentional ingestion of foreign bodies, however, is much less common, and involves subtleties in evaluation and management not usually seen in accidental ingestions. Here, we report a case of ingestion of a rolled, metal tuna can lid in a male prison inmate previously diagnosed with depression and paranoid schizophrenia. Following evaluation by the surgical team, the foreign body was removed by laparotomy and the patient was discharged bac…

Development of new heparin-like compounds and other antithrombotic drugs and their interaction with vascular endothelial cells.

The anticlotting and antithrombotic activities of heparin, heparan sulfate, low molecular weight heparins, heparin and heparin-like compounds from various sources used in clinical practice or under development are briefly reviewed. Heparin isolated from shrimp mimics the pharmacological activities of low molecular weight heparins. A heparan sulfate from Artemia franciscana and a dermatan sulfate from tuna fish show a potent heparin cofactor II activity. A heparan sulfate derived from bovine pancreas has a potent antithrombotic activity in an arterial and venous thrombosis model with a negligib…

Ciguatera fish poisoning: an outbreak associated with fish caught from North Carolina coastal waters.

Ten persons who had eaten at a seafood meal in North Carolina had gastrointestinal and neurologic symptoms suggestive of ciguatera fish poisoning. In five persons, the neurologic morbidity lasted 30 days or longer. The meal included barracuda, dolphin fish (mahimahi), and yellow-fin tuna, all of which were caught in North Carolina coastal waters. Analysis of food-specific attack rates implicated the barracuda as the probable cause of the outbreak. We believe this is the first suspected or confirmed report of ciguatera fish poisoning associated with consumption of fish harvested from mainland U…

Levels of mercury in canned tuna, swordfish, and shark were based on FDA survey data. Scombroid and Anaphylaxis: Different Diseases Despite Featural Similarities.

Food toxicity is a common cause of morbidity worldwide. Presentations of food toxicity can take diverse forms, depending on the underlying mechanisms. Scombroid is one of the most frequent forms of food toxicity; however, its exact incidence remains unknown owing to similarities with other pathologies. Scombroid is a type of food poisoning caused by the ectopic consumption of histamine that mimics angioedema. Here, we present the case of a 36-year-old male patient who presented to the emergency department (ED) after consuming tuna, with a clinical picture of generalized erythema accompanied by…

A case of histamine fish poisoning in a young atopic woman.

Histamine fish poisoning, also known as scombroid poisoning, is a histamine toxicity syndrome that results from eating specific types of spoiled fish. Although typically a benign syndrome, characterized by self-limited flushing, headache, and gastrointestinal symptoms, we describe a case unique in its severity and as a precipitant of an asthma exacerbation. A 25-year-old woman presented to the emergency department (ED) with one hour of tongue and face swelling, an erythematous pruritic rash, and dyspnea with wheezing after consuming a tuna sandwich. She developed abdominal pain, diarrhea and h…

Flushing associated with scombroid fish poisoning.

Scombroid poisoning is a form of toxicity caused by the ingestion of spoiled dark-flesh fishes, mainly of the scombroid family. The clinical picture is secondary to histamine toxicity, manifested as flushing, headache, palpitations, and abdominal cramps. The diagnosis is established on the basis of these typical clinical manifestations and fish-ingestion history. We report a case of a man with scombroid poisoning. He was diagnosed after presentation of two similar patients with the same epidemiological history.

Circulating serotonin in vertebrates.

The role of circulating serotonin is unclear and whether or not serotonin is present in the blood of non-mammalian species is not known. This study provides the first evidence for the presence of serotonin in thrombocytes of birds and three reptilian species, the endothermic leatherback sea turtle, the green sea turtle and the partially endothermic American alligator. Thrombocytes from a fresh water turtle, American bullfrog, Yellowfin tuna, and Chinook salmon did not contain serotonin. Serotonin is a vasoactive substance that regulates skin blood flow, a major mechanism for endothermic body t…

Morphological and molecular characterization of four Ceratomyxa species parasitizing marine fish in Japan including descriptions of three new species.

A parasitological survey in western Japan identified the myxosporean parasites in the bile and gallbladders of four marine fish species: farmed white trevally Pseudocaranx dentex (Bloch & Schneider), Pacific bluefin tuna Thunnus orientalis (Temminck & Schlegel), and red seabream Pagrus major (Temminck & Schlegel), as well as wild false kelpfish Sebastiscus marmoratus (Cuvier). Morphological characterization and molecular analysis of the small subunit rDNA confirmed that these parasites belong to the genus Ceratomyxa. Three novel species were identified: Ceratomyxa shimaaji n. sp. from P. dente…
