Episodic dyscontrol syndrome.

Episodic dyscontrol syndrome (EDS) or intermittent explosive disorder (IED) is a clearly identified category in the Diagnostic and Statistical Manual of Mental Disorders (DSM IV). It affects children and adults. Children are often considered to have epilepsy or a mental health problem. The episodes comprise of recurrent attacks of uncontrollable rage, usually after minimal provocation, and may last up to an hour. Following an episode, children are frequently exhausted, may sleep and will usually have no recall. These features often raise the possibility of epilepsy, and children may be referre

Is the chest x-ray an appropriate screening exam for ER patients with AMS?

Emergency department patients with altered mental status (AMS) regularly undergo a routine chest radiograph at our institution. While there are many causes of chest pathology seen on the chest radiograph that may cause an altered mental status, it is not clear that a routine chest radiograph for these patients affects management. The goal of this study is to determine if a chest radiograph is an appropriate screening examination for AMS. This is a retrospective review of 100 consecutive patients who underwent head computed tomography for altered mental status in the emergency department and ha

[Pseudo-glaucoma in type VI mucopolysaccharidosis: case report].

The authors report a case of a 19-year-old patient presenting with type VI mucopolysaccharidosis, diagnosed by genetic-clinical examination, demonstrating several systemic manifestations, including ocular disorders such as: corneal opacity, elevated intra-ocular pressure and increase of corneal thickness. The authors discuss the characteristic syndromic findings and the influence of corneal thickness associated with an increase in intraocular pressure leading to unnecessary antiglaucomatous treatment.

Total gastrectomy is not always necessary for advanced gastric cancer of the cardia.

BACKGROUND: It is unclear whether total gastrectomy (TG) is always necessary for gastric cancer of the cardia. We therefore investigated whether cardiac cancers treated by TG would have been cured by proximal gastrectomy (PG). METHODS: Photocopies of the resected stomachs of 55 patients who had received TG for cardiac cancer were reviewed. A simulated resection line for PG was drawn connecting a point 5 cm from the pyloric ring on the lesser curvature with a point 15 cm from the pyloric ring on the greater curvature. The distal surgical margin between the tumor edge and the simulated resectio

Tonsillectomy for biopsy in children with unilateral tonsillar enlargement.

OBJECTIVE: Unilateral tonsillar enlargement (UTE) may be a sign of underlying malignancy and tonsillectomy is often recommended for histology. The limited evidence available suggests that the incidence of malignancy in children with UTE is very low, and that in many cases, the apparent enlargement is due to asymmetry of the mucosa of the tonsillar pillars. The purpose of this study was to evaluate the necessity of tonsillectomy as a routine practice for every case of UTE. METHODS: We attempted to identify every child (age < 16 years) undergoing tonsillectomy in our unit from January 1991 to J

Appropriateness of use of indwelling urinary catheters in patients admitted to the medical service.

Of 836 medical admissions evaluated over a 1-month period, 89 (10.7%) had a urinary catheter placed within 24 hours; 34 placements (38%) had no justifiable indication. Risk for inappropriate catheterization was independent of age, gender, functional status, and mental status at admission. Preventive measures should focus on increasing awareness among healthcare providers.

Benign myoclonus of early infancy: an imitator of West's syndrome.

Benign myoclonus of early infancy is a rare condition characterized by nonepileptic spasms that may resemble the epileptic spasms seen in West's syndrome. The spells in benign myoclonus of early infancy begin before age 1 year and are self-limited. The electroencephalogram (EEG) is invariably normal, and neurologic development is not affected. West's syndrome is characterized by infantile spasms that appear before 1 year of age, an abnormal EEG with hypsarrhythmia, and a poor prognosis. We describe six infants who presented for evaluation of clusters of head, trunk or extremity spasms, eye bli

Choosing wisely in antimicrobial stewardship: results from a national survey.

The French Society for Infectious Diseases conducted a survey following the "Choosing Wisely" standardized process, amongst French infectious diseases specialists, between 2024 and 2025. It aimed to identify the "Top 5" recommendations to reduce unnecessary procedures and treatments in infectious diseases. This work provides a strategic tool to target priority areas for antimicrobial stewardship.

Paraparetic Guillain-Barre syndrome: An uncommon diagnosis of acute flaccid paralysis of the lower limbs.

Apart from the usual differentials of transverse myelitis and cord compression, paraparetic GBS should be considered when sudden, flaccid paralysis of the lower limbs occurs, as prompt diagnosis and management can minimize sequel and unnecessary procedures. We do report a case wherein we managed a similar situation without the use of an immunomodulatory therapy.

Catheterization prior to evacuation of uterus and diagnostic curettage.

Dilatation and curettage and evacuation of the uterus are the 2 most common minor gynaecological procedures that trainees perform. Some trainees are taught to catheterize the patient before these procedures. This study was carried out with 2 aims, firstly to establish the prevalence of significant residual urine volume prior to these procedures and secondly to gain information on the practices among members and fellows of the RNZCOG residing in New Zealand with regard to catheterization prior to these operations. The measured residual urine volume was greater than 50 mL in 37.5% of patients an

Donut rush to laparoscopy: post-polypectomy electrocoagulation syndrome and the 'pseudo-donut' sign.

Colonoscopic polypectomy is a routine procedure with the potential for rare but well-known complications, including perforation and bleeding. Post-polypectomy electrocoagulation syndrome (PPES) is a less recognized cause of abdominal pain following this procedure. However, it is important to diagnose PPES in order to avoid unnecessary intervention. We present the case of a patient with abdominal pain after polypectomy. The patient underwent an unnecessary diagnostic laparoscopy on the basis of misinterpreted radiological findings. Her CT scan demonstrated the "donut" sign that was suggestive o

Securing the position of the nasoseptal flap in endoscopic transsphenoidal surgery: No need for a Foley catheter (technical report).

BACKGROUND: The pedicled nasoseptal mucoperiosteal flap is currently widely used for the reconstruction of a skull base defect following transsphenoidal surgery. The flap is generally secured in position by a Foley catheter balloon. We describe an alternative technique using cylinders of Gelfoam™ (Pfizer Incorporated) to buttress the flap in place, obviating the need for a balloon catheter. MATERIAL AND METHODS: A review of our database identified patients who underwent endoscopic transsphenoidal surgery for a pituitary macroadenoma with nasoseptal flap, secured with small rolls of Gelfoam™ (

A Developing Anterior Mediastinal Mass in a Breast Cancer Patient: Thymic Hyperplasia.

INTRODUCTION: True thymic hyperplasia following chemotherapy has been described mostly in children. There are a few cases of thymus hyperplasia that have been reported in breast cancer patients. Diagnosis of this unusual entity is very crucial to pretend unnecessary surgery or interventional diagnostic procedures. CASE PRESENTATION: We report a case of thymus hyperplasia in a patient who was operated and treated with adjuvant chemotherapy for stage 2 breast cancer two years ago. In the follow-up CT scans, an anterior mediastinal mass was noted. Radiologic evaluation and follow-up revealed thy

Thoracic splenosis: know it--avoid unnecessary investigations, interventions, and thoracotomy.

Thoracic splenosis (TS) is autoimplantation of ectopic splenic tissue in the thoracic cavity that occurs following splenic injury. Most cases of TS are asymptomatic and are diagnosed during the course of an evaluation of incidentally discovered pulmonary lesions. Some cases may be difficult to diagnose, especially if features suggesting TS are not recognized. This may lead to an extensive workup and unnecessary invasive diagnostic procedures including thoracotomy. Multiple, asymptomatic, left-sided pleura-based lesions associated with a history of thoracoabdominal injury and splenectomy are th

Parotid mass as presenting symptom of lymphoma.

BACKGROUND: Lymphomas arising from the parotid gland are an uncommon entity, said to account for only 0.6-5% of tumors or tumor-like lesions of the parotid, and are therefore commonly overlooked. This misdiagnosis often leads to unnecessary diagnostic procedures, delaying the initiation of proper treatment. OBJECTIVES: To examine the clinical, diagnostic, therapeutic and survival data of patients with this disease. METHODS: We retrospectively reviewed our experience with patients diagnosed and treated for parotid lymphoma in our medical center during the period 1998-2008. RESULTS: The 13 pa

Aberrant hormone production from ovarian neoplasms: strategies for diagnosis and therapy.

Syndromes involving peptide or nonsex steroid hormone secretion due to aberrantly located tumors are rare. We report a collected series of 16 patients with ectopic hormone production from ovarian neoplasms, including 3 patients recently encountered at our institution as well as 13 additional cases identified in the recent literature. These tumors included 2 insulin-producing ovarian carcinoids, 1 ACTH-producing pituitary adenoma within a benign ovarian cystic teratoma, 2 cortisol-producing ovarian neoplasms, 8 gastrin-producing ovarian cystadenomata or cystadenocarcinomata, and 3 thyroxine-pro

Drug-induced Creutzfeldt-Jakob like syndrome.

A patient with progressive neurological deterioration characterized by cognitive impairment, myoclonus, Parkinson's syndrome, an abnormal electroencephalogram and fasciculations was considered for brain biopsy for suspected Creutzfeldt-Jakob disease. Complete clinical recovery followed discontinuation of lithium and nortriptyline. Awareness of this unusual drug-induced Creutzfeldt-Jakob like syndrome can avoid costly, invasive and unnecessary investigative procedures.
