[Neurogenic urinary incontinence: current treatment concepts].

Neurogenic urinary tract dysfunction is characterized by inadequate voiding and urinary incontinence. The aim of therapy nowadays is adequate bladder emptying and control of urinary incontinence. Neurogenic urinary incontinence can be caused by (a) detrusor hyperreflexia, (b) sphincter hypo- or areflexia, (c) a combination of both, or also (d) detrusor hyporeflexia with consequent overlow incontinence. Based on a simple urodynamic classification the current treatment strategies are presented. (a) Detrusor hyperreflexia can be transformed into hypo- or are-flexia pharmacologically with potent d…

Intermittent catheterisation for neuropathic urinary incontinence.

Eighty six children with neuropathic urinary incontinence who had been treated by clean intermittent catheterisation for more than five years were reviewed. Eighty five had congenital lesions, and one traumatic paraplegia. During the day, 72 (84%) patients were dry or had minor stress incontinence only, 11 were damp but controlled with pads, and only three were continuously wet. Eleven abandoned clean intermittent catheterisation, five because of poor control, four by choice despite good control, and only two because of deterioration of upper urinary tract disease. Most of them usually had bac…

Practical approach to stress urinary incontinence.

Stress urinary incontinence (SUI) in women is more frequent in the multiparous patient, especially in those postmenopausal after withdrawal of hormonal stimulation of the pelvic supportive tissues. With loss of support of the vesical neck and urethra, sphincter incompetence may result. A patient whose symptoms are purely irritative (urinary frequency, nocturia, urgency, or urge incontinence), without a significant degree of stress incontinence, should not benefit from any operation.

Sexual dysfunction associated with urinary incontinence.

208 female patients attending an incontinence clinic gave information about their marital lives and sexual habits. 90 women (43%) maintained that their urinary disorder had adversely affected sexual relations. Women with bladder instability had a significantly higher incidence of sexual dysfunction than women with pure stress incontinence.

Reflex electrical stimulation for urinary incontinence.

Our previous observations have shown that the electrical stimulation of muscles is prevalently reflex. One of the advantages of reflex stimulation is that it activates not only a limited number of motor units, but rather a number of muscles connected by the same reflex from a single stimulation site. Consequently, it is not necessary to place electrodes into the muscle to be activated. They can be put elsewhere provided that the same effect is obtained and that it is more convenient for the patient. Such an opportunity arises when treating urinary incontinence which involves not only the ureth…

Reflex electrical stimulation for urinary incontinence.

Experiments with several patients suffering from stress incontinence have shown that indirect stimulation of the levator ani with a vaginal stimulator and especially of the anal sphincter with an anal stimulator affects the urethral sphincter in the same way as direct stimulation. These findings are significant since they enable us to avoid the use of implantable stimulators by substituting external ones. External stimulation is worth trying in all cases of stress incontinence, where other conservative measures have failed. In our cases the results have been very satisfactory.

Urinary incontinence after stress incontinence surgery: a risk factor for depression.

OBJECTIVES: To determine whether urinary incontinence after stress incontinence surgery is a risk factor for concomitant symptoms of depression. METHODS: Women who underwent stress urinary incontinence surgery from 1993 to 2002 were given 2 validated quality-of-life questionnaires, the Incontinence Symptom Index and the Patient Health Questionnaire, to assess the severity of urinary incontinence symptoms after surgery and to identify moderate-to-severe depressive symptoms, respectively. Bivariate and multivariate statistical models were then constructed to determine the independent parameters…

Mixed urinary incontinence symptoms: urodynamic findings, incontinence severity, and treatment response.

OBJECTIVE: To investigate the relationship between the symptom of mixed urinary incontinence and incontinence severity, urodynamic findings, and treatment response. METHODS: This is a secondary analysis of data from 553 women randomized into a double-blind, placebo-controlled study evaluating duloxetine (serotonin-norepinephrine reuptake inhibitor) for the treatment of predominant stress urinary incontinence. Assessment variables included incontinent episode frequency, the Incontinence Quality of Life Questionnaire (I-QOL), and the Patient Global Impression of Severity Scale (PGI-S). Urge sym…

Chronic urinary bladder torsion causing urinary incontinence in a cat.

CASE SUMMARY: A 10-month-old female spayed domestic shorthair cat was presented for urinary incontinence. The cat was azotaemic, and ultrasound examination identified hydroureter and hydronephrosis. Subsequent computed tomography (CT) contrast pyelography allowed a diagnosis of urinary bladder torsion. Surgical findings and treatment are described. Postoperatively, incontinence and azotaemia resolved, and marked improvements were documented with ultrasound and CT in urinary tract structural abnormalities. RELEVANCE AND NOVEL INFORMATION: To our knowledge, urinary bladder torsion has not been …

Treatment of urinary incontinence with AMS 800 artificial urinary sphincter.

BACKGROUND: Urinary incontinence has a significant impact on the quality of life for patients. The introduction of the AMS 800 artificial urinary sphincter by American Medical Systems (Minnetonka, MN, USA) has revolutionized the treatment of urinary incontinence due to pure sphincteric incompetence. PATIENTS AND METHODS: From April 1989 to October 1997, a total of 52 patients (42 men and 3 women) underwent implantation of the AMS 800 artificial sphincter. Patient age ranged from 31 to 87 years (mean 69.5 years). They all were suffering from urinary incontinence caused by sphincteric damage, a…

Artificial urinary sphincter for neurogenic urinary incontinence: a narrative review.

BACKGROUND AND OBJECTIVE: The artificial urinary sphincter (AUS) is most known for its use in the treatment of moderate to severe post-prostatectomy stress urinary incontinence. However, another lesser-known indication includes stress incontinence related to intrinsic sphincter deficiency (ISD) in the neurogenic bladder population. The purpose of this review is to discuss specific technical considerations related to device implantation in this population, efficacy, durability, and complications. METHODS: We performed a non-systematic literature review using the PubMed Database to identify art…

The Effect of Conservative Treatment on Bladder Diary Parameters in Women With Urgency Urinary Incontinence and Mixed Urinary Incontinence.

BACKGROUND: Urinary incontinence (the involuntary loss of urine) is a worldwide problem, and bladder diaries are used to characterize the nature and severity of the disease. eHealth and mobile apps are used as tools to aid in conservative treatment, and the mobile app TätII has been developed for the treatment of urgency urinary incontinence (UUI) and mixed urinary incontinence (MUI) in women. PURPOSE: The aim of this study was to evaluate how conservative treatment affected bladder diary parameters in women with UUI and MUI. METHODS: For this purpose, bladder diaries produced by 123 partici…
