Use of large bore devices in the treatment of pulmonary embolism.

Pulmonary embolism (PE) is one of the leading causes of cardiovascular mortality in the United States. Even in those patients who get treatment in the acute phase, they are at risk of developing pulmonary hypertension, recurrent PE, which can lead to functional impairment and decreased survival. Interventional therapies for PE are reserved for unstable patients with high-risk PE or stable patients at high risk for decompensation. Several large-bore devices are currently approved for the treatment of PE and can provide expeditious thrombus removal without the need for lytic agents. Here, we rev

Modern Approach to Vascular Access in the Critically Ill Patient.

Vascular access is a key intervention in the management of critically ill emergency department patients. This can include peripheral or central venous access and intraosseous access. In combination with arterial catheterization, central venous access can provide key information to guide volume and vasoactive medication titration in isolation as well as in combination with dynamic measures of fluid responsiveness. This article will review the indications and contraindications as well as decision-making for the use of these modalities in light of the current evidence.

Patient perspective and satisfaction with different types of vascular access in hemodialysis: a systematic review and meta-analysis.

BACKGROUND: To assess satisfaction and perspectives of adult hemodialysis (HD) patients on different vascular access (VA) types (AVF, AVG, CVC) by synthesizing quantitative studies using validated questionnaires, focusing on overall satisfaction, physical symptoms, social functioning, and complications. METHODS: We systematically searched Medline, Web of Science, and Scopus for studies assessing adult HD patients' perspectives on VA using structured questionnaires. Risk of bias was assessed using the Newcastle-Ottawa Scale. Meta-analysis was performed using a random-effects model across three

Mapping existing research on flushing peripheral vascular catheters: a scoping review protocol.

INTRODUCTION: The peripheral vascular catheter (PVC) is the most common intravenous medical device used in hospitals, given it is crucial for the delivery of intravenous therapies. Despite its widespread use, PVC complications such as occlusion, infiltration, extravasation, phlebitis, thrombophlebitis, catheter-related thrombosis and catheter-related bloodstream infection significantly affect patient safety, leading to increased morbidity and healthcare costs. One approach to maintain PVC patency and therefore device longevity is to flush the PVC. This usually involves the use of 5 to 10 mL sy

Perspectives on Quality of Life, Vascular Access, and Fluid Balance in Incremental Hemodialysis: A Narrative Review.

Hemodialysis is an intensive treatment with a substantial time burden for patients. Generally, a standard hemodialysis schedule consists of three treatments per week, each lasting 4 hours. However, it can be debated whether this schedule is appropriate for all patients. Incremental hemodialysis refers to a treatment approach where hemodialysis is initiated with a less intensive schedule ( i.e ., fewer sessions per week) and gradually increased as residual kidney function declines. The effects of incremental hemodialysis on key outcomes-health-related quality of life, vascular access-related co

Adverse events associated with umbilical catheters: a systematic review and meta-analysis.

OBJECTIVE: To determine the incidence of adverse events (AEs) associated with umbilical catheters in the neonatal population. STUDY DESIGN: Systematic review and meta-analysis of observational studies and randomized controlled trials published between 2010 and 2020. RESULTS: In total 14,226 umbilical venous catheters (UVCs) and 4228 umbilical arterial catheters (UACs) were included. Overall, 13.4% of UVCs were associated with an AE (95% CI: 10.1-17.0) or 2.4 per 1000 catheter days (95% CI: 1.8-3.0). UACs had an AE rate of 9% (95% CI: 5.9-12.8) or 0.87 per 1000 catheter days (95% CI: 0.4-1.3)

Ultrasound-guided supraclavicular nerve block for implantable port catheters: Does it show a significant difference in pain control?

BACKGROUND: Nerve blocks have been used for decades at head and neck region interventions and recently introduced as an alternative option for subcutaneous port implantation. This study aimed to compare two different local anesthesia techniques used during subcutaneous port implantation. METHODS: This retrospective study was conducted with 107 patients who were categorized into two groups according to the local anesthesia techniques used during port implantations. Group 1 underwent local infiltration anesthesia and Group 2 received an ultrasound-guided supraclavicular nerve block. In both gro

An improved reservoir for the flushing test to diagnose shunt insufficiency.

OBJECTIVE Reservoirs integrated into hydrocephalus shunts are commonly used for the removal of CSF and for intra-ventricular pressure measurement. Pumping with the reservoir to diagnose shunt sufficiency is still a matter of controversy. The authors describe an improved flushing device and its characteristic features in vitro and in vivo. METHODS The flushing reservoir is constructed with a sapphire ball in a cage as a nonresistance valve to also enable the detection of distal occlusions. The most important reservoir parameters were investigated in vitro, simulating total and partial proximal

A novel model of large vessel ischemic stroke in rabbits: microcatheter occlusion of the posterior cerebral artery.

BACKGROUND AND PURPOSE: The rabbit small clot embolic model for large vessel occlusion (LVO) is well established, yet has limitations. Blind introduction of autologous thrombus often fails to completely occlude a target vessel and, when successful, the precise timing of occlusion and revascularization is difficult to control. Studies of cellular biology and neuroimaging of acute reversible cerebral ischemia (ie, penumbral tissue) would benefit from a rabbit model in which LVO can be reliably induced, easily confirmed, and in which the time of occlusion and revascularization can be precisely co

Hybrid nanomaterial for stabilizing the antibiofilm activity of Eugenia carryophyllata essential oil.

The aim of the present study was to demonstrate that Fe(3)O(4)/oleic acid core/shell nanostructures could be used as systems for stabilizing the Eugenia carryophyllata essential oil (EO) on catheter surface pellicles, in order to improve their resistance to fungal colonization. EO microwave assisted extraction was performed in a Neo-Clevenger (related) device and its chemical composition was settled by GC-MS analysis. Fe(3)O(4)/oleic acid-core/shell nanoparticles (NP) were obtained by a precipitation method under microwave condition. High resolution transmission electron microscopy (HR-TEM) wa

Ultrastructure of a novel bacterial form located in Staphylococcus aureus in vitro and in vivo catheter-associated biofilms.

Bacterial biofilms are ubiquitous in nature, industry, and medicine, and understanding their development and cellular structure is critical in controlling the unwanted consequences of biofilm growth. Here, we report the ultrastructure of a novel bacterial form observed by scanning electron microscopy in the luminal vegetations of catheters from patients with active Staphylococcus aureus bacteremia. This novel structure had the general appearance of a normal staphylococcal cell but up to 10 to 15 times as large. Transmission electron microscopy indicated that these structures appeared as sacs e

Comparative Study of Single- Versus Double-ProGlide Device Strategy for Access Site Closure of Transfemoral Transcatheter Aortic Valve Replacement.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is a first-line treatment for severe symptomatic aortic stenosis (AS) patients, and the preferred approach for TAVR is transfemoral. The double-ProGlide preclose technique offers an effective and safe method for closure of transfemoral access site after TAVR. However, the debate is still ongoing whether a single-ProGlide strategy is equal or even superior to a double one in terms of hemostasis and vascular complications. AIMS: The aim of this study is to examine the safety and efficacy of a single- versus double-ProGlide strategy for v

Access site complications following Impella-supported high-risk percutaneous coronary interventions.

Mechanical circulatory support (MCS) devices are increasingly used to provide hemodynamic stability for patients with severe coronary artery disease, comorbidities, and/or impaired hemodynamics during high-risk percutaneous coronary interventions (PCI). Vascular access site complications, particularly those due to the use of large-bore sheaths, may limit outcomes in these patients. The aim of this study was to investigate the incidence and predictors of vascular complications in protected high-risk PCIs. Therefore, we included patients undergoing high-risk PCI with an Impella device from Janua

Management of patients with challenging vascular access needs.

The proportion of patients on dialysis with multiple access procedures, failed transplant(s), or no suitable sites for conventional arteriovenous (AV) fistula is increasing. We report on 4 patients at our institution requiring unusual measures to achieve vascular access for dialysis. All 4 patients had temporary femoral vein catheters for dialysis, but 3 had tunneled femoral catheters, with 1 patient using this modality of access for 28 months. One patient who had 13 vascular access procedures developed collateral circulation that was amenable to a new autogenous AV fistula. Two patients died

Clinical outcomes following single access transfemoral transcatheter aortic valve implantation.

OBJECTIVES: We describe the first experience using calcification of anatomical landmarks to obviate the need for transcatheter aortic valve implantation (TAVI) alignment aortography and secondary TAVI access. BACKGROUND: TAVI alignment conventionally involves secondary femoral access for contrast aortography using a second catheter. Secondary femoral access accounts for up to 25% of all vascular complications. Heavily calcified aortic leaflets are often visible fluoroscopically and can act as markers for TAVI alignment. METHODS: We considered 100 consecutive patients for transfemoral TAVI. T
