Skull Base Dural Closure Using a Modified Nonpenetrating Clip Device via an Endoscopic Endonasal Approach: Technical Note.

Skull base reconstruction after an endoscopic endonasal approach into the cerebrospinal fluid (CSF) space is always challenging. Various reconstructive methods are available, but no standard technique is established. This report describes the endoscopic skull base dural closure using a modified nonpenetrating clip device with shaft length of 15 cm. Six patients with an intra-suprasellar or suprasellar tumor who underwent extended endoscopic endonasal transsphenoidal surgery were targeted. For closure of the skull base dural defect after tumor removal, fascia lata was first placed as an inlay g

Comparison of plug-based versus suture-based vascular closure for large-bore arterial access: a collaborative meta-analysis of observational and randomized studies.

BACKGROUND: Large-bore arteriotomies can be percutaneously closed with suture-based or plug-based vascular closure device (VCD) strategies. The efficacy of both techniques remains controversial. AIMS: We conducted a meta-analysis of comparative studies between both VCD strategies, focusing on the most commonly applied VCDs (MANTA and ProGlide). METHODS: We searched MEDLINE, the Cochrane Central Register of Controlled Trials and Google scholar for observational studies (OS) and randomized controlled trials (RCT) comparing vascular closure with the MANTA-based and the ProGlide-based technique.

Outcomes of ultrasound-guided and conventional Angio-Seal closure following antegrade femoral artery access: a systematic review and meta-analysis of observational studies with an exploratory indirect comparison.

PURPOSE: Vascular closure devices (VCDs), particularly the Angio-Seal device, are widely used for rapid hemostasis following antegrade femoral artery access during peripheral vascular interventions. However, the comparative safety and efficacy of ultrasound-guided vs. conventional non-ultrasound-guided antegrade femoral access closure with Angio-Seal have not been quantified. This meta-analysis synthesizes available evidence comparing these approaches in patients undergoing antegrade femoral artery access closure with Angio-Seal devices. METHODS: Relevant studies were systematically identifie…

Single versus double use of a suture-based closure device for transfemoral aortic valve implantation.

BACKGROUND: The most common method of percutaneous closure in transfemoral transcatheter aortic valve implantation (TAVI) employs two obtusely oriented ProGlide devices. The aim of this study was to assess the feasibility and safety of using a single ProGlide system for primary access site closure in comparison with the double ProGlide approach in an all-comers TAVI population. METHODS: Between March 2016 and December 2018, a total of 1105 patients underwent transfemoral TAVI for severe aortic stenosis at our center. Application of two ProGlide systems was standard until April 2017 (n = 432),

Transcatheter closure of coronary arterial fistula in children and adolescents.

BACKGROUND: Transcatheter closure of coronary arterial fistula (CAF) is being utilized increasingly as an alternative to surgical closure, but most of the literature on transcatheter closure of CAF in children and adolescents consists of case reports and reports of small series. The aim of this study was therefore to describe our experience of percutaneous closure of CAF in children and adolescents. METHODS: Between April 1998 and December 2012, 37 children and adolescents with congenital CAF underwent percutaneous transcatheter closure. After aortic root angiography and selective coronary an

Single Perclose Plus Plug Based Device versus Double Perclose for Percutaneous Large Bore Arterial Access: An Systematic Review and Meta-analysis.

BACKGROUND: Vascular Closure primarily utilizing suture-based devices (SBDs) are frequently used for large bore percutaneous arterial access in patients undergoing Transcatheter Aortic Valve Implantation and Endovascular Aneurysm Repair. Little is known about the intentional combination of SBD with plug-based device (PBD). Main objective was to determine the efficacy of adopting a combined SBD and PBD strategy compared to double SBD for large bore arterial access. METHODS: We searched randomized and observational studies comparing both strategies and that reported the outcomes of major vascul

Percutaneous axillary arteriotomy closure during transcatheter aortic valve replacement using the MANTA device.

Percutaneous aortic valve replacement is performed predominantly via the transfemoral approach. The transaxillary (subclavian) approach may be utilized if an alternative route access is required. Conventional access and closure for this approach necessitates open surgical techniques. We report a nonsurgical fully percutaneous axillary TAVR using a collagen-based vascular closure device.

Feasibility of the Figure-of-8-Suture as Venous Closure in Interventional Electrophysiology: One Strategy for All?

Background: Venous vascular access with higher sheath size is common in interventional electrophysiology. In contrast to arterial vascular access, no dedicated closure devices exist for closure after venous access with higher sheath sizes. The Figure-of-8-Suture, an easy to apply suture, may be as a feasible approach for closure venous puncture. Our aim was to evaluate the feasibility of closure of femoral venous access. Methods: From November 2016 to February 2018, patients undergoing electrophysiological procedures, closure of left atrial appendage or patent foramen ovale were included. Unti

Transcatheter closure of a coronary fistula with an Amplatzer vascular plug: should a retrograde approach be standard?

Coronary artery fistulae are rare disorders. Only 2 reports in the medical literature discuss the use of vascular plugs to occlude coronary fistulae, and the same device-the Amplatzer Vascular Plug-was deployed via different techniques to treat those patients. The safety, the feasibility, and the standard approach to deployment have yet to be established. Herein, we describe the case of a 15-year-old boy who presented with a continuous murmur at the left sternal border. The patient was diagnosed with a large coronary fistula that originated from the right coronary artery and emptied into the r

A minimally invasive technique for closing an iatrogenic subclavian artery cannulation using the Angio-Seal closure device: two case reports.

INTRODUCTION: In the two cases described here, the subclavian artery was inadvertently cannulated during unsuccessful access to the internal jugular vein. The puncture was successfully closed using a closure device based on a collagen plug (Angio-Seal, St Jude Medical, St Paul, MN, USA). This technique is relatively simple and inexpensive. It can provide clinicians, such as intensive care physicians and anesthesiologists, with a safe and straightforward alternative to major surgery and can be a life-saving procedure. CASE PRESENTATION: In the first case, an anesthetist attempted ultrasound-gu

Transaneurysmal Suturing of Pseudoaneurysm Neck: Novel and Not a Pseudo-Technique for Treatment of Complicated Femoral Pseudoaneurysm.

Introduction  Femoral pseudoaneurysm is one of the common complications following femoral artery catheterization. It poses a significant morbidity and a nonsurgical technique with the utmost low complication risk is warranted. Various methods have been used to treat these pseudoaneurysms including manual compression, ultrasound-guided compression, and percutaneous injection of thrombin or glue. Various novel methods are also being used to treat the pseudoaneurysms, including angioseal closure device, vascular plugs, etc. Cirrhosis poses a separate challenge with deranged coagulation and hyperf
