Right lower quadrant abdominal pain is a common presentation to the Emergency Department. While appendicitis is a frequently diagnosed pathology associated with this complaint, there are multiple, well-documented alternatives that must be considered. The authors present a unique case of a 20 year old active duty male who presented with 3 days of acutely worsening right lower quadrant abdominal pain, right flank pain, and anorexia. Following a detailed work-up, computed tomography and magnetic resonance imaging (MRI) demonstrated a bland thrombus within the inferior vena cava (IVC) secondary to

A 32-year-old female presented to the emergency room with severe right loin pain. Abdominal color Doppler ultrasound (CDUS) observed a large thrombus in the inferior vena cava (IVC). Computed tomography with contrast described a large floating thrombus in the IVC with no visible wall attachment. Emergency transjugular suprarenal IVC filter was inserted and overnight infusion thrombolysis over 24 h. The patient started to show constitutional symptoms of pyelonephritis such as high-grade fever and pyuria post-thrombolysis. Immediate empiric antibiotic coverage was started. Overnight infusion thr

Cardiac defects in neonates are often associated with neurological or neuroanatomical anomalies. We present a patient who developed macrocephaly secondary to superior vena cava syndrome, resultant from repair of her congenital tetralogy of Fallot. She was managed conservatively with serial imaging and close observation from the neurosurgical aspect, and her head growth stabilized without requiring ventriculoperitoneal shunting after stent placement in her superior vena cava. In patients with macrocephaly secondary to cardiac issues, cerebrospinal fluid diversion is often performed quickly. For

Superior vena cava syndrome occurs from obstruction of the superior vena cava. The most common cause is malignancy. Small cell lung cancer and non-Hodgkin lymphoma are the most frequent culprits. Intravascular devices associated with thrombus are becoming more common causes. Classic symptoms include edema, plethora, and distended veins of the face, neck, and chest; shortness of breath; cough; headache; and hoarseness. Treatment in the emergency department is mostly supportive, with head elevation, oxygen, and steroids. Rarely, emergent airway issues and cerebral edema must be addressed. Defini

BACKGROUND: Advanced-stage thymic tumors infiltrating the superior vena cava (SVC), when radically resectable, can be surgically treated by SVC prosthetic replacement within a multimodality therapeutic approach. We hereby present our series of patients undergoing SVC resection and prosthetic reconstruction for stage III or IV thymic malignancies.
METHODS: Between 1989 and 2015, 27 patients with thymic tumors (21 thymoma, 6 thymic carcinoma) infiltrating the SVC underwent radical resection with a SVC prosthetic replacement by a bovine pericardial conduit in 12 cases, a polytetrafluoroethylene

BACKGROUND: Inferior vena cava thrombosis (IVCT), although rare, has a potential for significant morbidity and mortality. IVCT is often a result of IVC filter thrombosis, but it can also occur de novo. Although anticoagulation remains the standard of care, endovascular techniques to restore IVC patency have become key adjunctive therapies in recent years. This study examines a single-center experience with diagnosis and management of IVCT.
METHODS: A retrospective Institutional Review Board-approved review of a single-center institutional database was screened to identify IVCT thrombosis usin

We present a case of a 62-year-old woman with end-stage renal disease who displayed a persistent left superior vena cava detected following placement of haemodialysis catheter through the left internal jugular vein (IJV). The diagnosis was confirmed by left internal jugular vein cathetogram, computed tomography (CT) thorax and transthoracic echocardiography. The catheter was removed without immediate complications. She remained well during the outpatient follow-up.

Resection and reconstruction of the superior vena cava in patients with lung cancer is still considered a surgical challenge; the numerous problems related to patient selection, the choice of the most appropriate surgical strategy and technique, the most suitable material for replacement, and the number of potential complications definitively show an impact on short-term outcome and long-term prognosis. However, with the appropriate indications and surgical technique, a clear benefit has been documented in a select group of patients with lung cancer.

Superior vena cava (SVC) aneurysms are rare mediastinal vascular lesions. We report a case of a 42-year-old female, who presented to the outpatient department with features suggestive of lower respiratory tract infection. Chest X-ray showed abnormal contour and widening of right border of mediastinum. Computed tomography (CT) thorax revealed fusiform aneurysmal ectasia of SVC measuring 4.5 × 5.5 × 8.9 cm without internal thrombosis or dissecting flap. Management options include observation with follow-up and in some cases anticoagulation and surgical excision may be considered. The general con

The paper analyzes indications for, frequency, safety, and efficiency of inferior vena cava (IVC) suprarenal filter implantation. The study based on the Medline database has indicated that an IVC suprarenal filter should be implanted following very strict indications, such as extensive caval thrombosis; renal vein thrombosis with a thrombus in the IVC; some vascular anomalies; a renal tumor process with a tumor thrombus in the IVC; prevention of pulmonary embolism in pregnant women with deep vein thrombosis; and prior to surgery. IVC suprarenal implantation done as clinically indicated is an e

Inferior vena cava (IVC) filters are the mainstay for pulmonary embolic prophylaxis in patients with high venous thromboembolic (VTE) risk-in particular, patients with acute VTE (deep venous thrombosis and/or pulmonary embolism) who have contraindication to therapeutic anticoagulation. Technology continues to evolve regarding IVC filters, with the most exciting changes over the past several decades including techniques of percutaneous insertion from laparotomy and retrieval of these devices. This paper will review current IVC filter designs and concepts and will discuss developments on the hor

In the last 30 years patients with pulmonary embolism have increasingly often been implanted with a vena cava filter. Evidence of safety and efficacy in comparison to pure anticoagulation are in short supply. Re-removable filter reduces neither the risk of recurrence nor mortality, according to a study now revealed.

The superior vena cava (SVC) is the largest central systemic vein in the mediastinum. Imaging (ie, radiography, computed tomography [CT], magnetic resonance [MR] venography, and conventional venography) plays an important role in identifying congenital variants and pathologic conditions that affect the SVC. Knowledge of the basic embryology and anatomy of the SVC and techniques for CT, MR imaging, and conventional venography are pivotal to accurate diagnosis and clinical decision making. Congenital anomalies such as persistent left SVC, partial anomalous pulmonary venous return, and aneurysm a

Intentional interruption of the inferior vena cava with caval ligation and a Mobin-Uddin filter was once commonly used to prevent recurrent pulmonary emboli and was associated with significant mortality and morbidity, including a high incidence of post-thrombotic syndrome. Recanalization of an intentionally interrupted inferior vena cava has been rarely described in literature and is commonly considered futile. We describe two patients with post-thrombotic syndrome as a late complication of caval ligation and a thrombosed Mobin-Uddin filter, with significant and sustained improvement after end

Transvenous placement of inferior vena cava (IVC) filters is commonly performed in selected patients with deep venous thrombosis and pulmonary embolism. However, filter placement is sometimes associated with serious complications. A common complication is asymptomatic perforation of the IVC and penetration of adjacent organs by the filter. Here, we report a case of an 83-year-old man whose prophylactic IVC filter penetrated the aorta. The patient was closely followed without surgical intervention for more than a year, and no additional complications were observed.

It was analyzed the results of treatment of 8 patients. Combined resection of liver and inferior vena cava was done in all cases. All resections of inferior vena cava were performed in combination with right-sided hemihepatectomy. Circular resection of inferior vena cava was done in 6 cases, tangential-in 2 cases. Allograft of donor inferior vena cava was used in 3 cases for reconstruction of inferior vena cava. Average duration of combined resection of liver and inferior vena cava was 675±189 min, average hemorrhage - 1800±1402 ml. The need for transfusion of packed red blood cells was 270±72
