Outcomes following the surgical management of left ventricular outflow tract obstruction; A systematic review and meta-analysis.

BACKGROUND: Left ventricular outflow tract obstruction (LVOTO) causes exertional symptoms in two thirds of patients with hypertrophic cardiomyopathy (HCM). Consensus guidelines recommend surgical intervention in patients with drug refractory symptoms. The primary aim of this study was to perform a systematic review and meta-analysis to determine morbidity and mortality after surgery. METHODS: Study Selection: Studies reporting outcomes following surgical intervention for symptomatic LVOTO in HCM. DATA EXTRACTION: Articles from searching two scientific databases (PubMed and Web of Science) we

Transient left ventricular outflow tract obstruction with systolic anterior motion of the mitral valve: A stunning cause.

Left ventricular outflow tract obstruction (LVOTO) and systolic anterior motion (SAM) of the mitral valve may have various etiologies, of which hypertrophic cardiomyopathy is the most common. More rarely, an acute coronary syndrome, myocardial stunning, and takotsubo cardiomyopathy may give rise to LVOTO and SAM. Here, we present a 70-year-old female patient with a non-ST-elevation acute coronary syndrome treated with percutaneous coronary intervention. Echocardiography the day after, because of dyspnea and hypotension, revealed apical akinesia, LVOTO, and SAM, which proved completely reversib

Dynamic left ventricular outflow tract obstruction secondary to hypovolemia in a German Shepard dog with splenic hemangiosarcoma.

Dynamic left ventricular outflow tract obstruction (DLVOTO) is a common condition in cats and humans. In this case report, a dog is described with DLVOTO secondary to severe intra-abdominal hemorrhage caused by a hemangiosarcoma. The dog was a 9-year-old, 35.7-kg, spayed female German Shepard dog that presented with a history of tachypnea and collapse. A Levine II/VI systolic murmur was present at the heart base. Abdominal ultrasonography revealed a splenic mass and a large amount of ascites. Echocardiography showed a reduced left ventricular diameter and an increased aortic velocity caused by

An accessory mitral valve leaflet causing left ventricular outflow tract obstruction and associated with severe aortic incompetence.

This case report describes a 20-year-old woman with Turner's syndrome who presented with reduced effort tolerance limited by dyspnea. She had previously been on pediatric cardiology follow-up for congenital subvalvular aortic stenosis first diagnosed at age 7. Unfortunately she defaulted after two visits before any intervention could be done. Transthoracic echocardiography demonstrated severe aortic incompetence (AI) with a membrane-like structure in the left ventricular outflow tract (LVOT). The mean pressure gradient across the LVOT on continuous wave Doppler was 41 mmHg. The membranous inte

Dynamic change of mitral apparatus as potential cause of left ventricular outflow tract obstruction in hypertrophic cardiomyopathy.

AIMS: the geometry of the mitral apparatus changes dynamically throughout systole and diastole. We investigated how geometric dynamics of the mitral apparatus could affect the haemodynamics of the outflow tract in patients with hypertrophic cardiomyopathy presenting with systolic anterior motion (HCM(SAM)) using three-dimensional (3D) echocardiography. METHODS AND RESULTS: we obtained transthoracic volumetric images in 21 patients with HCM(SAM) with differing trans-left ventricular (LV) outflow tract pressure gradient (PG(LVOT)) and in 23 controls. Original software was used to crop the 3D da

Left ventricular outflow tract obstruction in complete transposition of the great arteries--echocardiography criteria for surgical strategies.

BACKGROUND: The arterial switch operation (ASO) may be infeasible in patients with transposition of the great arteries (TGA) and left ventricular outflow tract obstruction (LVOTO). METHODS AND RESULTS: Patients with LVOTO were identified among the 229 patients with TGA repaired between 1991 and 2006. Dynamic LVOTO was defined as LVOTO without LVOT abnormalities requiring resection (group I, receiving ASO). Fixed LVOTO was defined as LVOTO with LVOT abnormalities that could (group IIa, receiving ASO) or could not be resected (group IIb, receiving operations other than ASO). Preoperative echoca

Dynamic left ventricular outflow tract obstruction without basal septal hypertrophy, caused by catecholamine therapy and volume depletion.

Hypertrophic cardiomyopathy (HCM) with hypertrophy of the basal septum is the most common etiology of left ventricular outflow tract (LVOT) obstruction. In this article, we report the case of a patient with a structurally normal heart who developed hemodynamic deterioration due to severe LVOT obstruction following treatment with catecholamines. Hypovolemia accompanied with a hyperdynamic condition, resulting from catecholamine treatment, may cause dynamic LVOT obstruction due to the systolic anterior motion of the mitral valve leaflet. The solution for this is early recognition and correction

[Left ventricular outflow tract obstruction in transposition of the great arteries. Correlation between anatomic and echocardiographic findings].

INTRODUCTION AND OBJECTIVES: Left ventricle outflow tract obstructions in transposition of the great arteries are frequent. We report the correlations between two-dimensional echocardiographic and autopsy findings to draw attention to the usefulness of this diagnostic method in the preoperative evaluation of these anomalies. MATERIAL AND METHODS: Of 73 hearts with transposition of great arteries, 26 specimens (38%) with different types of left ventricular outflow tract obstruction were selected to establish the relationship between an anatomical substrate of obstruction and echocardiographic

Left ventricular outflow tract obstruction in the transposition of great arteries defined by transthoracic three-dimensional echocardiography.

We assessed the feasibility of transthoracic three-dimensional reconstruction of the pulmonary valve and subpulmonary left ventricular outflow tract in two patients with transposition of great arteries, ventricular septal defect, and obstruction to the left ventricular outflow tract. Three-dimensional reconstruction of the pulmonary valve could be displayed as "en face" through a three-dimensional generated "pulmotomy view," allowing an overview of the pulmonary aspect of the valve from a surgeon's perspective. In similar fashion, reconstruction of the subpulmonary outflow tract could be displ

Fate of dynamic left ventricular outflow tract obstruction after anatomic correction of transposition of the great arteries.

Fourteen patients undergoing successful anatomic correction for transposition of the great arteries had subpulmonary gradients of 20 to 120 mm Hg (mean 40) across the left ventricular outflow tract before surgery. Ten patients had an intact ventricular septum, and four had an additional ventricular septal defect. In one patient obstruction was due to ballooning of the septal leaflet of the tricuspid valve through the ventricular septal defect. In the remaining patients obstruction was due to bulging of the interventricular septum plus or minus septal hypertrophy and with or without a fibromusc

Accessory mitral valve tissue causing left ventricular outflow tract obstruction in D-transposition of the great arteries.

Two patients with D-transposition of the great arteries (D-TGA) were found to have an unusual form of left ventricular outflow tract obstruction. Both had ball-like accessory mitral valve tissue that partially occluded the outflow tract. In one patient there was an intact septum, while in the other there were ventricular septal defects. The appearances have been described previously, although not in D-TGA. Recognition of accessory mitral valve tissue may allow resection of the tissue at the time of repair of the transposition complex.

Accessory mitral valve tissue: a differential diagnosis of an obstructive mass on the left ventricular outflow tract.

Accessory mitral valve tissue is a rare congenital cardiac anomaly that is typically discovered incidentally during echocardiographic evaluation prompted by an asymptomatic murmur. This pathology has characteristic echocardiographic elements and is usually associated with other CHD. The decision to perform surgical resection depends on factors such as the degree of obstruction, presence of symptoms, presence of other CHDs, and risk of thrombosis. The researchers hereby present a case of an asymptomatic paediatric patient with accessory mitral valve tissue that produced left ventricular outflow

Unruptured Sinus of Valsalva Aneurysm Obstructing the Left Ventricular Outflow Tract: An Uncommon Presentation in Childhood.

Congenital aneurysms of the sinus of Valsalva are uncommon abnormalities that are usually silent and slowly progressive without symptoms of cardiac dysfunction unless catastrophic rupture occurs. However, in rare cases, unruptured aneurysms can produce symptoms resulting from compression of adjacent structures, ventricular outflow tract obstruction, heart block, and coronary and valvular insufficiency. We report a case of a single unruptured sinus of Valsalva aneurysm producing left ventricular outflow tract obstruction in an 8-year-old boy who presented with chest pain on exertion.

Stenting for external outflow graft obstruction of a left ventricular assist device via a surgical axillary approach.

External outflow graft obstruction is an uncommon late complication in patients supported with a fully magnetically levitated left ventricular assist device. Progressive extrinsic compression of the outflow graft can impair pump performance and lead to symptomatic deterioration, requiring intervention to restore graft patency. This video tutorial presents a hybrid procedure combining surgical transaxillary access with endovascular stent implantation for treatment of external outflow graft obstruction when transfemoral access is not feasible. A 72-year-old woman with ventricular assist device s

Subvalvular Aortic Stenosis: Current Insights into an Often Overlooked Cause of Left Ventricular Outflow Tract (LVOT) Obstruction.

BACKGROUND: Subvalvular aortic stenosis (SAS) is an important cause of fixed left ventricular outflow tract obstruction and the second most common form of aortic stenosis. Although traditionally considered a congenital lesion, increasing evidence suggests that SAS often behaves as a progressive condition influenced by anatomical and hemodynamic factors. The disease may lead to significant complications, including left ventricular hypertrophy and progressive aortic regurgitation. MATERIALS AND METHODS: A narrative review of the current literature was performed to summarize contemporary knowled
