Actinomycosis of the vocal cord: a case report.

A 30-year-old Chinese lady was admitted for hoarseness of voice of one month's duration. Clinical examination revealed a granuloma of the left vocal cord while chest X-ray showed an opacity in the lower lobe of the right lung. The provisional clinical diagnosis was tuberculous laryngitis. A biopsy of the vocal cord lesion revealed inflamed tissue with actinomycotic colonies. Cultures and sputum smears did not reveal any tuberculous bacilli. The patient responded to a 6-week course of intravenous C-penicillin, regaining her voice on day 5 of commencement of antibiotics. A subsequent CT scan of

[Phonosurgical treatment of vocal cord cysts].

INTRODUCTION: Functional-traumatic lesions of the vocal folds include mucous stranding, "nodular" lesions, polyps, cysts, contact hyperplasia and haematoma [1]. They all appear at the predilection sites of the vocal fold, at the junction between the anterior and middle thirds, slightly below the free edge. Vocal fold cysts are also located at the predilection site. They interfere with the glottic closure and vibration process. The treatment consists of surgical enucleation. This paper is aimed to present our results with direct and indirect surgical procedures. MATERIAL AND METHOD: A series o

Electromyographic evaluation of vocal cord disorders.

In order to perform its basic functions, those of coordination of breathing, swallowing and speech, the larynx requires an intact neural apparatus to permit the perfect, well-coordinated action of the neuromuscular structures of extra- and intralaryngeal muscles. Electrophysiological measurements including electromyography, reflexmyography, electric and magnetic stimulated myography of the central motor nerve functions are the methods of classification of dysfunctions into neurapraxia, axonotmesis, neurotmesis, regeneration or myopathy. Different types of electrodes, depending on the methods o

[Phonosurgical treatment of vocal cord polyps].

INTRODUCTION: A vast majority of polyps appear at the predilection site of the vocal fold and therefore belong to the functional-traumatic group of vocal fold lesions. According to their size, vocal fold polyps may severely interfere with the glottal closure, and induce hoarseness, vocal fatigue, and frequently hyperkynetic pattern of voice production. MATERIAL AND METHOD: A series of 699 polyps was operated by direct microlaryngoscopy (DML), indirect microstroboscopy (IMS), and indirect videostroboscopu (IVS). Postoperative functional results were estimated by stroboscopy. Based on the funct

[Bilateral vocal cord paralysis in children].

Eighteen infantile cases with bilateral vocal cord paralysis were treated at our hospital from 1970 to 1993. All cases were diagnosed using a flexible fiberscope to examine the larynx. Direct laryngoscopy was performed under general anesthesia for the definite diagnosis and differential diagnosis from laryngomalacia, subglottic stenosis, tracheal stenosis, or laryngeal web. Bilateral vocal cord paralysis in children is a rare disease, there have been few and reported cases. Eight cases were male and 10 cases were female. Thirteen cases were congenital, 4 cases acquired and 1 case was unknown.

The complex movements of the vocal process during abduction and adduction of the vocal cords produce a severe mechanical strain on the mucous membrane covering this part of the glottis.

Clinical aspects of vocal cord nodules.

A clinical study of 251 patients with vocal nodules who underwent endolaryngeal microsurgery and speech therapy, was carried out. The postoperative results were evaluated. All patients were examined by videostroboscopy. An original acoustic method was applied for the estimation of the voice before and after therapy. Different patterns for vocal nodules were discussed.

[A simple method of the vocal cord laterofixation].

Six patients were treated by the new method of vocal cord latero-fixation which was the combination of the extra-laryngeal and intra-laryngeal approach. The functional contraindication for this procedure is the anatomic configuration of larynx.

Reversal of vocal cord palsy by tamoxifen.

Advanced breast carcinoma responds to tamoxifen in more than 30% of cases (Mouridsen et al., 1978). It is widely used for hormonal treatment in women suffering from breast carcinoma, particularly if they are post-menopausal (Carter, Bakowski and Hellmann, 1981).

Vocal cord paralysis with Ebstein's anomaly.

Left recurrent laryngeal nerve runs a long intrathoracic course in close relationship to the aortic arch and adjacent heart structures and, hence, is liable to be injured by diseases affecting the heart and its great vessels. We report a case of Ebstein's anomaly causing left recurrent laryngeal nerve paralysis in a neonate. To our knowledge, this is the first reported case of vocal cord paralysis associated with this condition.

Management of early vocal cord carcinoma.

Endoscopic stripping or laser therapy are the primary modalities for patients with premalignant vocal cord lesions and patients with T1 carcinomas of the mid-portion of the vocal cord. Partial laryngectomy is advocated for patients whose resection margins are positive following laser cordectomy, or where tumor extends to the arytenoid, anterior commissure, opposite vocal cord or subglottis. Radiotherapy is reserved for patients with tumors involving the interarytenoid region, professional voice patients, or patients unfit for general anesthesia.

[Neurinoma of the vocal cord. A case report].

Neurilemmoma is an extremely rare tumour of the larynx. A case of a 75-year-old woman who complained of increasing hoarseness for several months is reported. Clinically the tumour looks like a polyp of the left vocal cord. The tumour was completely removed endoscopically. Histological examination, however, revealed a neurilemmoma. To the present day only two cases of neurilemmoma of the vocal cord have been reported in the literature.

Vocal-cord dysfunction presenting as asthma.

We identified five patients with a functional disorder of the vocal cords that mimicked attacks of bronchial asthma. Paroxysms of wheezing and dyspnea were refractory to standard therapy for asthma. During episodes of wheezing, the maximal expiratory and inspiratory flow-volume relationship was consistent with a variable extrathoracic obstruction. Laryngoscopy confirmed that wheezing was due to adduction of the true and false vocal cords throughout the respiratory cycle. During asymptomatic periods the maximal flow-volume relationship and laryngoscopic examination were normal. Provocation test…

Vocal cord paralysis and thyroid surgery.

A technique used to identify the recurrent and superior laryngeal nerves using a flexible fiberoptic bronchoscope is presented. The author has used this technique in 12 cases and found it superior to other techniques that he has tried.

Hereditary abductor vocal cord paralysis.

Familial bilateral abductor cord paralysis was described in the father and two sons of a family in which the ramaining siblings (obe boy and three girls) were normal. The onset of stridor ranged from six months to nine years after birth in these patients who were all treated with a tracheostomy. Normal development and intelligence was experienced by all three patients. Since previous reports of hereditary abductor cord paralysis described mental retardation in all patients who were maintained without tracheostomy, anoxia from unrelieved laryngeal obstruction may be a significant complication f
