Value of four models for selecting patients for local excision of invasive squamous cell carcinoma of the vulva.

For 75 women with squamous cell carcinoma of the vulva who underwent radical vulvectomy and inguinofemoral lymphadenectomy, the authors assessed the efficacy of four models for selecting patients who could have been treated adequately with local excision of the tumor. Each of the three models proposed by Andreasson and Nyboe covered 25% of the patients, none of whom had groin metastases or died of cancer. Local recurrence in the vulva occurred in 10%. A model suggested by the International Society for the Study of Vulvar Disease covered almost 10% of the patients. One of seven patients had gro…

Vulvar dermatofibrosarcoma protuberans, an unusual anatomical location.

Vulvar dermatofibrosarcoma protuberans (DFSP) is a rare pathology. So far, only limited number of cases have been reported in literature. In … rethra and clitoris without definite invasion. The patient underwent a wide local excision, radical vulvectomy, and left inguinofemoral lymphadenectomy. Postoperatively, she experienced fever, vulvar swelling, and dysuria. Follow-up MRI demonstrated total resolution of the vulvar mass and collection with no recurrence. Histopathology identified the mass as D …

Vulvar Dermatofibrosarcoma protuberance in Iran and a narrative review of literature: A first case report.

BACKGROUND: Dermatofibrosarcoma protuberans (DFSP) is a soft tissue tumor that originates from deep layers of the dermis and mainly is common in young adults to middle age. This tumor is rare in the vulva. CASE PRESENTATION: A 53-year-old multiparous menopause female had complained of asymptomatic swelling of the right labia major. Dermatofibrosarcoma protuberans was reported in primary tumor resection. Six months later, rapid growing mass recurred in the vulva. The patient underwent radical Vulvectomy and resection of the margin of about 2-3 cm along with bilateral Inguinofemoral lymphadenec…

Use of fluorescence to guide surgical resection in vulvo-vaginal neoplasia: Two case reports.

We report hereby two cases of fluorescence-guided surgical resection with Indocyanine green in vulvo-vaginal neoplasia. The first case was a 86-year old patient who had a high-grade squamous intraepithelial lesion of the left small lip and on the vulvar fork. After a first incomplete surgery, a second fluorescence-guided vulvectomy was performed with ICG injected intravenously to determine intraoperatively surgical margins. At final pathologic examination, surgical margins were free of disease and postoperative course was uneventful. The second case was a 44-year old patient who had a clear ce…

Primitive neuroectodermal tumor originating in the vulva: A case report.

Primitive neuroectodermal tumors (PNETs) exhibit chromosomal translocations in common with those of Ewing's sarcoma. They usually originate in bone or soft tissue but rarely arise in the vulva. The current case report presents a case of PNET originating in the vulva in a 60-year-old female, who previously underwent enucleation of a vulvar tumor in another hospital. The pathologist suspected a histopathological diagnosis of PNET, and simple vulvectomy and resection of the inguinal lymph nodes were performed. An ~3 cm mass recurred in the right side of the vulva four years following the initial …

Metastatic melanoma of the vulva identified by peritoneal fluid cytology.

Malignant melanoma of the vulva is an uncommon disease, with a significant portion of cases demonstrating metastasis to inguinal lymph nodes with potential distal spread. Identification of such metastases often requires fine-needle aspiration or biopsy. The cytologic diagnosis of metastatic vulvar melanoma from peritoneal effusions has not been previously described. We present the case of a 54-yr-old woman who underwent en bloc radical vulvectomy with bilateral inguinal lymphadenectomy for melanoma of the right labium minora. No evidence of metastatic disease was identified, and all surgical m…

Dermal lymphangiomata of the vulva and laser therapy: a case report and literature review.

Dermal lymphangioma of the vulva is a rare disorder of the lymphatic system. The mainstay of therapy has been vulvectomy. A 65-year-old woman with a remote history of cervical cancer who successfully underwent CO2 laser vaporization for extensive vulva lymphangiomata, and a literature review inclusive of all 28 previously reported cases are described. Ten (35.7%) of these patients had previously received pelvic radiation for cervical cancer, 8 of whom (80%) had also undergone radical hysterectomy. Including the present case, 5 patients, 3 of whom had previously received radiation therapy, have…

Cytokeratin and vimentin expression in primary and recurrent carcinoma of the vulva: correlations with prognostic factors and the course of disease.

Radical vulvectomy for the treatment of a vulvar carcinoma inevitably entails severe psychosexual consequences for the patients. Thus, for such tumors, reliable histological prognostic parameters are needed to allow; when appropriate, the use of less radical operative measures. One possible approach to this problem might be to examine tumors immunohistochemically for the presence of cytoskeletal components. To assess the utility of this method, we applied a panel of antibodies directed against cytokeratins (CKs) and vimentin to a groups of vulvar carcinomas (62 primary and 35 recurrent tumors)…

Preservation of anal function after total excision of the anal mucosa for Bowen's disease.

Six women with Bowen's disease of the anogenital area were treated by total excision of the anal mucosa, perianal skin and, in some cases, partial vulvectomy. Two patients had foci of microinvasive squamous carcinoma. Adequate tumor margins were determined by frozen sections. The resulting mucosal and cutaneous defects were grafted with medium split-thickness skin grafts applied to the anal canal and sutured circumferentially to the rectal mucosa. Grafts were held in place by a finger cot inserted in the anal canal and stuffed with cotton balls. Patients were constipated five or six days with …

Primary vulval sebaceous carcinoma: rare case report and literature review.

This study presents a rare case of primary vulval sebaceous carcinoma (SC) and a literature review. Sebaceous carcinoma is an aggressive skin malignancy, predominantly periocular, with extraocular cases being particularly uncommon. We detail a 50-year-old female patient diagnosed with primary vulval SC, exhibiting a 0.5 cm white lesion on her left vulva. Despite initial drug therapy, the lesion progressed to ulceration and induration. Histopathological examination confirmed SC. The patient underwent extensive local vulvectomy and sentinel lymph node biopsy, showing no evidence of metastasis. A…

Histologic prognosticators in stage I squamous cell carcinoma of the vulva.

Sixty-four cases of stage I vulvar squamous cell carcinoma were analyzed histologically to define a patient subset at minimum risk for recur … cal recurrence appeared more related to inadequate surgical margins than failure to perform radical vulvectomy. Carcinoma in situ with early stromal invasion represents a group at zero risk for nodal metastases. We recommend wide local excision for all stage I lesions. In general, omission of lymphadenectomy should be reserved for cases of carcinoma in situ …
