In the diagnosis of ulnar wrist pain, physical examinations including provacative tests are primary elements in identifying the site and nature of ulnar wrist pain. Imagings techniques are helpful for diagnosing occult fractures and distal radioulnar subluxation, but unsatisfactory for diagnosing ulnocarpal impaction syndrome, tears of the triangular fibrocartilage complex and lunotriquetral ligament, and joint mouse, imagings. Therefore, prior to surgical intervention arthroscopy is recommended for patients with persistent ulnar wrist pain that interferes with their daily activity.

We performed a neurological and radiological study of the wrists of 58 patients with Hansen's disease and 60 age-matched healthy control subjects. As more and more adults become physically active, primary care physicians are seeing an increasing number of wrist injuries. Effective treatment is based on (1) accurate diagnosis, (2) maintenance of bone and joint-surface alignment until healing of bone, soft tissue, or both occurs, and (3) institution of an appropriate rehabilitation program. The most common cause of wrist pain, due to neurological disorder is carpal tunnel syndrome. In aging patients with atrophy of the hand muscles it is to be differentiated from s.c. myelopathic hand, caused by denervation of anterior horn cells in conditions such as cervical spondylosis. The referred pain affiliated with palpable bands and trigger points in the muscles has to be also considered as origin of pain.

The authors have used a newly designed dorsal wrist splint on 65 patients since 1989. The splint base is made of Orthoplast. A technique and new instruments for intramedullary fixation of the wrist arthrodesis is described, based on the technique of Clayton, popularized by Nalebuff and Millender. This allows early mobilisation, secure fixation and simplicity of the technique. This paper describes a prosthesis for patients who have had a partial amputation of the hand, or who have congenital absence of all or part of the hand. The prosthesis incorporates a new concept whereby the grip is operated by flexion and extension of the wrist. A covering for the prosthesis has been developed using a silicone polymer which produces a lifelike flexible glove. Thirteen patients have so far been fitted with this type of prosthesis, which can give good function and cosmesis.
