Heart Rate Variability and Cerebral Autoregulation in Patients with Traumatic Brain Injury with Paroxysmal Sympathetic Hyperactivity Syndrome.

BACKGROUND: Severe traumatic brain injury (TBI) can lead to transient changes in autonomic nervous system (ANS) functioning and development of paroxysmal sympathetic hyperactivity (PSH) syndrome. Clinical manifestation of ANS disorders may be obscured by therapeutic interventions in TBI. This study aims to analyze ANS metrics and cerebral autoregulation in patients with PSH syndrome to determine their significance in early prognostication. METHODS: This single-center retrospective study investigated the relationship between changes in ANS metrics, cerebral autoregulation, and PSH syndrome. Ar

Oxygen Metabolic Stress and White Matter Injury in Patients With Cerebral Small Vessel Disease.

BACKGROUND: Chronic hypoxia-ischemia is a putative mechanism underlying the development of white matter hyperintensities (WMH) and microstructural disruption in cerebral small vessel disease. WMH fall primarily within deep white matter (WM) watershed regions. We hypothesized that elevated oxygen extraction fraction (OEF), a signature of hypoxia-ischemia, would be detected in the watershed where WMH density is highest. We further hypothesized that OEF would be elevated in regions immediately surrounding WMH, at the leading edge of growth. METHODS: In this cross-sectional study conducted from 2

White Matter Integrity in High-Altitude Pilots Exposed to Hypobaria.

INTRODUCTION: Nonhypoxic hypobaric (low atmospheric pressure) occupational exposure, such as experienced by U.S. Air Force U-2 pilots and safety personnel operating inside altitude chambers, is associated with increased subcortical white matter hyperintensity (WMH) burden. The pathophysiological mechanisms underlying this discrete WMH change remain unknown. The objectives of this study were to demonstrate that occupational exposure to nonhypoxic hypobaria is associated with altered white matter integrity as quantified by fractional anisotropy (FA) measured using diffusion tensor imaging and re

Traumatic midline subarachnoid hemorrhage on initial computed tomography as a marker of severe diffuse axonal injury.

OBJECTIVEThe objective of this study was to test the hypothesis that midline (interhemispheric or perimesencephalic) traumatic subarachnoid hemorrhage (tSAH) on initial CT may implicate the same shearing mechanism that underlies severe diffuse axonal injury (DAI).METHODSThe authors enrolled 270 consecutive patients (mean age [± SD] 43 ± 23.3 years) with a history of head trauma who had undergone initial CT within 24 hours and brain MRI within 30 days. Six initial CT findings, including intraventricular hemorrhage (IVH) and tSAH, were used as candidate predictors of DAI. The presence of tSAH wa

Application of diffusional kurtosis imaging to detect occult brain damage in multiple sclerosis and neuromyelitis optica.

Multiple sclerosis (MS) and neuromyelitis optica (NMO) are two common types of inflammatory demyelinating disease of the central nervous system. Early distinction of NMO from MS is crucial but quite challenging. In this study, 13 NMO spectrum disorder patients (Expanded Disability Status Scale (EDSS) of 3.0 ± 1.7, ranging from 2 to 6.5; disease duration of 5.3 ± 4.7 years), 17 relapsing-remitting MS patients (EDSS of 2.6 ± 1.4, ranging from 1 to 5.5; disease duration of 7.9 ± 7.8 years) and 18 healthy volunteers were recruited. Diffusional kurtosis imaging was employed to discriminate NMO and

Exploring transient global amnesia: a 14-year retrospective study of 142 cases.

BACKGROUND: Transient global amnesia (TGA) provides a unique perspective into the relationship between stress and memory formation. This study examines the clinical and diagnostic profiles of TGA and its mimics and reviews the current evidence to enhance understanding of its elusive pathophysiology. METHODS: We retrospectively reviewed 142 consecutive acute amnesia cases (January 2011 to July 2024) at a tertiary centre in Sydney. Cases were identified via diagnostic codes and confirmed through manual review. Patients were classified as either TGA or non-TGA according to established criteria.

Atypical Postpartum Psychosis and Neuropsychiatric Manifestations Coexisting With Global Cortical Atrophy.

The neuropsychiatric sequelae resulting from postpartum hemorrhage (PPH)-induced hypoxic-ischemic brain injury (HIBI) are sparsely documented, presenting a complex diagnostic and therapeutic challenge for clinicians. We report the case of a 47-year-old woman who experienced PPH 13 years before her current presentation. Her PPH resolved following a partial hysterectomy and multiple transfusions, resulting in a coma from which she awoke with vivid hallucinations and altered memory. These symptoms initially improved, although she never regained baseline function and continued to experience cognit

Dystonia associated with pontomesencephalic lesions.

Secondary dystonia is well known subsequent to lesions of the basal ganglia or the thalamus. There is evidence that brainstem lesions may also be associated with dystonia, but little is known about pathoanatomical correlations. Here, we report on a series of four patients with acquired dystonia following brainstem lesions. There were no basal ganglia or thalamic lesions. Three patients suffered tegmental pontomesencephalic hemorrhage and one patient diffuse axonal injury secondary to severe craniocerebral trauma. Dystonia developed with a delay of 1 to 14 months, at a mean delay of 6 months. T

Neuropathological diagnosis of vascular cognitive impairment and vascular dementia with implications for Alzheimer's disease.

Vascular dementia (VaD) is recognised as a neurocognitive disorder, which is explained by numerous vascular causes in the general absence of other pathologies. The heterogeneity of cerebrovascular disease makes it challenging to elucidate the neuropathological substrates and mechanisms of VaD as well as vascular cognitive impairment (VCI). Consensus and accurate diagnosis of VaD relies on wide-ranging clinical, neuropsychometric and neuroimaging measures with subsequent pathological confirmation. Pathological diagnosis of suspected clinical VaD requires adequate postmortem brain sampling and r

Functional connectivity within lateral posterior parietal cortex in moderate to severe traumatic brain injury.

OBJECTIVE: Functional brain networks converge on areas of heteromodal processing such as lateral posterior parietal cortex (PPC). Traumatic brain injury (TBI) alters global connectivity patterns secondary to both focal and diffuse damage, but little is known about how it impacts regional environments. We examined local PPC functioning in individuals with moderate-severe TBI and controls during resting-state functional magnetic resonance imaging (rs-fMRI). METHOD: Eighteen individuals with moderate-severe TBI and 19 healthy controls underwent rs-fMRI and neurocognitive testing. Seed-based anal

Specific relations between neurodevelopmental abilities and white matter microstructure in children born preterm.

Survivors of preterm birth have a high incidence of neurodevelopmental impairment which is not explained by currently understood brain abnormalities. The aim of this study was to test the hypothesis that the neurodevelopmental abilities of 2-year-old children who were born preterm and who had no evidence of focal abnormality on conventional MR imaging were consistently linearly related to specific local changes in white matter microstructure. We studied 33 children, born at a median (range) gestational age of 28(+5) (24(+4)-32(+1)) weeks. The children were recruited as infants from the Neonata
