Time Course of Neoangiogenesis After Indirect Bypass Surgery for Moyamoya Disease : Comparison of Short-term and Long-term Follow-up Angiography.

OBJECTIVE: Indirect bypass surgery, which induces spontaneous neoangiogenesis in ischemic brain tissue and improves cerebral blood flow, is an effective treatment for moyamoya disease (MMD). The time course of neoangiogenesis development has not yet been investigated. This study aimed to determine the critical period of neovascularization after indirect bypass in MMD patients. METHODS: Patients with MMD who underwent indirect bypass surgery at Peking University International Hospital between January 2015 and October 2017 were retrospectively reviewed. Surgically treated hemispheres with short

Thirty-Day Outcome of a Multicenter Registry Study of Stenting for Symptomatic Intracranial Artery Stenosis in China.

BACKGROUND AND PURPOSE: Although recent trials have suggested that stenting is worse than medical therapy for patients with severe symptomatic intracranial atherosclerotic stenosis, it is not clear whether this conclusion applies to a subset of patients with hypoperfusion symptoms. To justify for a new trial in China, we performed a multicenter prospective registry study to evaluate the safety and efficacy of endovascular stenting within 30 days for patients with severe symptomatic intracranial atherosclerotic stenosis. METHODS: Patients with symptomatic intracranial atherosclerotic stenosis

Is Precise Targeting of the Recipient Artery in a Superficial Temporal Artery-Middle Cerebral Artery Anastomosis Crucial?

OBJECTIVE: To investigate the difference in treatment outcomes according to the method used to select the recipient artery in superficial temporal artery-middle cerebral artery (STA-MCA) anastomoses. METHODS: We retrospectively analyzed the cases of 35 patients who underwent an STA-MCA anastomosis for internal carotid artery/MCA occlusion or stenosis. Patients were divided into two groups based on whether the recipient artery was precisely targeted by single-photon emission computed tomography (SPECT group) or less precisely targeted by visual assessment (Visual group). Then the bypass result

Thrombolysis is an Independent Risk Factor for Poor Outcome After Carotid Revascularization.

BACKGROUND: Thrombolysis is the standard of care for acute ischemic stroke patients presenting in the appropriate time window. Studies suggest that the risk of recurrent ischemia is lower if carotid revascularization is performed early after the index event. The safety of early carotid revascularization in this patient population is unclear. OBJECTIVE: To evaluate the safety of carotid revascularization in patients who received thrombolysis for acute ischemic stroke. METHODS: The Nationwide Inpatient Sample database was queried for patients admitted through the emergency room with a primary

Safety and Efficacy of Surgical and Endovascular Treatment for Distal Anterior Cerebral Artery Aneurysms: A Systematic Review and Meta-Analysis.

BACKGROUND: Aneurysms of the distal anterior cerebral artery (DACA) are rare, representing between 1% and 9% of all intracranial aneurysms. The best treatment strategy for these aneurysms continues to be debated. OBJECTIVE: We conducted a systematic review of the literature to evaluate the safety and efficacy of treatment strategies of DACA aneurysms. METHODS: A systematic search of Medline, Embase, Scopus, and Web of Science was performed for studies published from January 2000 to August 2015. We included studies describing treatment of DACA aneurysms with ≥10 patients. Random effects meta-

Stepwise revascularization for prevention of postoperative hyperperfusion.

Abrupt normalization of cerebral blood flow (CBF) after surgical procedures to improve excessive cerebral hypoperfusion can cause irreversible brain parenchymal damage. Such hyperperfusion, which is caused by inflow at normal blood pressure into maximally dilated fine vessels, is an important complication following carotid endarterectomy (CEA). Strict control of blood pressure in the perioperative period can prevent this complication except in a few patients, who have severe cerebral hypoperfusion and poor cerebrovascular reserve due to extremely severe stenosis of the ipsilateral or the bilat

Impaired washout--embolism and ischemic stroke: further examples and proof of concept.

We previously posited that impaired washout of emboli was an important mechanism of brain infarction in patients with cerebral hypoperfusion. Hyperviscosity and hypoperfusion enhance thrombus formation promoting embolization of fresh thrombi. Hypoperfusion also reduces dissolution of emboli due to reduced flow velocity leading to classic hemodynamic patterns of stroke. To prove this concept further, we identified patients with severe hemodynamic compromise of either arterial or venous origin, or both, and natural or iatrogenic mechanisms of embolism. Three conditions were investigated in a str

Clinical features of moyamoya disease in sibling relations under 15 years of age.

This study was undertaken to define the clinical features of moyamoya disease in sibling relations less than 15 years of age. We analysed five pairs of siblings (6 boys, 4 girls) with moyamoya disease from among 56 moyamoya patients less than 15 years of age. Of 56 paediatric patients with moyamoya disease, 5 were sibling pairs. At onset of the disease, all patients were under 10 years of age. Clinical manifestations in the proband and the affected sibling tended to present as transient ischaemic attacks; none of the 10 patients presented with intracranical haemorrhage. The probands were not a

Physiologic studies of cerebral ischemia.

Using the technology of his day, limited to direct observation and histological techniques, Fisher conclusively established the importance of the atherosclerotic carotid plaque in stroke. Recognizing the limits of his observations, he raised a number of questions as regards the degree of carotid disease necessary to adversely affect the brain and the effects of silent carotid occlusion. He also suggested the possible beneficial effects of revascularization. These questions have been addressed and in large part answered by PET. Thresholds of electrical activity and of cellular viability have be

[An operated case of a giant fusiform aneurysm of the middle cerebral artery with alternating radiological findings within a short period].

We report an operated case of a giant fusiform aneurysm of the middle cerebral artery (MCA). An 18-year-old man presenting with a 6-month history of severe left temporal headache was admitted to our department. On admission, MRI revealed a mass lesion measuring 45 mm in diameter in the left frontal lobe. A left carotid angiogram revealed aneurysmal dilatation and stenosis in the M2 portion of the left MCA, which was diagnosed as a giant fusiform dissecting aneurysm. An intracarotid amobarbital test (Wada test)demonstrated ischemic tolerance to occlusion of the parent artery. Spontaneous occlus

[Coronary artery surgery in patients with diabetes mellitus].

Diabetes mellitus is present in 25-30% of patients undergoing coronary artery bypass grafts surgery. Early and late post-operative prognoses are different for the diabetic patient. Coronary artery bypass grafts are indicated in 2 or more vessel lesions, but it can also be preferred to percutaneous angioplasty in 1-vessel lesions, when this is the anterior descending artery or there is a great area under ischemia. Diabetic candidates to renal transplant must be investigated and revascularized pre-operatively, if necessary. Morbidity is greater in these patients, mainly due to respiratory, renal
