Radioimmunoscintigraphy (RIS) and radioimmunoguided surgery (RIGS) were assessed for their usefulness in patients with colorectal carcinoma. Based on a retrospective analysis of 100 penetrating duodenal injuries, the role of primary repair or resection and anastomosis was assessed prospectively in 66 patients (1986-1992). Duodenal exclusion was reserved for extensive combined pancreato-duodenal injuries. Seven of the 66 patients died from extensive abdominal trauma (mean Abdominal Trauma Index, ATI 70) within 48 hours of admission. One hundred and seventy patients with intra-abdominal infection with non-sporing anaerobes were prospectively studied in an international multicentre study. We present this unusual case of a 60-year-old gentleman who presented with meningism and reduced conscious level. Imaging demonstrated a perforated sigmoid colon with retroperitoneal air associated with pneumorrhachis and pneumocranium. He required a Hartmann's procedure and broad spectrum intra-thecal antibiotics which led to resolution of the pneumorrhachis and pneumocranium.

Occult enterorrhagia in infancy may become obscured and lead to diagnostic dilemma. Abnormal findings on investigative modalities, if not properly interpreted and clinically correlated, might complicate the matter leading to missed diagnosis and chronic anaemia. Occult enterorrhagia is an uncommon presentation of intestinal duplications. We describe the intriguing case of chronic anaemia in an infant, where concomitant endoscopic findings misled the diagnosis of jejunal duplication cyst as the cause of enterorrhagia.

We performed a structured retrospective chart review to describe clinical characteristics of Emergency Department (ED) patients diagnosed by history, physical examination, and abdominal computed tomography (CT) scan with epiploic appendagitis (EA). EA is a disease caused by inflammation of the appendix epiploica, subserosal adipose tissue along the colon. It may mimic surgical causes of acute abdominal pain, but is treated conservatively with pain management. There were 19 patients diagnosed with EA, with follow-up performed on 85%. BACKGROUND: The aim of this study was to review our experience with laparoscopic sigmoid colectomy for diverticular disease.
METHODS: All patients presenting with acute or chronic diverticulitis, obstruction, abscess, or fistula were included. Symptomatic diverticular disease was the main surgical indication (95%).

This case demonstrates the importance for surgeons to use the endoscope. A multimorbid patient in nearly dying condition, with a megacolon due to a functional sigmastenosis could be successfully treated by endoscopic intervention. After the patients recompensation, an early elective sigmaresection could be performed. The opportunity of endoscopy, instead of operation helped the patient and improved quality of treatment also economically. Therefore, a plea ist made for more endoscopic education in general surgical training.

Colonscopy performed in five patients because of narrowing of the sigmoid colon with intramural diverticula revealed several bright red, slightly elevated patches. In three patients, some bright red polypoid structures 1 cm in size and hemispherical in shape were also present. On light microscopy the polyps showed the features of transitional mucosa but revealed a different pattern of mucus secretion.
