Preliminary validation of the IOF QLQ and comparison with the SF-36 in patients after a distal radius fracture.

AIM: The aim of this study was to report preliminary validation data on the Polish version of the International Osteoporosis Foundation Quality of Life Questionnaire (IOF QLQ) for patients with a distal radius fracture (DRF). MATERIALS AND METHODS: Patients were eligible if they were between 18-80 years and were within 1-3 days after a non-comminuted DRF. All patients filled out the Polish version of the IOF QLQ, the Short Form 36 (SF-36) and a demographic questionnaire. Assessment points were set as soon as possible after the fracture, 7 days, 6 weeks, and 3 months after the fracture. Standa

Anatomical variations of the proximal radius and their effects on osteosynthesis.

PURPOSE: In fractures of the radial head and neck requiring open reduction and internal fixation, osteosynthesis may be safely applied in a limited zone. We conducted a morphometric study of the proximal radius at the level of the safe zone to identify different morphologic types of this anatomical region. METHODS: We analyzed 44 dried cadaveric radii. We measured the whole length of the radius, the length of the neck and head, and the minimum and maximum diameter of the radial head. The morphologic aspect of the neck-head curvature of the safe zone was evaluated qualitatively and quantitativ

Effect of the sequence of lateral osteotomy and hump removal on the aesthetic outcome.

BACKGROUND: The authors hypothesized that by modifying the sequence of the rhinoplasty procedure they could avoid comminuted fractures resulting in suboptimal cosmetic results. METHODS: Percutaneous perforated lateral nasal osteotomies were performed in 36 fresh human cadaver heads. In the first group of 19 cadaver heads with big nasal humps, the perforations of the lateral wall were done before and digital infracturing after hump removal in 10 cases (subgroup A1). In the remaining 9 noses with big humps, the perforations and digital infracturing were performed after hump removal (subgroup gr

Treatment of type C3 distal radius fracture resulted from high-energy injuries by volar plate in combination with external fixator.

BACKGROUND: With a type C3 distal radius fracture it is extremely difficult to maintain the reduction and to restore congruity of the articular surface because the support for the volar and dorsal bone cortex has been lost. An external fixator crossing wrist in combination with Kirschner wire (K-wire) fixation was popularly used by most studies in recent years. But loss of reduction often occurred especially in the volar sides. METHODS: A total of 30 cases of type C3 distal radius fracture were treated by a volar buttress plate combined with a transarticular external fixator and other techniq

[Marburg shoulder radiography splint (MSR splint) for standardized and high quality plain film radiography in fractures of the proximal humerus].

To ensure safe, quick, pain-relieving, standardized, and reproducible high quality plain film radiography in fractures of the proximal humerus, the MSR splint was introduced into clinical practice. With the rectangular and completely radiolucent splint the shoulder radiographs are obtained in supine position by a sole radiographer. Two radiographs are taken in projection at 90 degrees to one another: the true anteroposterior and axillary views, the most important views for fracture visualization and assessment. The smooth flat bottom part of the splint glides easily across the X-ray table belo

Tibial Overgrowth and Ipsilateral Femoral Overgrowth after the Fracture of Tibial Shaft in Children.

BACKGROUND: Overgrowth of the limb often occurs after pediatric tibial shaft fractures; however, risk factors for this phenomenon differ across studies. In addition, overgrowth of the ipsilateral femur is not well known. This study aimed to analyze ipsilateral femoral overgrowth after pediatric tibial shaft fracture and to compare the risk factors for tibial and ipsilateral femoral overgrowth. METHODS: Among the 85 patients who were diagnosed with tibial shaft fractures between January 2012 and December 2022 under the age of 14 years and followed up for more than a year, 6 patients who had mi

Plate or Arthroplasty for complex Mason Type-III Radial Head Fractures? Mid-to-long term results from a blinded outcome assessor study.

BACKGROUND: The surgical treatment of radial head fractures like Mason type III comminuted radial head fractures is challenging and controversial. Whether to use open reduction and internal fixation (ORIF) or radial head arthroplasty (RHA) as treatment methods is under constant debate. METHODS: We retrospectively analyzed clinical and radiographic mid- to long-term results of 42 patients with a mean follow-up time of 4.56 years (1.92-7.58 years). Patients were grouped according to fragment numbers and the type of intervention performed (3-4 fragments vs. 4+ fragments; ORIF vs. RHA). At the fi

The potential protective effects of temporal bone pneumatization: A shock absorber in temporal bone fracture.

OBJECTIVES: We hypothesize that when temporal bone fractures occur, the pneumatic cells in the temporal bone are able to absorb most of the impact force during a traumatic event. This study aims to correlate the degree of pneumatization of the temporal bone with the severity of temporal bone fracture (TBF). METHODS: Charts and computed tomography scans representing 54 TBFs, diagnosed from 2012 to 2017 at a single tertiary hospital, were retrospectively reviewed. Temporal bone pneumatization (TBP) in the petrous apex and mastoid region was evaluated using previously published classification sy

Metaphyseal screw augmentation of the LISS-PLT plate with polymethylmethacrylate improves angular stability in osteoporotic proximal third tibial fractures: a biomechanical study in human cadaveric tibiae.

OBJECTIVES: The incidence of osteoporotic proximal tibial fractures has increased during the last 2 decades. A promising approach in osteoporotic fracture fixation is polymethylmethacrylate-based cement augmentation of implants to gain better implant purchase in the bone. This study investigates the biomechanical benefits of screw augmentation in less invasive stabilization system-proximal lateral tibial (LISS-PLT) plates in cadaveric extraarticular comminuted proximal tibial fractures (OTA-41-A3.3). METHODS: Standardized extraarticular proximal tibial fractures were stabilized with the LISS-

Radiography of the Distal Radius.

Imaging of the distal radius is of paramount importance in diagnosing injuries, indicating patients for operative or nonoperative treatment, and appropriately following patients for associated injuries and complications. Plain radiographs and fluoroscopy are the dominant imaging modalities in all settings because they are readily available and provide valuable information about the osseous structures and any instrumentation. Computed tomography can offer more information about implant placement and the level of comminution of the articular surfaces but is more expensive and exposes patients to

Patellar tendon reconstruction with semitendinosus-gracilis autograft.

We present a case of a 24-year-old, otherwise healthy, man who sustained a right knee injury after a fall. A small, comminuted inferior pole patella fracture with medial and lateral retinacular tears was encountered that required a small, nonarticular partial patellectomy and patellar tendon repair. An uneventful postoperative course was complicated by a fall onto a flexed knee and rerupture of the patellar tendon at 3 months following surgery. Intraoperatively, a significant tissue void was seen in the area of the prior repair. The patellar tendon was reconstructed with semitendinosus and gra

The Mt. Tyndall incident.

The authors describe the 53-hour rescue of a 6-foot, 1-inch tall, 250-pound hiker in the face of harsh environmental conditions in Sequoia National Park. This 43-year-old man fell 25 feet, injured his leg, and was noted to be hypothermic and hypovolemic. Weather, altitude, and the patient's size delayed and complicated his evacuation. After being carried down 1,500 vertical feet, he was hoisted into a hovering helicopter and flown to University Medical Center in Fresno, California. On arrival, the patient was determined to have a comminuted subtrochanteric right femur fracture, which was ultim

Pitfalls, errors, and complications in the use of locking Küntscher nails.

The addition of percutaneously inserted transfixion screws or internally deployed fins supplements the stabilization potential of the cloverleaf nail. The use of these systems increases the indications for reamed closed intramedullary nailing. Fractures with comminution or bone loss, malunions, and nonunions located outside of the isthmic region of the femur and the tibia can now be stabilized effectively with a Küntscher-type implant. These closed nailing techniques reduce blood loss, surgical stress, and complications associated with open reduction and offer the patient the advantages of ear

Surgeons Consider Initial Nonoperative Treatment With Potential for Future Conversion to Reverse Arthroplasty a Reasonable Option for Older, Relatively Infirm, and Less-Active Patients.

OBJECTIVES: Two recent developments favor initial nonoperative treatment of proximal humerus fractures among the older, relatively infirm, less-active patients that represent most patients with such fractures: (1) evidence of minimal benefit of open reduction, internal fixation over nonoperative treatment, and (2) evidence of the effectiveness of the reverse total shoulder arthroplasty. As one step in determining the feasibility of a strategy of initial nonoperative treatment of proximal humerus fracture among older, relatively infirm, less-active people, we performed a survey-based experiment
