[Clinical aspects and follow-up of dopamine-induced psychoses in continuous dopaminergic therapy and their implications for the dopamine hypothesis of schizophrenic symptoms].

We present the case reports of 11 Parkinsonian patients who developed acute psychosis under continuous dopaminergic stimulation. Psychopathologically, two of the patients mainly suffered from organic hallucinosis, while nine patients showed the clinical symptoms of delirium. The clinical course and psychopathological findings in these patients did not differ from other acute organic psychoses. However, the symptoms of these dopaminergically induced psychoses varied significantly from the psychopathological findings of paranoid schizophrenic patients who were regularly treated and evaluated in

Modulation of angiotensin II-mediated cardiac remodeling by the MEF2A target gene Xirp2.

RATIONALE: The vasoactive peptide angiotensin II (Ang II) is a potent cardiotoxic hormone whose actions have been well studied, yet questions remain pertaining to the downstream factors that mediate its effects in cardiomyocytes. OBJECTIVE: The in vivo role of the myocyte enhancer factor (MEF)2A target gene Xirp2 in Ang II-mediated cardiac remodeling was investigated. METHODS AND RESULTS: Here we demonstrate that the MEF2A target gene Xirp2 (also known as cardiomyopathy associated gene 3 [CMYA3]) is an important effector of the Ang II signaling pathway in the heart. Xirp2 belongs to the evol

Substitution of night-time continuous subcutaneous insulin infusion therapy for bedtime NPH insulin in a multiple injection regimen improves counterregulatory hormonal responses and warning symptoms of hypoglycaemia in IDDM.

In patients with insulin-dependent diabetes mellitus (IDDM) good glycaemic control confers an enhanced risk of hypoglycaemia. Nocturnal hypoglycaemia occurs frequently and contributes to the syndrome of hypoglycaemia unawareness. In order to avoid nocturnal hypoglycaemia we substituted night-time continuous subcutaneous insulin infusion (CSII) therapy in 14 patients with well-controlled IDDM using a multiple injection regimen for the more variable bedtime NPH insulin. During a stepwise hypoglycaemic clamp we studied the effect of this regimen on counterregulatory hormonal responses, warning sy

[Bupivacaine toxicity and propofol anesthesia : animal study on intravascular bupivacaine injection].

BACKGROUND: Several reports have confirmed the efficacy of Intralipid® (containing soya bean oil, egg phospholipids, glycerin and water) in the therapy of systemic local anesthetic intoxication. Pretreatment with Intralipid® shifted the dose-response to bupivacaine-induced asystole in rats. Whether intravenous anesthesia with propofol in the widely used medium chain triglyceride lipid emulsion increases the therapeutic range of systemically administered bupivacaine or not is unknown and was investigated in this study. METHODS: A total of 30 piglets aged 2-6 weeks and weighing 4.5-6.5 kg were

Impact of a computer-generated alert system on the quality of tight glycemic control.

PURPOSE: To assess the impact of a computer-generated blood glucose (BG) alert, generated by a Patient Data Management System (PDMS) and superimposed on a paper-based guideline, on tight glycemic control (TGC) in the intensive care unit (ICU). METHODS: TGC in the Leuven University Hospitals is performed by nurses using a paper-based guideline that allows anticipative, intuitive decision-making. An electronic alert was implemented on 1 August 2007 in which a pop-up appears in the PDMS at the following BG thresholds: >180, >110, 60-80, 40-60 and <40 mg/dl. The impact of this electronic alert wa

Continuous infusion of UHMWPE particles induces increased bone macrophages and osteolysis.

BACKGROUND: Aseptic loosening and periprosthetic osteolysis resulting from wear debris are major complications of total joint arthroplasty. Monocyte/macrophages are the key cells related to osteolysis at the bone-implant interface of joint arthroplasties. Whether the monocyte/macrophages found at the implant interface in the presence of polyethylene particles are locally or systemically derived is unknown. QUESTIONS/PURPOSES: We therefore asked (1) whether macrophages associated with polyethylene particle-induced chronic inflammation are recruited locally or systemically and (2) whether the r

Migration of pump for intrathecal drug delivery into the peritoneal cavity. Case report.

BACKGROUND: We report on a patient in whom a subfascially implanted pump for the intrathecal delivery of baclofen spontaneously migrated into the peritoneal cavity. CASE DESCRIPTION: A 54-year-old male patient presented with a refilling problem of a drug pump that had been implanted 16 months earlier subfascially through a right-sided incision 10 cm below the costal margin. Because we were unable to refill the pump even under fluoroscopy, we presumed a backward turning of the pump. At exploration, we found that the deeper part of the pocket had spontaneously eroded and had caused the migratio

Intranasal delivery of a thyrotropin-releasing hormone analog attenuates seizures in the amygdala-kindled rat.

PURPOSE: Thyrotropin-releasing hormone (TRH) is known to have anticonvulsant effects in several animal seizure models and is efficacious in treating patients with certain intractable epilepsies. However, the duration of TRH's action is limited due to low bioavailability and difficulty penetrating the blood-brain barrier (BBB). Since direct nose to brain delivery of therapeutic compounds may provide a means for overcoming these barriers, we utilized the kindling model of temporal lobe epilepsy to determine if intranasal administration of a TRH analog, 3-methyl-histidine TRH (3Me-H TRH), could s

Locally applied Simvastatin improves fracture healing in mice.

BACKGROUND: HMG-CoA reductase inhibitors, statins, are widely prescribed to lower cholesterol. High doses of orally administered simvastatin has previously been shown to improve fracture healing in a mouse femur fracture model. In this study, simvastatin was administered either subcutaneously or directly to the fracture area, with the goal of stimulating fracture repair at acceptable doses. METHODS: Femur fractures were produced in 70 mature male Balb-C mice and stabilized with marrow-nailing. Three experiments were performed. Firstly, 20 mice received subcutaneous injections of either simvas

Recombinant polio-rhinovirus immunotherapy for recurrent paediatric high-grade glioma: a phase 1b trial.

BACKGROUND: Outcomes of recurrent paediatric high-grade glioma are poor, with a median overall survival of less than 6 months. Viral immunotherapy, such as the polio-rhinovirus chimera lerapolturev, is a novel approach for treatment of recurrent paediatric high-grade glioma and has shown promise in adults with recurrent glioblastoma. The poliovirus receptor CD155 is ubiquitously expressed in malignant paediatric brain tumours and is a treatment target in paediatric high-grade glioma. We aimed to assess the safety of lerapolturev when administered as a single dose intracerebrally by convection

Lost in conversion: navigating the challenges of converting intrathecal opioids to oral equivalents-a case report and literature review.

BACKGROUND: Chronic intrathecal opioid use via intrathecal drug delivery system (IDDS), while effective in some patients, can lead to serious complications and side effects. Complications related to IDDS include mechanical/device malfunction, intrathecal catheter problems, infections, and provider errors. These issues often require urgent surgical intervention, IDDS removal, and abrupt discontinuation of the analgesic infusion. In these scenarios, the safe and effective transition to alternative opioid delivery routes such as intravenous or oral administration is critical. However, an accurate

Central venous pressure, rather than cardiac output, is associated with peripheral chemoreceptor sensitivity in heart failure patients with left ventricular assist device.

BACKGROUND: Heart failure (HF) patients experience elevated sympathetic tone and abnormal peripheral chemoreceptor (PChR) function (elevated sensitivity [PChS] and tonicity [PChT]), which can be attributed to a reduction in cardiac output (CO) and resultant hypoperfusion of the carotid bodies. Left ventricular assist devices (LVADs) are known to increase CO, and can be used as a unique model to examine the impact of acute hemodynamic changes on PChR. METHODS AND RESULTS: Fourteen HF patients with LVADs underwent assessment of PChS (using the transient hypoxia test) and PChT (using low-dose do

Hydrogen sulfide mitigates spatial learning deficits in angiotensin II-Induced hypertension via inhibition of endothelial NOX2 and neuroinflammation.

Hypertension (HTN), when prolonged, extends beyond the cardiovascular system, impairs neurovascular function, and progresses into cognitive impairment. Thus, it is essential to understand the mechanism that drives HTN-induced brain pathogenesis for devising novel therapy. This study explored spatial learning, vessels-associated microglia and neuroinflammation during angiotensin II (Ang II) induced HTN with the hypothesis that after HTN induction, endogenous H2S level contributes to neuropathology in HTN. Male Sprague Dawley rats received Ang II (600 ng/kg/min) or saline using an implanted osmo

Coronary artery spasm complicated by atrial septal puncture: A case report.

RATIONALE: The occurrence of ST-segment elevation on electrocardiogram (ECG) during transseptal puncture in atrial fibrillation procedures is relatively uncommon, and its pathogenesis remains unclear. In this report, we clearly identified severe coronary artery spasm during atrial septal puncture as one of the causes of ECG ST-segment elevation. PATIENT CONCERNS: A 66-year-old woman was admitted to the hospital due to "paroxysmal palpitations for 1 year, with worsening symptoms for 1 week." Both the admission ECG and previous ECGs diagnosed her with atrial fibrillation. Immediately after rout
