Four year experience with cholangiocarcinoma: a survey of patients, clinical presentation, management and prognosis in two hospitals.

Seventeen patients with cholangiocarcinoma diagnosed in Toa Payoh and Tan Tock Seng Hospitals from 1986-90 were studied retrospectively. There was a male preponderance (male:female = 12:5) with a mean age of 58 years (range 28-82 years). All presented with obstructive jaundice. Three had cholangitis. Biliary stones were associated in 3 (18%). Two patients (12%) had choledochal cysts. The level of obstruction was identified at the hilum in 12 (70.5%), lower third in 4 (23.5%) and at a choledochojejunostomy anastomosis in 1 (6%). Ultrasound and percutaneous cholangiography (PTC) were the commone

New perspectives on biliary atresia.

An investigation into the aetiology, diagnosis, and treatment of biliary atresia was carried out because the prognosis remains so poor.In an electron microscopical study no viral particles or viral inclusion bodies were seen, nor were any specific ultrastructural features observed. An animal experiment suggested that obstruction within the biliary tract of newborn rabbits could be produced by maternal intravenous injection of the bile acid lithocholic acid.A simple and atraumatic method of diagnosis was developed using(99) (m)Tc-labelled compounds which are excreted into bile. Two compounds, (

[Hemobilia. (Apropos of 5 cases)].

Hemobilia or hemorrhage through the biliary tract is increasing in frequency. With spreading knowledge of the syndrome, the diagnosis is now rarely missed. There is also a real increase due to traffic accidents which often cause liver injuries. The typical triade with digestive hemorrhage, biliary colic and jaundice should always arouse suspicion of hemobilia. Hepatic angiography is the best and most accurate diagnostic method. Accidental or operative trauma constitute the major cause of macroscopic hemobilia where as microscopic hemobilia is a very frequent symptom in gallstone-disease. In th

Biliary atresia: pathogenesis and treatment.

Biliary atresia is a disorder of infants in which there is obliteration or discontinuity of the extrahepatic biliary system, resulting in obstruction of bile flow. Untreated, the resulting cholestasis leads to progressive conjugated hyperbilirubinemia, cirrhosis, and hepatic failure. Biliary atresia has an incidence of approximately one in 10,000 live births worldwide. Evidence to date supports a number of pathogenic mechanisms for the development of biliary atresia. An infectious cause, such as by a virus, would seem most pausible in many cases. The clinical observation that biliary atresia i

Multiple small bowel obstructions due to unusual formation of biliary phytobezoars following laparoscopic cholecystectomy in a sickler patient: A case report, an atypical surgical approach.

INTRODUCTION & IMPORTANCE: Bezoar formation in gastrointestinal tract is relatively a rare condition. A phytobezoar is the most common type of bezoar (Kement et al., 2012 [6]). Biliary phytobezoar is an extremely rare condition and is usually reported in patients had previous biliary procedures or in presence of bilioenteric fistula (Albogami et al., 2018; Kement et al., 2012; Kim et al., 2006 [2, 6, 7]). CASE PRESENTATION: 35-year-old female with sickle cell anemia (SCA). On 2003 she had gallstones (GS) and obstructive jaundice. She was treated at that time by endoscopic retrograde cholangio

Hyperbilirubinemia in hospitalized patients: Etiology and outcomes.

There is little information on the differential diagnosis and prognosis of hospitalized patients with hyperbilirubinemia. Here, we hypothesized that hyperbilirubinemia in hospitalized patients is associated with specific diseases and outcomes. This retrospective cohort analysis included patients admitted to the Medical University of South Carolina with a total bilirubin >3 mg/dL from January 9, 2015 to August 25, 2017. Collected clinical data included demographics, primary diagnosis, Charlson Comorbidity Index (CCI), laboratory data, and clinical outcomes. We separated and analyzed the cohort

Perforin and granzymes work in synergy to mediate cholangiocyte injury in experimental biliary atresia.

BACKGROUND & AIMS: Biliary atresia represents obstructive cholangiopathy in infants progressing rapidly to cirrhosis and end-stage liver disease. Activated NK cells expressing Nkg2d have been linked to bile duct injury and obstruction by establishing contact with cholangiocytes. To define the mechanisms used by cytotoxic cells, we investigated the role of perforin and granzymes in a neonatal mouse model of rotavirus (RRV)-induced biliary atresia. METHODS: We used complementary cell lysis assays, flow cytometric analyses, quantitative PCRs and in vivo systems to determine the mechanisms of bil

Outcome for self-expandable metal stents in malignant gastroduodenal obstruction: single-center experience with 104 patients.

BACKGROUND: Malignant gastric outlet obstruction (GOO) leads to malnutrition and limits quality of life. Gastrojejunostomy has been the traditional treatment for GOO. Recently, the results of releasing duodenal obstruction with self-expandable metal stents (SEMS) have been encouraging. METHODS: After the exclusion of 13 patients with gastrojejunal or jejunal strictures and 1 patient with intraabdominal lymphoma, the authors palliated the malignant GOO in 104 patients with 130 SEMS at a single center during the years 1999-2007. RESULTS: The GOO was caused by pancreatic (n = 51), gastric (n =

The role of sonography in imaging of the biliary tract.

Sonography is the recommended initial imaging test in the evaluation of patients presenting with right upper quadrant pain or jaundice. Dependent upon clinical circumstances, the differential diagnosis includes choledocholithiasis, biliary stricture, or tumor. Sonography is very sensitive in detection of mechanical biliary obstruction and stone disease, although less sensitive for detection of obstructing tumors, including pancreatic carcinoma and cholangiocarcinoma. In patients with sonographically documented cholelithiasis and choledocholithiasis, laparoscopic cholecystectomy with operative

CT diagnosis of afferent loop syndrome.

OBJECTIVE: To report the computed tomography (CT) features of afferent loop syndrome which is often clinically unsuspected. MATERIALS AND METHODS: The CT studies of five patients with afferent loop syndrome were reviewed. These patients had undergone gastroenterostomy and other surgical procedures, for malignant neoplasms in four and for peptic ulcer in one. Patients presented between 4 months and 15 years (average 5.5 years) after surgery. Symptoms were acute in all patients, the most common symptom being abdominal pain. In addition three of the patients had chronic symptoms including recurr

Biliary obstruction exacerbates the hepatic microvascular inflammatory response to endotoxin.

Gram-negative sepsis is a serious complication for patients with obstructive jaundice. The present study was conducted to elucidate the response of hepatic microcirculation to endotoxin 2 weeks after bile duct ligation (BDL) or sham-operation in rats. Two hours after lipopolysaccharide (LPS) injection (1, 10, or 100 microg/kg, iv.), the hepatic microvasculature was examined using in vivo microscopy. BDL elicited increases in leukocytes adhering to the sinusoidal wall, swelling of sinusoidal endothelial cells as well as phagocytic activity of hepatic macrophages and a decrease in the numbers of

Pancreaticoduodenectomy after Laparoscopic Sleeve Gastrectomy with Preservation of Gastric Arterial Inflow: A Case Report.

INTRODUCTION: Obesity is an established risk factor for pancreatic ductal adenocarcinoma. As laparoscopic sleeve gastrectomy (LSG) has become increasingly prevalent, pancreatic surgeons may encounter malignancies in patients with a surgically altered gastric anatomy. Pancreaticoduodenectomy (PD) after an LSG is rare and requires meticulous preoperative planning to prevent ischemic and anastomotic complications. CASE PRESENTATION: A 47-year-old man with morbid obesity (BMI 59.1 kg/m2) underwent LSG. Twenty-three days later, obstructive jaundice led to the diagnosis of pancreatic head adenocarc

Reconsidering pancreatic masses: A case of IgG4-related autoimmune pancreatitis mimicking malignancy.

Autoimmune pancreatitis (AIP) is an uncommon, immune-mediated form of chronic pancreatitis characterized by lymphoplasmacytic infiltration, fibrosis, and frequently elevated serum IgG4 levels. Clinically and radiologically, AIP often mimics pancreatic ductal adenocarcinoma, posing a substantial diagnostic challenge that risks unnecessary surgical interventions. This article presents the case of a 53-year-old female patient with epigastric pain, obstructive jaundice, and a pancreatic head mass initially suggestive of malignancy. Detailed imaging (CT, MRI, MRCP), serological evaluation showing e

Extensive intraventricular hemorrhage, complicated by dengue-induced coagulopathy, initially misdiagnosed as gastroenteritis in a toddler: A case report.

Incomplete physical examinations especially omissions of Central nervous system (CNS) assessments remain a major cause of missed or delayed diagnoses of serious neurological conditions in febrile children with nonspecific symptoms. I report the case of a 22-month-old male initially presenting with prolonged fever, coffee-ground vomiting, nonbloody diarrhea, and recurrent convulsions, leading to a presumptive diagnosis of gastroenteritis with sepsis at Mwananyamala regional referral hospital. Despite initial management, his condition deteriorated. Subsequent evaluation at Muhimbili National Hos

[Role of Imaging in Autoimmune Pancreatitis].

Autoimmune pancreatitis is a specific form of chronic recurrent pancreatitis whose etiopathogenesis involves immunological mechanisms. Historically, two varieties of the disease have been described, each with clinical, biochemical and histological particularities. Recently, a third subtype has been described as a consequence of the adverse effects associated with immunotherapy, which is increasingly indicated for several tumors. The diagnosis of autoimmune pancreatitis is a challenge for professionals, in part because of its heterogeneous clinical presentation, which includes obstructive jaund

Hepaticocystic Duct in an Era of Laparoscopic Cholecystectomy.

The biliary tract is notorious for its variable anatomy. A persistent hepaticocystic duct with agenesis of common bile duct is a rare biliary anomaly that creates a diagnostic dilemma and can add to the operative difficulties. It is important to diagnose this anomaly preoperatively since the gallbladder forms an integral part of bilioenteric continuity and an inadvertent cholecystectomy can lead to a surgical catastrophe. If diagnosed, surgeons can plan definitive treatment in the form of biliary diversion. We present a case of a 22-year-old man, who presented to us with obstructive jaundice a
