Physical characteristics as risk factors for sports injuries: a four year prospective study.

A variety of physical characteristics of athletes has been proposed to be related to the risk to sustain a sports injury. The aim of the present study was to determine the influence of flexibility, anthropometric characteristics and malalignment of the lower extremities on the risk to sustain a sports injury, using a prospective study design and subjects exposed to rather equal extrinsic risk factors. Physical education students (N = 136) were followed during their four-year education. Sixteen flexibility indices, four anthropometric characteristics and five malalignment indices of the lower e

[Value of ultrasound of the support and locomotor system--with special reference to radiation exposure and cost reduction].

In recent years, ultrasonic assessment of the locomotory system has become established both as a screening method in patients with chronic disorders and for evaluating acute injuries. Thanks to technical improvements to the equipment (use of high-frequency probes up to 20 MHz), ever smaller structures can be visualized. Ultrasonography can be carried out immediately following the clinical- and possibly radiological-assessment, by one and the same investigator, and requires little extra time. A bilateral examination in particular permits good evaluation of the extent of an injury and, with appr

Review of typical ice hockey injuries. Survey of the North American NHL and Hockey Canada versus European leagues.

Ice hockey is considered to be one of the fastest and roughest of all sports. Prospective injury reports of the North American National Hockey League, the Canadian Amateur Hockey Association and of several European teams (UdSSR, CSSR, Sweden and Switzerland) are reviewed to evaluate the patterns, anatomic locations, circumstances and sequelae of ice hockey-related injuries. Although different injury reporting systems are used in North America and Europe, knee injuries (sprains of the collateral ligaments) accounted for the majority of games missed (40%), followed by injuries to the shoulder (d

Epidemiology of acute knee injuries: a prospective hospital investigation.

During a 1-year period, data on all acute injuries treated at emergency departments were entered into a registry; of these injuries, 6% involved the knee joint. The patients were followed until a definite diagnosis was established. The rate of anterior cruciate ligament injuries and meniscus tears was, respectively, 0.3 and 0.7 (injuries per 1,000 inhabitants per year). The rate of dislocations of the patellofemoral joints was equal to the rate of anterior cruciate ligament injuries. Ten percent of the cases involved ligament injuries, and isolated ruptures of the anterior cruciate ligaments w

Ultrasound assessment of medial meniscal extrusion: a validation study using MRI as reference standard.

OBJECTIVE. The purpose of this article is to validate both semiquantitative and quantitative ultrasound assessment of medial meniscal extrusion using MRI assessment as the reference standard. SUBJECTS AND METHODS. Ninety-three consecutive patients with chronic knee pain referred for knee MRI were evaluated by ultrasound and MRI on the same day. Two musculoskeletal radiologists assessed meniscal extrusion on ultrasound and MRI separately and independently and graded it semiquantitatively as follows: 0 (< 2 mm), 1 (≥ 2 mm and < 4 mm), and 2 (≥ 4 mm). Agreement between the ultrasound and MRI eval

Medial patellofemoral ligament anatomy: is it a predisposing factor for lateral patellar dislocation?

PURPOSE: Because the medial patellofemoral ligament (MPFL) is the primary restraint to lateral dislocation of the patella, we aimed, in this controlled study, to verify whether the MPFL with different measurements could be considered another predisposing factor for patellar dislocation. METHODS: A group of 100 consecutive individuals without the criteria for patellar dislocation (trochlear dysplasia, patella alta and lateral patellar tilt) was recruited as a control group and underwent magnetic resonance imaging (MRI) study and another group of 50 patients with patellar instability. Femoral c

Patella instability: building bridges across the ocean a historic review.

The diagnosis of and treatment for musculoskeletal disease and injuries have seen an explosion of new knowledge. More precise imaging, correlative injury anatomy, more focused physical examination features, among others, have led this upsurge of current insight. Crucial to this knowledge revolution is the expansion of international knowledge, which is aided by an adoption of a universal scientific language, electronic transfer of information, and personal communication of surgeons and scientists across national boundaries. One area where this is particularly evident is in our knowledge and tre

Isolated medial patellofemoral ligament reconstruction for recurrent patellar dislocation.

BACKGROUND: Several medial patellofemoral ligament reconstruction procedures have been proposed to manage recurrent patellar dislocation. HYPOTHESIS: Reconstruction of the medial patellofemoral ligament using a hamstring graft with a 2 transverse patellar tunnels technique is an effective treatment for recurrent patellar dislocation in patients without any evident predisposing factors. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Twenty-eight patients with chronic patellar instability without any anatomic predisposing factors, experiencing recurrent unilateral patellar dislocat

[Hypermobile snapping medial meniscus: features on flexion-extension MRI].

We report 3 cases of snapping medial meniscus due to hypermobility confirmed on extension and flexion MR imaging. The anterior horn of the medial meniscus was anteriorly subluxed during extension with reduction during flexion. Analysis of our imaging data and a review of the literature confirm that this relates to hypermobile medial meniscus, an anatomical variant.

Arthroscopic surgery for primary traumatic patellar dislocation: a prospective, nonrandomized study comparing patients treated with and without acute arthroscopic stabilization with a median 7-year follow-up.

BACKGROUND: No data exist whether patients with primary traumatic patellar dislocation benefit from initial arthroscopic medial repair surgery. PURPOSE: To compare long-term outcomes of patients treated with acute arthroscopic stabilization for patellar dislocation with those treated nonoperatively except for removal of loose bodies. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: The study group included 76 consecutive military recruits (72 men, 4 women), with a median age of 20 years (range, 19-22) at the time of dislocation. Thirty patients (group 1) underwent initial arthrosc

Knee locking due to a single gouty tophus.

Knee locking is a highly incapacitating condition attributable to mechanical or functional factors. Pain, mostly of capsuloligamentous or intraosseous origin, is the cause of functional locking. Meniscal injuries are the most frequent arthroscopic finding in the locked knee, due to mechanical factors. We describe a patient experiencing locking during extension whose history and examination suggested a tear of the anterior horn of the lateral meniscus of the right knee; a diagnostic/therapeutic arthroscopy revealed an intraarticular gouty tophus growing from the anterior horn of the lateral men

Meniscal subluxation: association with osteoarthritis and joint space narrowing.

OBJECTIVE: Since complete meniscectomy leads to knee OA, we investigated the potential links among meniscal subluxation, joint space narrowing and symptomatic OA. MATERIALS AND METHODS: 233 cases with symptomatic knee OA and 58 asymptomatic controls underwent radiography and MR imaging of the knee. Joint space narrowing was measured on weight-bearing PA fluoroscopy-positioned radiographs. The amount of medial or lateral meniscal subluxation was measured on coronal MR images. The prevalence and severity of meniscal subluxation was compared in cases and controls. We evaluated the correlation of

[Clinical and radiologic results of surgically treated patellar dislocation].

In this retrospective study 72 patients were followed up clinically and radiologically after surgical treatment for dislocation of the patella. The period of observation varied between 13 and 76 months, with an average of 32 months. Acute traumatic dislocation of the patella was only rarely encountered (n = 6 [8%]). Primary dislocation occurred most frequently during the second and third decades of life. Chondral and osteochondral fractures were fairly frequent (n = 24 [33%]). Additional injuries involved the anterior cruciate ligament, the medial collateral ligament and the menisci. At clinic

Experimental hyaline cartilage lesions: two-dimensional spin-echo versus three-dimensional gradient-echo MR imaging.

The value of magnetic resonance (MR) imaging, with two-dimensional (2D) spin-echo and FISP (fast imaging with steady-state precession) and FLASH (fast low-angle shot) three-dimensional (3D) gradient-echo sequences, for the detection of hyaline cartilage defects of the femoral condyle and the tibial plateau, was investigated in an animal model. In eight dogs, the anterior cruciate ligament was transected in one knee joint, resulting in rapid development of osteoarthritis with degeneration of the hyaline cartilage. At autopsy, 24 cartilage lesions were found, which were classified into four grad

Three dimensional imaging of bone from analysis of computed tomography data.

A computer system was developed to reconstruct three dimensional images of bone from analysis of computed tomography data. Images of bone can be made within minutes and rotated for viewing from any direction. An editing process allows visualization of the articular surfaces of any joint. Solid models of the bone images can be produced with an accuracy of 1 mm to 3 mm by interfacing the image data with a computer numerically controlled milling machine. This technology will provide better information to the surgeon for preoperative diagnosis and planning and for the design of customized implants

Trochlear Morphology Development: Study of Normal Pediatric Knee MRIs.

INTRODUCTION: Trochlear dysplasia is a known risk factor for patellar dislocations yet normal trochlea development is not well described. This study will define the articular cartilage (AC) and subchondral trochlear morphology development in pediatric patients using magnetic resonance imaging (MRI) evaluation. METHODS: A retrospective knee MRI review included patients aged 3 to 16 years with nonpatellofemoral-related diagnoses. International classification of diseases-9/International classification of diseases-10 codes were used to identify eligible study patients. Measurements of the trochle
