BACKGROUND: Chronic ankle instability (CAI) is associated with abnormal joint loading and an increased risk of early joint degeneration, yet the underlying biomechanical mechanisms remain unclear. It is unknown whether altered ankle loading reflects chronic effects of repeated ankle sprains rather than a single traumatic event, and how muscle forces contribute to these differences.
METHODS: Ankle joint contact forces (JCF) and muscle contributions during walking were compared among 21 uninjured controls, 21 copers (individuals with a prior ankle sprain but no persistent symptoms), and 21 indi

Necrotizing fasciitis is a rapidly progressive, life-threatening infection originating in a fascial plane which requires emergency surgery. Although uncommon, with an incidence of 0.63 per 100,000 per year, it affects the limbs in 40% of cases and carries a 22% 30-day mortality rate. The diagnosis is principally clinical. Severe pain, out of proportion to any visible abnormality of the skin, is the cardinal early feature, frequently accompanied by tenderness beyond the area of erythema, swelling, and local hypoaesthesia. Crucially, the skin may initially appear normal or nearly normal while ra

BACKGROUND: Rheumatology outpatient clinics are facing increasing referral volumes, including new and long-term patients. International studies suggest that initial assessments by non-rheumatologists can be effective.
AIM: This study evaluates the role of a general medicine clinic in managing patients initially referred to rheumatology at an Australian tertiary hospital.
METHODS: We retrospectively reviewed rheumatology referrals diverted to the general medicine clinic at The Northern Hospital, Victoria, from 1 January 2023 to 30 June 2024. Data included patient demographics, referral reason

BACKGROUND: Amyloidosis is a systemic disease characterized by buildup of abnormal amyloid deposits that may lead to multiple organ failure and is associated with early onset musculoskeletal manifestations. Orthopedic surgeons are positioned to aid with early diagnosis in the form of a biopsy which allows for workup for other areas of disease, specifically cardiac manifestations, and intervention prior to irreversible cardiac damage. In this review, we describe the biopsy results of a series of 182 patients and workup and treatment of the 46 amyloid positive patients biopsied by a single surge

PURPOSE: Pelvic obliquity (PO) has not been extensively investigated, and there is no gold standard for measurement. The PO is essential for surgeons in planning hip arthroplasty, which includes the restoration of leg length discrepancy (LLD). We aimed to establish a normative range of PO angles by measuring healthy individuals without musculoskeletal disorders.
METHODS: Our study included 134 consecutive cases (70 females) referred to our institution between April 2020 and September 2021 for non-orthopedic problems. Patients were screened for normal gait and posture using the visual observat

BACKGROUND: Spina bifida is a congenital malformation involving an open vertebral column resulting from failure in neural tube closure. It is among the most frequently occurring birth defects, observed in 1-3 cases per 1,000 live births worldwide. Conventionally requiring surgical repair, it can cause severe neurologic and musculoskeletal complications. However, consumption of prophylactic folic acid in mothers, at least 3 months before to 12 weeks after conception (periconceptional) has been shown to reduce the incidence of spina bifida by approximately 75%. This makes ascertaining parental u

BACKGROUND: Prolonged and repeated sensorimotor training is a crucial driver for promoting use-dependent plasticity, but also a main risk factor for developing musculoskeletal pain syndromes, yet the neural underpinnings that link repetitive movements to abnormal pain processing are unknown.
METHODS: Twenty healthy musicians, one of the best in vivo models to study use-dependent plasticity, and 20 healthy non-musicians were recruited. Perceptual thresholds, reaction times (RTs) and event-related potentials (ERPs) were recorded using nociceptive intra-epidermal and non-nociceptive transcutaneo

BACKGROUND: Monitoring frailty indicators in elderly people is recommended to identify those who could benefit from disability prevention programs. To contribute to the understanding of the development of frailty in the elderly, we have created the FREEDOM-LNA cohort constituting an observational study of ageing in general population. Here, we described the characteristics of a cohort of elderly subjects who are followed for determination of frailty and loss of independence trajectories.
RESULTS: The cohort was composed of 1085 subjects in advanced age (mean: 83.7 ± 6.0 years) and of women in

BACKGROUND: While risk factors have been published for readmissions following primary total joint arthroplasty, little is known about the etiology of those costly adverse events. In this study, we sought to identify the reasons for 30-day readmission following primary total joint arthroplasty in a contemporary national patient sample.
METHODS: The American College of Surgeons National Surgical Quality Improvement Program was queried to identify 367,199 patients who underwent primary total knee (TKA) or hip arthroplasty (THA) between 2011 and 2018. The primary outcomes were the annual rates of

Crutches are commonly prescribed in physiatric settings to help offload the lower extremities. Crutch use, however, results in abnormal and repetitive weight-bearing through the upper extremities that may result in secondary injury. This narrative review investigates injury types and risk factors associated with crutch use in order to guide healthcare providers on injury prevention strategies. Medline, EMBASE, CINAHL, and the Cochrane Library were systematically reviewed for publications between 1950 and 2018 on neurological, musculoskeletal, or vascular complications associated with crutch us

Joint contracture is one of the common musculoskeletal disorders. It has seriously disturbed patients' activities of daily living in various aspects. The pathogenesis of it is eager to explore to distinct degree. Nowadays the thickeness and fibrosis of joint capsular is redarded as the major reason to joint contracture. It is reported that excessive fibroblasts and myofibroblasts activity, collagen hyperplasia, and extracellular matrix (ECM) deposition in these fibrotic condtions lead to the contracture. In addition, upregulators of myofibroblast and collagen synthesis, transforming growth fac

BACKGROUND Unbalanced standing and gait asymmetry are common in individuals with musculoskeletal disorders. Achieving symmetrical posture and gait is an important goal of rehabilitation. This study investigated the biomechanical differences in the lower extremities observed immediately after an insole was used and without the use of different one-sided insoles. MATERIAL AND METHODS Thirty young, healthy adult males received 3 different insole interventions: experimental group A had a customized 3-dimensional (3D)-printed single-sided lateral wedge insole (CLWI) inserted on the left side, and e

BACKGROUND: Sternocleidomastoid anomalies are mostly discovered in cadavers during routine dissection. Such anomalies causing torticollis are rare in adults. We report a case of accessory mid-clavicular head of the sternocleidomastoid causing torticollis in an adult.
CASE: A previously healthy 27-year-old man presented with neck and shoulder pain following heavy manual work. On examination, he had mild restriction of left tilt of the head and right rotation of the neck. Palpation revealed a bipartite right sternocleidomastoid. There was no abnormal posturing of other body parts. The range of

BACKGROUND: The presence of pain as a harbinger of bearing-related problems has recently been challenged. Adverse local tissue reactions (ALTRs) have been noted on cross-sectional imaging even in asymptomatic patients. The purpose of this study was to determine the natural history of such lesions in asymptomatic patients.
METHODS: Eighty-three asymptomatic patients with modular metal-on-metal total hip arthroplasties underwent metal ion reports and metal artifact reduction sequence magnetic resonance imaging (MARS MRI). MARS MRI images were reviewed and evaluated for the presence or absence o
