Demographic prompt cues shift clinical recommendations in multimodal large language models: a multi-model audit of 25,380 dental radiograph assessments.

BACKGROUND: Multimodal large language models (LLMs) are increasingly being evaluated for clinical image interpretation, but whether patient demographic cues influence their recommendations remains unclear in dental radiology-a leading domain for AI-assisted diagnostics, and one marked by pronounced disparities in oral health. METHODS: We conducted a within-image, between-condition audit study using 705 panoramic dental radiographs from the publicly available DENTEX dataset. Each image was submitted to three commercial multimodal LLMs (Gemini 2.5 Flash, GPT-5.4, Claude Sonnet 4.6) under 12 exp

AI assisted reader evaluation in acute CT head interpretation (AI-REACT): protocol for a multireader multicase study.

INTRODUCTION: A non-contrast CT head scan (NCCTH) is the most common cross-sectional imaging investigation requested in the emergency department. Advances in computer vision have led to development of several artificial intelligence (AI) tools to detect abnormalities on NCCTH. These tools are intended to provide clinical decision support for clinicians, rather than stand-alone diagnostic devices. However, validation studies mostly compare AI performance against radiologists, and there is relative paucity of evidence on the impact of AI assistance on other healthcare staff who review NCCTH in t

Beyond the white line: a multicentre retrospective analysis of radiographic and clinical predictors of inferior alveolar and lingual nerve injury following mandibular third molar surgery.

BACKGROUND: Inferior alveolar nerve (IAN) and lingual nerve (LN) injury remain among the most significant complications of mandibular third molar surgery. Earlier studies relied primarily on panoramic predictors such as interruption of the white line (IoWL), often without three dimensional validation. This multicentre retrospective analytical study aimed to identify independent predictors of neurosensory injury and to develop and internally validate a composite risk prediction model. METHODS: Records of 2,850 third molar extractions performed between January 2019 and December 2024 across three

Hemodynamic Study of Plaque Progression and Regression Based on Coronary CTA Imaging using Computational Fluid Dynamics Method: Preliminary Results.

This study evaluated coronary computed tomography angiography (CCTA)-based computational fluid dynamics (CFD) for predicting plaque dynamics in coronary artery disease. We retrospectively analyzed 22 patients (34 lesions) with paired CCTAs (mean interval 2 years). Lesions were categorized as progression (increase in diameter stenosis ≥ 5%), stable (change within - 5% to 5%), or regression (decrease in diameter stenosis ≥ 5%). Hemodynamic indices were normalized to adjacent non-diseased segments. Logistic regression identified predictors: normalized minimum wall shear stress (odds ratio (OR) =

Development of a score for assessment of radiologic damage in large-vessel vasculitis (Combined Arteritis Damage Score, CARDS).

OBJECTIVE: The authors hypothesized that it is safe to combine local infiltration analgesia (LIA) in total knee arthroplasty (TKA) with a retransfusion drain since ropivacaine concentrations would not exceed the arterial toxicity threshold concentrations of 4.3 mg/L for total and 0.56 mg/L for unbound ropivacaine. MATERIALS AND METHODS: 22 patients scheduled for primary TKA were included. During surgery three peri-articular injections with ropivacaine (300 mg) were given. Plasma and shed blood samples were taken at 0, 1, 3, 6, 7, and 24 hours postoperatively. RESULTS: At 6 hours postoperativ

Dynamics of homemade aortic endografts: in vivo study in humans with computed tomography scanner modeling.

BACKGROUND: Several cases of aortic endograft rupture have been described. In most cases, they stem from component wear and perforation of the graft, leading to leakage. Friction of the stents on the graft can cause abrasion and perforate the textile. This friction results from movements inside the endograft implanted in the aorta exposed to blood flow, arterial pressure, and the movements of the aorta itself. METHODS: To study in vivo the movements of homemade stent grafts (HMSGs) designed and constructed by the surgeons at La Pitié Salpêtrière Hospital (Paris), the displacements of the meta

Distribution of coronary artery calcium volume and density by age, sex, and race using AI-based quantification algorithm.

BACKGROUND: The Agatston score is commonly used for coronary artery calcium (CAC) quantification which integrates calcium volume and density into a single measure. Recent studies suggest CAC volume and mean density offer independent prognostic value. However, normative distributions of these parameters across demographic subgroups are not well characterized. METHODS: A total of 23,844 patients were analyzed who underwent non-contrast ECG-gated cardiac CT at 120 kVp from 2010 to 2023 at our institution. Patients included were older than 35, asymptomatic, and without prior atherosclerotic car

Automatic CT-based quantification of myocardial extracellular volume fraction and high ECV burden and their prognostic value for death and heart failure in patients with aortic stenosis: a real-world data analysis.

BACKGROUND: Computed tomography (CT)-based myocardial extracellular volume (ECV) assessment during transcatheter aortic valve implantation (TAVI) planning is prognostically informative but limited by operator-dependent workflows. We developed a fully automated, standardized pipeline for volumetric ECV quantification and a derived metric reflecting the burden of myocardial extracellular matrix expansion (hECV-B) and evaluated their association with clinical outcomes after TAVI. METHODS: In a single-centre study of consecutive pre-TAVI cardiac CT examinations (10/2020-12/2023), a standardized p

Patient Age and Hip Morphology Alter Joint Mechanics in Computational Models of Patients With Hip Dysplasia.

BACKGROUND: Older patients (> 30 years) undergoing periacetabular osteotomy (PAO) to delay THA often have inferior patient-reported outcomes than younger adult patients (< 30 years). It is unclear how patient age affects hip morphology, mechanics, or patient-reported outcome scores. QUESTIONS/PURPOSES: (1) Is increased patient age associated with computationally derived elevations in joint contact stresses? (2) Does hip shape affect computationally derived joint contact stresses? (3) Do computationally derived joint contact stresses correlate with visual analog scale (VAS) pain scores evaluat
