A new biomechanical classification system for split fractures of the humeral greater tuberosity: guidelines for surgical treatment.

BACKGROUND: Split fractures of the humeral greater tuberosity (HGT) are common injuries. Although there are numerous surgical treatments for these fractures, no classification system combining clinical and biomechanical characteristics has been presented to guide the choice of fixation method. METHODS: We created a standardised fracture of the HGT in 24 formalin-fixed cadavers. Six were left as single-fragment fractures (Group A), six were further prepared to create single-fragment with medium size full-thickness rotator cuff tear (FT-RCT) fractures (Group B), six were cut to create multi-fra

Acute Respiratory Distress and Hyperchloremic Metabolic Acidosis as a Result of Massive Irrigation Fluid Extravasation After Arthroscopic Shoulder Surgery: A Case Report and Recommendations for Preventable Complications.

BACKGROUND Airway compromise caused by massive fluid extravasation in association with arthroscopic shoulder surgery is rare. However, it is even more rare to occur as a result of pleural effusion. We present this case to increase the awareness of this rare complication and show how to minimize the likelihood of this uncommon incident. CASE REPORT We describe a case of a 68-year-old woman who underwent elective shoulder arthroscopy for rotator cuff repair. She had immediate postoperative shortness of breath and was found to have bilateral pulmonary edema and pleural effusion accompanied by met

Comparison of reliability, validity and responsiveness of the Japanese Orthopaedic Association Shoulder 36 Ver. 1.3 among different diagnoses of shoulder lesions.

BACKGROUND: Shoulder 36 (Sh-36) is an original quality of life measure for shoulder lesions with high reliability and validity; however, in some cases, especially in those with a Bankart lesion, we observed no improvement in Sh-36 during the postoperative follow-up. Sh-36 may be less effective for a certain shoulder lesion. This study aimed to compare the reliability, validity, and responsiveness of Sh-36 among different representative diagnoses of shoulder lesions. METHODS: Sh-36 and the Disability of the Arm, Shoulder and Hand (DASH) were measured in 192 patients with a Bankart lesion (Bank

Operative vs Nonoperative Treatment for Atraumatic Rotator Cuff Tears: A Trial Protocol for the Arthroscopic Rotator Cuff Pragmatic Randomized Clinical Trial.

IMPORTANCE: Rotator cuff disorders remain the most common cause of shoulder pain and are among the most common reasons for patients to seek care in primary and specialty settings. Although operative and nonoperative treatments are offered to patients with atraumatic rotator cuff tears, there is a lack of evidence to support operative vs nonoperative treatment. This paucity of evidence has been highlighted by several professional agencies and experts. OBJECTIVE: To perform a pragmatic randomized clinical trial, the Arthroscopic Rotator Cuff trial, comparing pain and functional outcomes in pati

Shoulder impingement.

Understanding the pathophysiology and treatment of rotator cuff disorders is the key to understanding all other aspects of shoulder rehabilitation. Impingement rehabilitation focuses on strengthening the humeral head depressors, while ignoring the deltoid and supraspinatus muscles. Later treatment includes specific retraining of scapular balancing muscles. The final phase of treatment includes strengthening the prime humeral movers in positions that avoid further stress to the injured rotator cuff tendons and, last of all, specifically strengthening the supraspinatus muscle.

Comparison of 3 Suture Constructs on Upper One-third Subscapularis Tendon Tear Healing: An Animal Study Comparing Biomechanical, Histological, and Imaging Characteristics of Different Techniques.

BACKGROUND: Although arthroscopic single-row repair is a common approach for treating subscapularis tendon tears, its tendon-to-bone healing efficacy remains to be fully validated, particularly in terms of collagen remodeling and local inflammatory response. The H-loop knotless technique aimed to simplify the procedure and enhance repair stability. However, its healing outcomes compared with either the single-row (SR) or modified Mason-Allen (MMA) technique require experimental evaluation. PURPOSE: The aim of the study was to compare single-row, MMA, and H-loop techniques for repairing upper

Surgical anatomy of the shoulder.

The glenohumeral articulation is a versatile joint that requires a complex integration of bony ligamentous, musculotendinous, and neurovascular structures for proper function. Injuries resulting from dysfunction are common and potentially debilitating. Many of these injuries can be managed nonsurgically; however, if surgical treatment is indicated, a thorough knowledge of the anatomy of the shoulder girdle is critical. It is important for the surgeon to be aware of commonly used arthroscopic and surgical approaches to the glenohumeral joint along with anatomic structures at risk with each surg

Principles of arthroscopic repair of large and massive rotator cuff tears.

Minimally invasive arthroscopic techniques for rotator cuff tears have been greatly advanced during the past decade. It is important to review the clinical presentation and common physical findings of a large or massive rotator cuff tear, essential preoperative imaging, and the principles and technical aspects of all-arthroscopic repair. An anatomic repair of the footprint must begin with an understanding of the three-dimensional morphology of the rotator cuff tear and an accurate reduction of the tear. A contracted, immobile massive rotator cuff tear is challenging. Advanced arthroscopic mobi

[Two patients with an isolated traumatic rupture of the subscapular tendon].

A man aged 65 and a woman aged 55 were referred to the orthopaedic clinic because of persistent pain in the right shoulder following a fall, 5 and 16 months earlier respectively. On physical examination they were found to have a painful shoulder and loss of strength on endorotation of the right arm. Ultrasound examination of the shoulder revealed subscapular tendon rupture in both patients, this was confirmed by MRI scan in the first patient. Treatment was operative repair of the tendon by a deltopectoral approach, followed by wearing a exorotation-limiting sling. After six months the first pa

Axillary artery laceration after anterior shoulder dislocation reduction.

INTRODUCTION: Glenohumeral dislocation is the most commonly encountered dislocation in the emergency department. The most frequent complications of glenohumeral dislocation are rotator cuff tears and an increase in the risk of recurrent dislocation. Less common acute complications include fractures, neurological complications and vascular injuries. The incidence of axillary artery injury associated with shoulder dislocation is reported to be about 1-2%. CASE: An 81-year-old male presented to the emergency department with pain in the right shoulder after a fall. On physical examination, the sh

Morel-Lavallee Lesion Associated with Subtrochanteric Femur Fracture in a Young Female Resulting in Extensive Soft-Tissue Necrosis and Hypertrophic Non-union Managed with Staged Surgeries and Exchange Nailing: A Rare Injury Pattern and Review of Literature.

INTRODUCTION: Interferential screws and anchors have played a major role in fixation of soft tissue to bony tunnel. With advent of time, there developed metal anchors, biodegradable anchors, bioinert anchors, etc. Biodegradable anchors had complications such as reactive synovitis, cyst formation, soft-tissue inflammation, and local osteolysis. Polyether ether ketone (PEEK) which was biologically inert and radiolucent was introduced to overcome the disadvantages of anchors. We present a case of non-suppurative tissue reaction to PEEK anchor following rotator cuff repair. CASE REPORT: A 58-year

The optimum tension for bridging sutures in transosseous-equivalent rotator cuff repair: a cadaveric biomechanical study.

BACKGROUND: Transosseous-equivalent (TOE) rotator cuff repair can increase contact area and contact pressure between the repaired cuff tendon and bony footprint and can show higher ultimate loads to failure and smaller gap formation compared with other repair techniques. However, it has been suggested that medial rotator cuff failure after TOE repair may result from increased bridging suture tension. PURPOSE: To determine optimum bridging suture tension in TOE repair by evaluating footprint contact and construct failure characteristics at different tensions. STUDY DESIGN: Controlled laborato

Changes in Tracheal Tube Cuff Pressure and Recurrent Laryngeal Nerve Conductivity During Thyroid Surgery.

BACKGROUND: The aetiology of recurrent laryngeal nerve (RLN) neurapraxia is unclear in most RLN palsies post-thyroidectomy. We hypothesised that high intralaryngeal pressures impede RLN conductivity, in turn contributing to RLN palsy. Therefore, we measured tracheal tube (TT) cuff pressure (as a surrogate for intralaryngeal pressure) and RLN conduction during ten standard manoeuvres in thyroidectomy, to assess for correlation between cuff pressure and RLN conductivity. METHODS: A prospective cohort study of thyroidectomy during 2018. For each thyroid lobe, TT cuff pressure was continuously me

Relationship between subscapularis tears and injuries to the biceps pulley.

PURPOSE: The purpose of this study was to analyse the relationship between long head of the biceps brachii (LHBT) lesions and subscapularis tears. The hypothesis was that a bicipital pulley might remain intact, even in the case of a subscapularis tear. METHODS: Between 2010 and 2011, all patients who had a primary arthroscopic repair of a subscapularis tear were potentially included in this prospective study. The outcome of interest was the prevalence and type of arthroscopic lesions of the LHBT and bicipital pulley. Furthermore, the supposed pathomechanics of injury and the treatment propose

Superior labrum, anterior-posterior lesions and biceps injuries: diagnostic and treatment considerations.

Proximal biceps tendon rupture, biceps tendinitis, and medical biceps subluxation are injuries that occur alone and in association with other pathologic conditions of the shoulder. The literature describes various strategies for imaging and treating these biceps tendon complex injuries but offers no clear consensus on a gold-standard approach. The keys to successful treatment are an accurate diagnosis, identification of associated pathologic conditions, and assessment of functional limitations of the injury.

Imaging techniques of the shoulder: present perspectives.

Through its ability to evaluate the individual components of the rotator cuff, the labral-capsular complex, the subacromial and subdeltoid spaces, and the glenoid, humerus, clavicle, and acromion, MRI provides a means of comprehensively evaluating the shoulder. Further advances in the use of intra-articular MRI contrast agents will undoubtedly enhance the role of MRI in the evaluation of subtle labralcapsular pathology. Although they are experimental, kinematic studies of the shoulder may provide valuable insight into the pathogenesis of disorders such as impingement syndrome. The ability of M
