Cholelithiasis, cholecystitis, and common bile duct stones.

Cholecystitis and cholelithiasis are being recognized with increasing frequency in infancy, childhood, and adolescence. Hematologic disorders account for a large proportion of cases; however, in most cases the etiology is uncertain. Infants and children are noted with stones in association with total parenteral nutrition, prolonged fasting, or ileal resection. Biliary dyskinesia, a disorder of impaired gallbladder contractility, is being recognized with increased frequency in late childhood and teenage years. Spontaneous stone resolution is frequently noted in infancy, and a period of observat

Acute cholecystitis with calculous biliary duct obstruction in the gravid patient. Management by ERCP, papillotomy, stone extraction, and laparoscopic cholecystectomy.

The pregnant patient with cholelithiasis is subject to the manifold complications associated with this disease. The management of a gravid patient with acute cholecystitis complicated by biliary duct obstruction utilizing combined endoscopic therapy and laparoscopic cholecystectomy is reported. The benefits to the patient and fetus when successful are apparent, but it is important to anticipate potential problems associated with laparoscopy in pregnancy and to avoid them. Various safeguards such as fetal monitoring, careful trocar placement, and intra-abdominal pressure monitoring have been su

[Remaxol in perioperative infusion therapy for patients with calculous obstructive jaundice: a multiple-center observational study].

OBJECTIVE: To analyze the effect of Remaxol (Inosine + Meglumine + Methionine + Nicotinamide + Succinic Acid) in perioperative infusion therapy (PITh) on bilirubinemia in patients with cholelithiasis (CL) complicated by obstructive jaundice (OJ). MATERIAL AND METHODS: A multiple-center observational study included 283 patients with CL complicated by OJ. All patients underwent endoscopic papillosphincterotomy (EPST) with / without subsequent laparoscopic cholecystectomy (LC). Primary assessment of PITh efficacy was performed on days 6-7 after EPST. Bilirubinemia was compared with the day befor…

Impact of pancreatic and biliary stent on post-endoscopic papillectomy complications: A single-center retrospective study.

BACKGROUND: Endoscopic papillectomy (EP) is recommended as the first-line therapy for ampullary tumors, despite a relatively high incidence of complications. Pancreatic and/or biliary stents are placed at the endoscopist's discretion to prevent post-EP complications. The present study aimed to evaluate the efficacy of different stents. METHODS: A total of 117 patients who underwent EP and met the criteria between June 2006 and October 2022 were enrolled in the study. These patients were divided into a pancreatic stent group (PS group, n = 47), a biliary stent group (BS group, n = 38), and a t

Can prophylactic argon plasma coagulation reduce delayed post-papillectomy bleeding? A prospective multicenter trial.

BACKGROUND AND AIM: Endoscopic post-papillectomy bleeding occurs in 3% to 20% of the cases, and delayed bleeding is also problematic. However, there is no consensus on how to reduce delayed post-papillectomy bleeding. The aim of this study was to evaluate the efficacy of prophylactic argon plasma coagulation (APC) to minimize delayed bleeding and reduce the persistence of residual tumors after endoscopic papillectomy. METHODS: In a prospective pilot study of patients with benign ampullary tumors, the prophylactic APC group underwent APC at the resection margin following a conventional snaring

[Focus on Achalasia].

The pathophysiology of achalasia is largely unknown, and involves the destruction of ganglion cell in the esophageal myenteric plexus. High-resolution esophageal manometry is the key investigation. Endoscopic pneumodilatation and laparoscopic Heller myotomy have comparable short-term success rates, around 90%. The main complication after pneumodilatation is esophageal perforation, occurring in about 1% of cases. Peroral endoscopic myotomy is a promising treatment modality, however with frequent post-procedural gastroesophageal reflux.

Prospective randomized study of endoscopic biliary stone extraction using either a basket or a balloon catheter: the BasketBall study.

BACKGROUND: In Japan and Europe, a retrieval basket is generally used for endoscopic extraction of bile duct stones, while in the US, a retrieval balloon is mainly used. However, the efficacies of these two devices have not been previously compared. Therefore, the present multicenter, prospective, randomized study was performed to compare the efficacies of these two devices for endoscopic biliary stone extraction. METHODS: This study was designed as a non-inferiority study in comparing a basket removal with a balloon removal. Six Japanese institutions participated in this study, which include

Comparison of EUS-guided rendezvous and precut papillotomy techniques for biliary access (with videos).

BACKGROUND: Precut papillotomy after failed bile duct cannulation is associated with an increased risk of pancreatitis. EUS-guided rendezvous drainage is a novel alternative technique, but there are no data comparing this approach with precut papillotomy. OBJECTIVE: To evaluate the safety and efficacy of EUS-guided rendezvous drainage of the bile duct and compare its outcome with that of precut papillotomy. DESIGN: Retrospective study. SETTING: Tertiary care referral center. PATIENTS: Consecutive patients with distal bile duct obstruction, in whom selective cannulation of the bile duct at

[Tomato peel: rare cause of biliary tract obstruction].

Foreign bodies in the biliary tree are rare causes of obstructive jaundice. Food bezoars are infrequent as well. They can cause biliary obstruction after biliary tract interventions, or in the presence of biliary-bowel fistula or duodenum diverticulum. Food bezoars usually pass the gastrointestinal tract without any symptoms, but they can cause abdominal pain and obstructive jaundice in the case of biliary tract obstruction. Endoscopic retrograde cholangio-pancreatography has the major role in the diagnosis and the treatment of the disease. Authors summarize the medical history of a 91-year-ol

[Invasive endoscopy or surgery for pancreatic disorders?].

Endoscopic double papillotomy occupied the place of surgical transduodenal double sphincteroplasty for disorders of papilla of Vater or chronic pancreatitis several years ago. Endoscopic cystoenterostomy and cystogastrostomy can also replace surgery in the treatment of pseudocysts and walled-of necrosis even in cases of severe acute pancreatitis with/or without sepsis. In chronic pancreatitis endotherapy may be the treatment of choice at first, although surgical techniques give somewhat better long-term results for pain relief. Extracorporeal shock wave lithotripsy, stone resolution or extract

Endoscopic management of acute and chronic pancreatitis.

Endoscopic therapy has been increasingly recognized as the effective therapy in selected patients with acute pancreatitis and chronic pancreatitis (CP). Utility of endotherapy in various conditions occurring in acute pancreatitis and CP is discussed. Its efficacy, limitations, and alternatives are addressed. For the best management of these complex entities, a multidisciplinary approach involving expertise in all pancreatic specialties is essential to achieve the goal.

[Ultrasonic scanning and duodenoscopy in the diagnosis and treatment of obstructive jaundice].

In 186 patients with obstructive jaundice admitted to the clinic for diagnosis of its cause, the ultrasound scanning with duodenoscopy was employed. This permitted to establish the cause of jaundice in 84.9% of the patients and to choose proper therapeutic tactics at the shortest time after hospitalization. In 80% of the patients, an obstacle for bile outflow was liquidated by means of endoscopy. When it was not possible to established the cause of jaundice by the findings of ultrasound scanning and duodenoscopy, percutaneous transhepatic cholangiography was performed, which has proved to be s
