Behavioural phenotypes associated with specific genetic disorders: evidence from a population-based study of people with Prader-Willi syndrome.

BACKGROUND: Prader-Willi syndrome (PWS) is a genetic disorder resulting in obesity, short stature, cryptorchidism, learning disabilities (mental retardation) and severe neonatal hypotonia. Associated with the syndrome are a number of behaviours that are sufficiently distinctive that the syndrome is considered to have a specific 'behavioural phenotype'. METHODS: Through multiple sources we attempted to identify all people with PWS living in one region in the U K. This cohort was augmented by people with PWS from other regions, and a contrast group of people with learning disabilities of varied

Exotic and emerging viral encephalitides.

PURPOSE OF REVIEW: The exotic and emerging viral encephalitides are caused by animal or human viruses and characterised by sudden unexpected outbreaks of neurological disease, usually in tropical and sub-tropical regions, but sometimes spreading to temperate areas. Although a wide range of viruses come within this label, as this review highlights, there are common research questions as to the origin and spread of the viruses, the contribution of viral and host factors to the clinical presentations and outcome, and the possibilities for treatment and vaccination. RECENT FINDINGS: During 2002,

Infectious causes of acute flaccid paralysis.

PURPOSE OF REVIEW: Although acute flaccid paralysis (AFP) is more frequently seen in the tropics than in temperate regions, recent outbreaks of West Nile virus (WNV) in North America have drawn attention to this important presentation. Starting with anatomical and neurophysiological considerations, this article examines data on AFP caused by WNV, and considers recent data on paralysis caused by enteroviruses, and Guillain-Barré syndrome (GBS). RECENT FINDINGS: Neurophysiological, radiological and pathological studies suggest WNV causes AFP by damaging anterior horn cells in the spinal cord. T

Serial ferritin concentrations in hemodialysis patients receiving intravenous iron.

BACKGROUND: Treatment of the anemia of chronic renal failure with intravenous iron and erythropoietin is highly effective, but frequently leads to ferritin levels which are much higher than those seen in the general population. High ferritin concentrations raise concern about the potential toxicity of increased body iron stores. PATIENTS AND METHODS: We retrospectively evaluated parameters of iron metabolism over a 4-year period among all our chronic hemodialysis patients who had been receiving intravenous iron and erythropoietin. Initially, patients received intermittent infusions of 300 mg

An update on respiratory syncytial virus epidemiology: a developed country perspective.

Respiratory syncytial virus (RSV) is a leading cause of lower respiratory tract infections in infants and young children worldwide, and an important cause of morbidity, hospitalization, and mortality. The infections caused by RSV are seasonal, peaking predictably in the winter months in temperate climates, and in the hottest months and the rainy season in tropical climates. The involvement of the lower respiratory tract, manifest clinically as bronchiolitis or pneumonia, is the hallmark of severe RSV disease. Other indicators of severe disease include requirement for, and duration of, hospital

Long-term unpredictable foraging conditions and physiological stress response in mountain chickadees (Poecile gambeli).

Birds respond to short-term deterioration in foraging conditions by increasing their plasma level of corticosterone but the physiological effects of long-term deterioration in food supplies are not well known. In resident passerine birds that winter in temperate climates, such as the mountain chickadee (Poecile gambeli), the food supply may be limited and unpredictable over long periods of time. Whether the long-term limited and unpredictable food supply has an effect on (a) baseline levels of corticosterone and (b) the adrenocortical stress response to a standardized acute stress of handling

Cat-scratch disease: epidemiology, aetiology and treatment.

Cat-scratch disease (CSD) is a clinical syndrome that usually presents as a self-limiting lymphadenopathy associated with a cat scratch or bite. Commonly affecting children and young adults, it has a worldwide distribution. In temperate climates, higher rates are reported in the autumn and winter, which can be attributed to the seasonal breeding of the domestic cat. The organism responsible was identified in 1983, having eluded detection for 50 years. Initially, Afipia felis was thought to be the cause; however, subsequent study failed to confirm a link. During the 1990s, it was demonstrated c

Mosquito (Diptera: Culicidae) dispersal: implications for the epidemiology of Japanese and Murray Valley encephalitis viruses in Australia.

One hypothesis to explain the southern extension of Japanese encephalitis (JE) virus from Papua New Guinea into the Torres Strait islands in 1995 and to mainland Australia in 1998 is the dispersal of infected mosquitoes, particularly Culex annulirostris Skuse from which JE virus has been isolated repeatedly. To investigate whether this species disperses in this manner, mosquitoes were identified from 368 aerial kite trap collections operated at 50-310 m (altitude) at inland New South Wales between November 1979 to December 1984. Forty samples (9 during daylight and 31 at night) contained mosqu

Reaching the diagnosis of cystic fibrosis--the limits of the spectrum.

BACKGROUND: Cystic fibrosis is the most common life-limiting autosomal recessive genetic disorder in Caucasians. Typically it is a multisystem disease diagnosed by increased chloride levels on sweat testing, with mortality due mainly to progressive respiratory disease. The clinical spectrum of CF has recently been much expanded. Genetic testing for mutant CF transmembrane regulator has revealed atypical cases where sweat test results are borderline or normal. In other patients, genetic mutations cannot be identified but abnormal CFTR function is shown using nasal potential difference measureme

Dendritic cell-derived IL-12 promotes B cell induction of Th2 differentiation: a feedback regulation of Th1 development.

B cells convert what are normally conditions for Th1 differentiation into an environment suitable for Th2 development. This capacity is dependent on CD40 as B cells from CD40-/- mice do not elicit Th2 differentiation. To elucidate the basis of this effect, we surveyed cytokine RNA made by naive B cells after activation with anti-Ig and anti-CD40. Resting B cells make TGF-beta message only, however, 4 days after activation, RNA encoding IL-6, IL-10, and TNF-alpha was found. The expression of these messages was accelerated by 2 days in the presence of IL-12. The relevance of these observations t

Relationship of childhood maltreatment to the onset and course of major depression in adulthood.

The objective of this research was to determine the prevalence of retrospectively recalled childhood trauma among depressed patients and to examine the relationship between retrospective recall of childhood maltreatment and the onset, course, and severity of major depression in adulthood. Forty-seven adults with DSM-IV major depression and forty-one healthy comparison subjects were administered the Childhood Trauma Questionnaire (CTQ), a self-report measure of traumatic experiences in childhood. Ontogeny of interstitial cells of Cajal in the human intestine.

BACKGROUND/PURPOSE: Interstitial cells of Cajal (ICC) recently have been identified as intestinal pacemaker cells. Abnormalities in ICC are increasingly recognized in a number of neonatal disorders such as infantile hypertrophic pyloric stenosis, Hirschsprung's disease, and transient intestinal pseudo-obstruction. The aim of this study was to determine the fetal and postnatal differentiation and development of ICC in the human gastrointestinal tract to aid interpretation of pathological specimens. METHODS: Specimens of human gastrointestinal tract from (1) fetuses (9 to 17 weeks' gestation; n

[The black rat (Rattus rattus) and the plague in ancient and medieval western Europe].

The first time plague affected Western Europe was in the early Middle Ages: rom 541 to 767, there were no fewer than 15 outbreaks in southern parts of the continent. Plague then disappeared from Europe for some seven centuries but came back with a vengeance in 1347, this time by way of the Mediterranean, and ravaged the entire continent for five years, resulting in a serious demographic depression. From then on until 1722 (and 1771 in Moscow), the disease remained endemic to Europe, periodically undermining its economy. These epidemics were major determinants of medieval history, but their stu

Patterns of genetic variability in colonized strains of Lutzomyia longipalpis (Diptera: Psychodidae) and its consequences.

The genetic qualities of laboratory colonies of phlebotomine sand flies have not been compared with field specimens despite 1) probable genetic shifts due to the colonization process and 2) the problems associated with the extrapolation of experimental data derived from colonized organisms to field populations. The present study compared the genetic profiles of five laboratory colonies of geographic strains of the New World sand fly Lutzomyia longipalpis, and contrasted them with field populations. The profiles were based on the variability exhibited with polyacrylamide gels at 14 enzyme loci.

An epidemiologic study of Lyme disease in southern Sweden.

BACKGROUND: Lyme disease is the most common vector-borne infection in some temperate regions of the Northern Hemisphere. However, for most areas of endemic disease reliable epidemiologic data are sparse. METHODS: Over a one-year period, we conducted a prospective, population-based survey of cases of Lyme disease in southern Sweden. The diagnosis was made on the basis of the presence of erythema migrans at least 5 cm in diameter or characteristic clinical manifestations such as arthritis, neuroborreliosis, and carditis. RESULTS: We identified 1471 patients with Lyme disease, for an overall an

The genetics of the Gilles de la Tourette syndrome: a review.

The Gilles de la Tourette syndrome (GTS) is a hereditary, neuropsychiatric-neurobehavioral disorder with childhood onset that is characterized by motor and vocal tics that may vary from relatively mild to very severe. The exact pathogenesis is still unknown. Following anecdotal reports of familial occurrence, systematic family studies have been carried out since 1980 to establish the mode of inheritance and ultimately to map and clone the susceptibility gene(s). Currently, as a result of combined North American, English, and Dutch studies, the most accepted genetic model is an autosomal domina

Delayed repair of congenital diaphragmatic hernia.

Early reports of improved survival in newborns with congenital diaphragmatic hernias (CDH) utilizing extracorporeal membrane oxygenation (ECMO) and/or a delayed repair (DR) approach have been tempered by recent failures to document such an improvement. We have used ECMO to salvage emergently repaired patients with CDH since January 1984. From January 1990 to January 1991, we treated 16 CDH patients with mechanical ventilation and other supportive techniques until persistent pulmonary hypertension of the newborn resolved. We compared this DR group to 19 patients emergently repaired from Februar

[Self-reported health status among students in Scandinavia].

The article reports data from the WHO cross-national study on health behaviour in schoolchildren. A sample of 11,774 children aged 11, 13 and 15 years in Denmark, Finland, Norway and Sweden answered a questionnaire on social situation, health, health behaviour and lifestyle. Self assessed health is reported excellent among approximately two thirds of the Swedish children, half the Danish children, one third of the Finnish children and one fifth of the Norwegian children. In Denmark, Norway and Sweden self-assessed health is better among boys than girls. The most frequent symptom reported by th

Hemodynamic and renin responses to nifedipine in renovascular hypertension.

We investigated the acute effects of nifedipine (10 mg orally) on renin and systemic hemodynamics in twelve patients who had severe hypertension (diastolic values averaging 114 mm Hg) with unilateral renal artery stenosis (angiography) and hyperreninemia. Plasma renin activity was determined from blood samples drawn from the aorta and from both renal veins, so that "ischemic lateralization" could be evaluated through appropriately derived indexes. Nifedipine promptly and significantly lowered aortic pressure in all patients. At 30 minutes maximal circulatory responses were recorded, which cons

Surgical aspects of inflammatory bowel disease.

In temperate, developed countries, the principle indications for surgical intervention in inflammatory bowel disease are 1) for the removal of bowel severely damaged by acute or chronic colitis, either Crohn's disease or ulcerative colitis; and 2) for small-bowel Crohn's disease, to overcome the effects of fibrous stenosis and its sequelae, such as abscess or fistula. Unresponsive acute colitis that necessitates emergency surgical intervention involves a total colectomy, end ileostomy, and a mucous fistula of the rectosigmoid above the symphysis pubis. Once the diagnosis is confirmed, a pouch
