Acute Stroke Care for Intracranial Artery Occlusion in an Aging Rural Region: Insights From the Fukushima Large Vessel Occlusion (LVO) Stroke Registry.

BACKGROUND: Establishing an effective stroke care system for patients with acute intracranial artery occlusion is particularly challenging in regions affected by aging and depopulation with limited medical resources. We conducted a study, termed the Fukushima Large Vessel Occlusion (LVO) Stroke Registry, to clarify real-world treatment practices and patient pathways in such a setting. METHODS: This prospective multicenter registry enrolled consecutive patients with acute intracranial artery occlusion admitted within 24 h of symptom onset across 12 hospitals in Fukushima Prefecture between Jul

Emergent Carotid Artery Stenting in Patients With Acute Ischemic Stroke With Tandem Lesions: One-Year Follow-Up Results From the SECURIS Study.

BACKGROUND AND OBJECTIVES: Management of carotid lesions during endovascular therapy (EVT) in patients with acute ischemic stroke (AIS) due to tandem lesions (TL) remains controversial, and long-term prognosis data are scarce. We evaluated 90-day and 1-year follow-up outcomes of emergent carotid artery stenting (eCAS) vs non-eCAS in these patients. METHODS: Data from a prospective mandatory population-based registry of Code Stroke in Catalonia (Spain) between 2017 and 2023 were analyzed. Patients with large vessel occlusion and ipsilateral ≥50% atheromatous cervical internal carotid artery (I

Fluorine-18 Prostate-Specific Membrane Antigen-1007 PET/CT vs Multiparametric MRI for Locoregional Staging of Prostate Cancer.

IMPORTANCE: Prostate-specific membrane antigen (PSMA) demonstrates overexpression in prostate cancer and correlates with tumor aggressiveness. PSMA positron emission tomography (PET) is superior to conventional imaging for the metastatic staging of prostate cancer per current research but studies of second-generation PSMA PET radioligands for locoregional staging are limited. OBJECTIVE: To determine the accuracy of fluorine-18 PSMA-1007 PET/computed tomography (18F-PSMA-1007 PET/CT) compared to multiparametric magnetic resonance imaging (MRI) in the primary locoregional staging of intermediat

A Replication-Defective Influenza Virus Harboring H5 and H7 Hemagglutinins Provides Protection against H5N1 and H7N9 Infection in Mice.

The recent highly pathogenic avian influenza (HPAI) H5N1 and H7N9 viruses have caused hundreds of human infections with high mortality rates. Although H5N1 and H7N9 viruses have been limited mainly to avian species, there is high potential for these viruses to acquire human-to-human transmission and initiate a pandemic. A highly safe and effective vaccine is needed to protect against a potential H5N1 or H7N9 influenza pandemic. Here, we report the generation and evaluation of two reassortant influenza viruses, PR8-H5-H7NA and PR8-H7-H5NA These viruses contain six internal segments from A/Puert

Predictors of outcome after mechanical thrombectomy for anterior circulation large vessel occlusion in patients aged ≥80 years.

BACKGROUND: There is uncertainty about the role of endovascular recanalization procedures for the treatment of acute ischemic stroke in patients aged ≥80 years. Therefore, careful patient selection is mandatory. Our aim was to find valid predictors of clinical outcome after mechanical thrombectomy (mTE) based on the sparse information available in the emergency setting. METHODS: We included consecutive patients aged ≥80 years treated by mTE for anterior circulation thromboembolic vessel occlusion in our department between January 2008 and January 2013. Successful recanalization was defined as

Nursing time to program and assess deep brain stimulators in movement disorder patients.

The use of deep brain stimulation (DBS) to treat movement disorders such as Parkinson's disease, essential tremor, and dystonia is increasing. Although some published literature describes the methods for DBS programming, the time and nursing requirements to run a DBS surgical program have not been examined previously. For this study, we prospectively recorded the time required for both assessments and programming of the DBS from the preoperative period to 1 year after surgery in a variety of patients. Results showed that the mean total time spent programming the stimulator and assessing these

Use of shopping centres in screening for hypertension.

In two Edmonton shopping centres 9591 people were screened for hypertension: 3.3% were found to be normotensive but taking antihypertensive medication and another 8.8% were found to have elevated blood pressure. Systolic hypertension alone accounted for 45.3% of the hypertensive cases and diastolic hypertension, with or without systolic, for 54%. Of the group with elevated blood pressure 34.5% had been previously unaware of their condition, 18.7% had never received medication for it, 18.2% had received medication in the past but had discontinued it, 26.1% were still on medication and 2.5% were

Ambulance use, distance and outcomes in patients with suspected cardiovascular disease: a registry-based geographic information system study.

BACKGROUND: Despite guideline recommendations, the majority of patients with symptoms suggestive of acute coronary syndrome do not use emergency medical services to reach the emergency department (ED). The aim of this study was to investigate the factors associated with EMS utilisation and subsequent patient outcomes. METHODS: Using administrative data, all patients who presented to an ED in the metropolitan areas of Edmonton and Calgary in the years of 2007-2013 with main ED diagnosis of acute coronary syndrome, stable angina or chest pain were included. The travel distance was estimated usi

Asymptomatic bacteriuria is an independent predictor of urinary tract infections in an ambulatory cirrhotic population: a prospective evaluation.

BACKGROUND & AIMS: Asymptomatic bacteriuria (ASB) is a risk factor for urinary tract infections (UTIs) in many patients without liver disease. It remains unclear whether a diagnosis of ASB in an outpatient with cirrhosis could be utilized to predict the subsequent development of a UTI. We undertook this study to determine the prevalence and incidence of ASB in an outpatient population and its association with UTI. METHODS: We prospectively evaluated 108 adult outpatients with cirrhosis over a 6-month period. Monthly midstream urines (MSU) were performed to detect the occurrence of UTI and ASB

Prevalence and predictors of self-reported sexual abuse in severely obese patients in a population-based bariatric program.

BACKGROUND: Sexual abuse may be associated with poorer weight loss outcomes following bariatric treatment. Identifying predictors of abuse would enable focused screening and may increase weight management success. METHODS: We analyzed data from 500 consecutively recruited obese subjects from a population-based, regional bariatric program. The prevalence of self-reported sexual abuse was ascertained using a single interview question. Health status was measured using a visual analogue scale (VAS). Multivariable logistic regression was performed to identify sexual abuse predictors. RESULTS: The

Staphylococcus aureus bacteremia in patients receiving pegylated interferon-alpha and ribavirin for chronic hepatitis C virus infection.

Bacteremia has rarely been reported in patients receiving treatment for hepatitis C virus (HCV) infection. We describe the features and investigation of four cases of Staphylococcus aureus bacteremia occurring between 3 November 2004 and 10 January 2005 in patients on therapy for chronic HCV infection. The unusual occurrence of S. aureus bacteremia in a series of patients led to an epidemiologic investigation and molecular typing methods were employed to assess the relatedness of cases. The mean time of bacteremia onset was week 10 of HCV treatment. No patient had neutropenia previously. The a

Integrated, Community-Led, Culturally Safe Implementation of a Chronic Kidney Disease Detection Program for Indigenous Communities of Canada: The Kidney Check Program Report.

PURPOSE OF PROGRAM: Indigenous peoples in Canada experience a disproportionate burden of chronic kidney disease (CKD) yet face systemic, geographic, and cultural barriers to early detection and care. The Kidney Check program was established as a national initiative to address this gap by providing community-led, culturally safe, and integrated chronic disease screening. SOURCES OF INFORMATION: Kidney Check builds on the success of the First Nations Community-Based Screening to Improve Kidney Health and Prevent Dialysis (FINISHED) study in Manitoba and was expanded through the Can-SOLVE CKD Ne
