Lower body parkinsonism: rethinking MRI planimetry.

BACKGROUND: Magnetic resonance imaging (MRI) planimetric biomarkers such as the midbrain to pons (M/P) ratio, magnetic resonance parkinsonism index (MRPI), and MRPI 2.0 have demonstrated high diagnostic accuracy discriminating Progressive Supranuclear Palsy (PSP) patients from other atypical parkinsonisms and Parkinson's Disease (PD). However, these indexes have not been studied in the specific context of lower body parkinsonism (LBP), where the differential diagnosis between PSP, vascular parkinsonism (VP) and normal pressure hydrocephalus (NPH) is of great clinical relevance. OBJECTIVES: Ou

Brain ventricle parcellation using a deep neural network: Application to patients with ventriculomegaly.

Numerous brain disorders are associated with ventriculomegaly, including both neuro-degenerative diseases and cerebrospinal fluid disorders. Detailed evaluation of the ventricular system is important for these conditions to help understand the pathogenesis of ventricular enlargement and elucidate novel patterns of ventriculomegaly that can be associated with different diseases. One such disease is normal pressure hydrocephalus (NPH), a chronic form of hydrocephalus in older adults that causes dementia. Automatic parcellation of the ventricular system into its sub-compartments in patients with

Automated detection of imaging features of disproportionately enlarged subarachnoid space hydrocephalus using machine learning methods.

OBJECTIVE: Create an automated classifier for imaging characteristics of disproportionately enlarged subarachnoid space hydrocephalus (DESH), a neuroimaging phenotype of idiopathic normal pressure hydrocephalus (iNPH). METHODS: 1597 patients from the Mayo Clinic Study of Aging (MCSA) were reviewed for imaging characteristics of DESH. One core feature of DESH, the presence of tightened sulci in the high-convexities (THC), was used as a surrogate for the presence of DESH as the expert clinician-defined criterion on which the classifier was trained. Anatomical MRI scans were automatically segmen

Hemorrhage into cystic vestibular schwannoma following stereotactic radiation therapy.

The occurrence of a hemorrhage into a vestibular schwannoma is a rare phenomenon. Several reports, however, indicate a risk profile for a subgroup of patients with vestibular schwannoma which show hypervascularity in the histopathological examination. Cystic formation, large size, mixed Antoni type and anticoagulation therapy seem to enhance the risk of tumor hemorrhage. We report on a patient with a large cystic vestibular schwannoma who died from fatal bleeding into the tumor 15 months following stereotactic radiation therapy. Since hemorrhage seems to be a relevant risk in large cystic vest

High cervical disc herniation presenting with C-2 radiculopathy. Case report and review of the literature.

The authors report the case of a 78-year-old man with a C2-3 disc herniation that had migrated rostrally, causing C-2 radiculopathy. The C-2 radiculopathy manifested immediately after the patient underwent placement of a ventriculoperitoneal shunt for normal-pressure hydrocephalus. Myelography and computerized tomography scanning of the cervical spine revealed an extradural lesion anterolateral to the thecal sac eccentric to the right. The patient underwent a C1-3 laminectomy, C-2 nerve root decompression, and excision of the lesion. Postoperatively the patient's radiculopathy resolved complet

Epidural hematoma of the cervical spine after cervical laminectomy in a patient with ventriculo-peritoneal shunt.

Cervical laminectomy is one of the most common procedures performed in the sitting position. However in the presence of intracranial hypotension, a negative intradural pressure develops when the patients are in the sitting position. They are therefore at risk of developing cervical spinal epidural hematomas after cervical laminectomy. We report a case of a shunted patient who developed an epidural hematoma 3 days after a cervical laminectomy procedure in a seated position when he began to ambulate. When performing a cervical laminectomy procedure on a shunted patient, an upright position shoul

Aphasia, apraxia, and agnosia in the diagnosis of dementia.

The association of aphasia, apraxia and agnosia with cortical but not subcortical dementias, is a widely held belief. The purpose of the present study was to determine the frequency of aphasia, apraxia, and agnosia in groups of cortical and subcortical dementia patients, and to assess the diagnostic utility of these symptoms. Subjects were 64 patients with subcortical dementias (Parkinson's disease and normal pressure hydrocephalus) and 192 patients with cortical dementia (probable Alzheimer's disease) matched for sex, age, and Mini-Mental State Examination score. Each patient was evaluated fo

A study of confabulation.

Confabulation consequent to organic amnesia is a well-described clinical finding. A number of plausible theories of confabulation have been proposed, but the various claims and counterclaims have not been systematically tested. A standardized test battery that included four different kinds of questions plus measures of suggestibility and the tendency to utilize external cues was administered to ten amnesic patients who demonstrated varying degrees of confabulation. One additional patient, whose clinical condition changed dramatically during his hospital course, received the battery on a number

Cerebrospinal fluid flow. III. Pathological cerebrospinal fluid pulsations.

Cardiac- and respiration-related movements of the cerebrospinal fluid (CSF) were investigated by MRI in 71 patients. In most patients with arteriosclerotic occlusive vascular disease CSF pulsations are normal. Decreased pulsatile flow is detectable in those with arteriovenous malformations, intracranial air and following lumbar puncture and withdrawal of CSF. Increased pulsatile flow in the cerebral aqueduct was found in 2 patients with large aneurysms, idiopathic communicating syringomyelia and in most cases of normal pressure hydrocephalus (NPH). CSF flow in the cervical spinal canal is, how

Endovascular Treatment of Hydrocephalus: A Systematic Literature Review.

INTRODUCTION: Hydrocephalus is traditionally managed with ventriculoperitoneal shunting (VPS), which is associated with high rates of infection, mechanical failure, and surgical revisions. Recent innovations in endovascular techniques have led to the development of minimally invasive cerebrospinal fluid (CSF) diversion techniques. This literature review systematically examines the development, procedural techniques, efficacy, and safety profile of endovascular treatments for communicating hydrocephalus. METHODS: A systematic search was conducted in PubMed/MEDLINE, Embase, Scopus and clinicalt

Integrative analysis of intracranial pressure and R-R interval signals: a study of ICP B-wave using causal coherence.

Causal coherence analysis was applied to beat-to-beat mean intracranial pressure (ICP) and RR interval signals that were recorded from twelve normal pressure hydrocephalus patients. Data were organized into two groups including an ICP B-Wave group and a control baseline group. Maximal classic coherence between ICP and RR interval within [0.04-0.15] Hz was found to be significantly greater than zero for both B-wave and control groups. Causal coherence analysis further revealed that feedforward coherence due to RR interval's effect on ICP always exists for both B-wave and baseline ICP state whil

A dementia story with a happier ending.

A case is described of a 77-year-old lady who was diagnosed as having Parkinson's disease and was treated with antiparkinsonian drugs over a year with little effect. She was subsequently referred to a psychiatrist because of her aggressive outbursts and marked changes in personality. Investigations showed that she suffered from normal pressure hydrocephalus (NPH), and the insertion of a ventriculo-peritoneal shunt (VPS) led to a resolution of most of her problems, physical as well as mental.

Vascular endothelial growth factor in the CSF of elderly patients with ventriculomegaly: variability, periodicity and levels in drainage responders and non-responders.

OBJECTIVES: The aim of this study was to examine lumbar CSF-VEGF levels from elderly patients with ventriculomegaly to evaluate the possible circadian or periodic concentration profile and relevance to the prediction of drainage response. METHODS: Lumbar CSF samples were collected in 1-h interval over 35 h from 22 patients with ventriculomegaly. CSF-VEGF levels were measured to elucidate the possible circadian or periodic concentration profiles. These VEGF levels were evaluated for correlations with clinical response to CSF drainage, ventricle size and other clinical information. RESULTS: Th

Prospective analysis by computed tomography and long-term outcome of 23 adult patients with chronic idiopathic hydrocephalus.

From 1986 to 1989, 23 adult patients (average age, 70 yr) with idiopathic chronic hydrocephalus received shunts with medium-pressure Pudenz-Schulte valves for suspected normal pressure hydrocephalus. Prospective clinical and computed tomographic monitoring was continued for at least 5 years. We observed the formation of a hypodense subdural collection in each of 10 patients (43%). Those collections that occurred early, i.e., within the first 9 postoperative days, evolved differently from those that occurred late; only early hypodense collections became subdural hematomas (three cases). In one

P-Tau as prognostic marker in long term follow up for patients with shunted iNPH.

Objective: Diagnosis of idiopathic Normal Pressure Hydrocephalus (iNPH) relies solely on clinical and radiological criteria while, unlike other neurological diseases, the analysis of cerebrospinal fluid markers is not used in clinical practice. Nevertheless, the overlapping of neurodegenerative diseases affects the long-term shunt efficacy and this occurrence should be detected before surgery. Therefore, we performed this study in order to assess the correlation between pre-surgical levels of CSF Beta Amyloid protein, Total Tau protein and Phospho-Tau protein with long-term clinical outcome. Clinical Experience with Cerebrospinal Fluid Aβ42, Total and Phosphorylated Tau in the Evaluation of 1,016 Individuals for Suspected Dementia.

BACKGROUND: Elevated total tau (tTau), 181-phosphorylated phosphorylated tau (pTau), and low amyloid-β42 (Aβ42) in cerebrospinal fluid (CSF) represent a diagnostic biomarker for Alzheimer's disease (AD). OBJECTIVE: The goal was to determine the overall accuracy of CSF Aβ42, tTau, pTau, and the Aβ42/total tau index (ATI) in a non-research, clinical setting for the diagnosis of AD. METHODS: From medical records in 1,016 patients that had CSF studies for dementia over a 12-year period (2005 to 2017), we calculated the sensitivity and specificity of CSF Aβ42, tTau, and pTau and the ATI in relati
