Nausogenic properties of various dynamic and static force environments.

Motion sickness can occur when an accelerating force acting on the human body repeatedly changes amplitude and direction or both. It also can occur without any motion after transfer into a constant force field significantly different from Earth-gravity. Dynamic and static causes of motion sickness can be distinguished accordingly. Space sickness, too, has dynamic as well as static aspects. Dynamic space sickness might depend on increased bilateral differential sensitivity of the peripheral and central vestibular apparatus, whereas static space sickness may be caused by erroneous compensation o

Neurotological studies on Meniere's disease and sudden deafness.

During the period from January 1973 to June 1975, 60 cases with definite Meniere's disease and 30 cases with sudden deafness were listed according to such criteria for the diagnosis of both diseases as determined by the Research Committee of Japan. A neurootological assessment for both diseases were performed in our series. In patients with definite Meniere's disease, hearing impairment was characterized by deficiency in detecting low- and middle-pitched tones and fluctuation of auditory threshold in low frequency range. In sudden deafness, highly impaired hearing loss, i.e., total deafness, w

Cochlear implantation in patients with autoimmune inner ear disease including cogan syndrome: a comparison with age- and sex-matched controls.

OBJECTIVES/HYPOTHESIS: Evaluate the characteristics and outcomes of patients with autoimmune inner ear disease (AIED) who have undergone cochlear implantation (CI) and compare post-CI performance in AIED to matched controls. STUDY DESIGN: Retrospective case control study. METHODS: Study cohort was comprised of 25 adult implantees (AIED [n = 18], Cogan syndrome [n = 7]). The AIED group was defined by rapidly progressive bilateral sensorineural hearing loss leading to unusable hearing within weeks to months. Patients with Cogan syndrome, the archetypal inner ear autoimmune disease, were also e

Vibration-induced nystagmus in patients with vestibular disorders.

OBJECTIVE: Our goal was to clarify the clinical significance of vibration-induced nystagmus (VIN). METHODS: One hundred patients with unilateral vestibulocochlear disorders were enrolled into this study. However, patients with spontaneous nystagmus were excluded. Vibratory stimuli (approximately 100 Hz) were presented to the mastoids and the forehead. Patients also underwent caloric testing and vestibular evoked myogenic potential testing. RESULTS: Of the 100 patients, 60 (60%) showed VIN. The nystagmus was mainly horizontal. VIN was more frequently evoked on the mastoids than the forehead.

Vestibulo-ocular responses during static head roll and three-dimensional head impulses after vestibular neuritis.

This study aimed to investigate whether unilateral vestibular neuritis (VN) causes the same deficits of ocular counter-roll during static head roll (OCR(S)) and dynamic vestibulo-ocular reflex gains during head impulses (VOR(HI)) as unilateral vestibular deafferentation (VD). Ten patients with acute and 14 patients with chronic vestibular paralysis after VN were examined. The testing battery included fundus photography of both eyes with the head upright (binocular cyclorotation) and dual search coil recordings in a three-field magnetic frame. With one dual search coil on the right eye and the

Endoscopic diagnosis of idiopathic perilymphatic fistula.

The usefulness of endoscopic examination for the diagnosis of idiopathic perilymphatic fistula (IPLF) was investigated. Eight patients presenting with unilateral sensorineural hearing loss and vertigo underwent endoscopic examination by the transtubal or transtympanic approach. In 5 out of the 8 patients, transtubal endoscopy was carried out using a superfine flexible endoscope. With this approach, no abnormal findings were visualized. A perilymphatic leak from the round window was observed in 2 patients by means of transtympanic examination using a needle scope. These findings were confirmed

Electronystagmographic features in some peripheral and central vestibular disorders: application of multiple discriminant analysis of electronystagmographic parameters.

In routine clinical electronystagmographic (ENG) tests (postrotatory, optokinetic, caloric and tracking tests), eye movement signals were analyzed and a multiple discriminant analysis was carried out with the aid of a microcomputer. Six parameters were selected and, based on these, two functions for discriminating between peripheral and central disorders were established. Discrimination between 35 patients with peripheral lesions and 15 patients with central lesions was made with a correct classification rate of 97.1 and 86.7%, respectively. These rates are significantly higher than that of an

[Vestibular disorders in old age].

The elderly show a general reduction of their bodily and mental reactions. They become slower to react and their sensory ability decreases, e.g. hearing, vision, smell and taste. With increasing age, disturbances of the balance system are found more frequently, resulting in dysequilibrium, vertigo, lightheadedness and falling. We investigated the physiological changes in the vestibular system associated with the ageing processes. We selected 470 patients aged from 1-90 years from 1500 routine neurological patients. All of these patients underwent a routine neuro-otological test battery includi

Vertigo and imbalance in the elderly.

The diagnosis of vestibular disorders causing dizziness and imbalance in the elderly can present a difficult problem. Knowledgeable diagnosis depends on an understanding of the role of the vestibular end-organs as motion sensors and sensors of gravity as well as an understanding of how the vestibular system influences body balance and how visual mechanisms interact with the vestibular system. Based on a knowledge of these physiological principles, a number of interesting aspects of age-related vestibular disorders are discussed in more detail.

Computerized screening of the human vestibulospinal system.

The vestibular nerve projects rostrally via the ascending medial longitudinal fasiculus (MLF) to eventually supply the ocular muscles and also projects caudally via the descending MLF to eventually supply the spinal and limb musculature. Electronystagmographic techniques are designed to evaluate only portions of the vestibulo-ocular and oculomotor systems. Vestibular lesions affecting only the caudal vestibular nerve distributions or lesions affecting both neural distributions, therefore, cannot be objectively assessed except for the vestibulo-ocular component. A promising recording technique

Vestibular Function and Motion Sickness Susceptibility: Videonystagmographic Evidence From Oculomotor and Caloric Tests.

Purpose Motion sickness (MS) is a common condition that affects millions of individuals. Although the condition is common and can be debilitating, little research has focused on the vestibular function associated with susceptibility to MS. One causal theory of MS is an asymmetry of vestibular function within or between ears. The purposes of this study, therefore, were (a) to determine if the vestibular system (oculomotor and caloric tests) in videonystagmography (VNG) is associated with susceptibility to MS and (b) to determine if these tests support the theory of an asymmetry between ears ass

Extension patterns of vestibular schwannomas towards the middle ear: three new cases and review of the literature.

OBJECTIVE: Middle ear extension of vestibular schwannomas is not a common occurrence, and only a few cases have been described so far in past publications. We report three new cases of vestibular schwannomas extending to the middle ear and reviewed the literature to specify the patterns of such an extension. MATERIALS AND METHODS: We analysed databases of previously published articles to search for additional cases of middle ear extension of vestibular schwannomas and compared them to the cases we have documented. Extension patterns of the tumours were analysed, especially focusing on the ext
