Upper GI bleeding in healthy full-term infants: a case-control study.

OBJECTIVE: The aim of this case-control study was to evaluate the frequency and the type of mucosal lesions in newborn babies with upper GI bleeding (UGIB), the diagnostic role and safety of upper GI endoscopy, and the recognition of risk factors associated with the hemorrhagic event. METHODS: A population of 5180 infants born from June, 1988 to May, 1997 was examined. A case was defined as any patient who had UGIB within 4 days of delivery. The diagnosis was made by endoscopic examination in an endoscopy room. The following parameters were determined: amniotic fluid features, funicular blood

Streamlined extraction of nucleic acids and metabolites from low- and high-biomass samples using isopropanol and matrix tubes.

UNLABELLED: An essential aspect of population-based research is collecting samples outside of a clinical setting. This is crucial because microbial populations are highly dynamic, varying significantly across hosts, environments, and time points, a variability that clinical sample collection alone cannot fully capture. At-home sample collection enables the inclusion of a larger and more diverse group of participants, accounting for differences in ethnicity, age, and other factors. However, managing large studies is challenging due to the complexities involved in sample acquisition, processing,

Sustained Opening of the Blood-Brain Barrier with Progressive Accumulation of White Matter Hyperintensities Following Ischemic Stroke.

OBJECTIVE: To report a patient in whom an acute ischemic stroke precipitated chronic blood-brain barrier (BBB) disruption and expansion of vascular white matter hyperintensities (WMH) into regions of normal appearing white matter (NAWM) during the following year. BACKGROUND: WMH are a common finding in patients with vascular risk factors such as a history of stroke. The pathophysiology of WMH is not fully understood; however, there is growing evidence to suggest that the development of WMH may be preceded by the BBB disruption in the NAWM. METHODS: We studied a patient enrolled in the Nation

Towards a harmonized method for the global reconnaissance of multi-class antimicrobials and other pharmaceuticals in wastewater and receiving surface waters.

Antimicrobial resistance is a worldwide problem that is both pressing and challenging due to the rate at which it is spreading, and the lack of understanding of the mechanisms that link human, animal and environmental sources contributing to its proliferation. One knowledge gap that requires immediate attention is the significance of antimicrobial residues and other pharmaceuticals that are being discharged from wastewater treatment plants (WWTPs) on the dissemination of antimicrobial resistance in the environment. In this work we provide an approach to develop a harmonized analytical method f

Activity of selected oxidizing microbicides against the spores of Clostridium difficile: relevance to environmental control.

BACKGROUND: Clostridium difficile is an increasingly common nosocomial pathogen, and its spores are resistant to common environmental surface disinfectants. Many high-level disinfectants (eg, aldehydes) are unsuitable for environmental decontamination because they need several hours of contact to be sporicidal. This study tested the potential of selected oxidative microbicides to inactivate C. difficile spores on hard surfaces in relatively short contact times at room temperature. METHODS: The spores of a clinical isolate of C. difficile were tested using disks (1 cm diameter) of brushed stai

Transcranial Photobiomodulation Promotes Neurological Resilience in Current Collegiate American Football Players Exposed to Repetitive Head Acceleration Events.

Repetitive head acceleration events (RHAE) are common in contact sports and associated with neuroinflammation, axonal injury, and long-term neurological impairments, including increased risk for chronic traumatic encephalopathy. Current strategies for addressing RHAE focus on post-injury care rather than proactive neuroprotection, leaving athletes vulnerable to cumulative neurotrauma. Transcranial photobiomodulation (PBM) has shown promise in reducing neuroinflammation and promoting neuroprotection in traumatic brain injury; however, its potential to mitigate the structural brain changes assoc

Intraoperative endoscopic pylorus dilatation during minimally invasive Ivor Lewis oesophagectomy to prevent delayed gastric conduit emptying (DGCE): protocol for the WIDE randomised controlled trial.

INTRODUCTION: Oesophageal cancer has a poor prognosis and oesophagectomy has increasingly adopted minimally invasive techniques, but delayed gastric conduit emptying (DGCE) still occurs in ~10-50% of patients, depending on the used diagnostic criteria and definitions. DGCE can cause serious complications, such as aspiration pneumonia, and it prolongs hospital stay and the overall morbidity of patients. Surgical pyloric drainage procedures (pyloroplasty or pyloromyotomy) have not clearly improved outcomes. Endoscopic pyloric balloon dilatation (EPBD) effectively treats established DGCE, and eme

Impact of insurance and implant coverage on arthroscopic shoulder surgery patients: a prospective multicenter analysis.

BACKGROUND: Advances in implant technology for arthroscopic shoulder surgery allow patients to achieve similar success rates as traditional approaches with reduced morbidity and quicker recovery. However, in the U. S. health care system, insurance coverage for these implants remains variable. Unlike commercial carriers, patients with governmental insurance typically do not have coverage for arthroscopic shoulder implants. How this disparity impacts the health care of patients with shoulder pathology remains unclear. METHODS: We performed a prospective multicenter study analyzing the effects o

Sedoanalgesia with dexmedetomidine in daily anesthesia practices: a prospective randomized controlled trial.

BACKGROUND: Patient safety is important in daily anesthesia practices, and providing deep anesthesia is difficult. Current debates on the optimal anesthetic agents highlight the need for safer alternatives. This study was justified by the need for safer and more effective anesthetic protocols for outpatient hysteroscopic procedures, particularly those conducted outside the operating room. Propofol, while widely used, has significant limitations, including respiratory depression, hemodynamic instability, and delayed recovery when higher doses are required for adequate sedation. The addition of

Enhancing Compliance With Preoperative Fasting Guidelines: A Closed-Loop Quality Improvement Initiative to Optimize Patient Safety and Outcomes.

Introduction Preoperative fasting is essential in surgical care to reduce the risk of pulmonary aspiration during anesthesia. International guidelines, such as those from the American Society of Anesthesiologists (ASA) and the European Society of Anaesthesiology (ESA), recommend fasting durations of six hours for solids and two hours for clear liquids. However, adherence to these guidelines often varies in clinical practice, leading to prolonged fasting times that can negatively impact patient outcomes, including dehydration, hypoglycemia, discomfort, and delayed recovery. This quality improve
