Do patient age and sex influence tendon cell biology and clinical/radiographic outcomes after rotator cuff repair?

BACKGROUND: Many clinical and radiographic studies suggest that patient age and sex have an influence on rotator cuff (RC) repair outcomes. However, these findings result from retrospective statistical analyses and cannot provide a causal answer. PURPOSE: To analyze whether age and sex influence the biological potential at the time of RC repair or midterm clinical and radiographic outcomes. Also assessed was the effect of the biological potential on intraindividual clinical/radiographic results. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: A total of 40 patients underwent arth

Foreseeing the danger in the beach chair position: Are standard measurement methods reliable?

PURPOSE: The aim of this study is to show whether peripheral perfusion monitoring methods reflect central perfusion during shoulder arthroscopy at beach chair position. We hypothesized that mean arterial pressure (MAP), central heart rate (CHR) and peripheral oxygenation (SaO2) measurements individually will not parallel cerebral oximetry measurements by near-infrared spectroscopy (NIRS). METHODS: Between 2011 and 2012, 53 patients who had arthroscopic rotator cuff repair surgery in the beach chair position were enrolled prospectively. Median ages of the patients were 58 (range 42-68) years.

Effect of prior rotator cuff repair technique and timing on clinical outcomes of reverse shoulder arthroplasty.

AIMS: Reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) is commonly performed for recurrence of pain and arthritic progression; however, the influence of prior RCR timing and technique on outcomes of RSA has not been adequately explored. Our primary aim was to evaluate the relationship between the postoperative American Shoulder and Elbow Surgeons (ASES) score, whether prior RCR was performed open or arthroscopic, and the time interval between RSA and prior RCR. Secondary outcomes were forward elevation (FE) and external rotation (ER). METHODS: A prospectively collecte

Enthesis-related progenitors recruited from the subacromial bursa contribute to rotator cuff healing in rats.

BACKGROUND: Understanding the origin and regulatory mechanisms of enthesis-related progenitor cells expressing scleraxis (Scx) and SRY-box-containing gene 9 (Sox9) may help develop therapeutic approaches to improve endogenous rotator cuff (RC) tendon-to-bone healing capabilities. This study explores the characteristics of Scx+/Sox9+ cells during RC healing. The effects of subacromial bursa (SAB) preservation on Scx+ and Scx+/Sox9+ progenitor cell recruitment and the histological and biomechanical maturation of postoperative RC tendon-to-bone healing are evaluated in rats. METHODS: Twelve-week

The effect of the nonselective TNF inhibitor etanercept and of the selective TNF inhibitor XPro1595 on lesioned supraspinatus muscle.

The cytokine tumor necrosis factor (TNF), a major regulator of inflammatory responses, exists in both a membrane-bound form and a soluble form. We used the nonselective TNF inhibitor etanercept and the selective inhibitor XPro1595 and compared supraspinatus muscle cytokine levels, histology, and proteomic signatures in mice after supraspinatus tendon tear. The aim was to investigate the effect of anti-TNF treatment on the early inflammatory response in the muscle after tendon tear. In addition, the effect on body composition and bone mineral content was compared in naive mice after 2 months of

Lower trapezius tendon transfer vs. reverse shoulder arthroplasty for massive irreparable rotator cuff tears: an in-vivo scapulohumeral rhythm dynamic radiography study.

BACKGROUND: Reverse shoulder arthroplasty (RSA) and arthroscopic-assisted lower trapezius tendon (aLTT) transfer are both treatment options for massive irreparable rotator cuff tears (MIRCTs) without glenohumeral arthritis; however, the indications for each remain controversial. Although RSA is an excellent reconstructive option, preserving the joint, recentering the humeral head, and restoring in-line active external rotation with an aLTT transfer may be preferred in certain patients to better restore more normal shoulder biomechanics. In-vivo kinematics, however, have been historically diffi

Effects of a web application based on multimedia animations to support therapeutic exercise for rotator cuff-related shoulder pain: protocol for an open-label randomised controlled trial.

INTRODUCTION: Rotator cuff-related shoulder pain (RCRSP) is the most common cause of shoulder pain. Currently, exercise is proposed as the first-line treatment for patients suffering from RCRSP. However, adherence to therapeutic exercise programmes can be poor in the long term in a home setting. The aim of this study is to evaluate the effects of adding video animations to a traditional paper-based exercise programme. METHODS AND ANALYSIS: A single-centre, randomised, open-labelled clinical trial will be conducted in a hospital in Spain. Adults aged between 18 and 80 years diagnosed with RCRS

Dry needling as an adjunct treatment to multimodal rehabilitation protocol following rotator cuff repair surgery: a preliminary, randomized sham-controlled trial.

BACKGROUND: Rotator cuff repair (RCR) is one of the most prevalent procedures to manage rotator cuff tears (RCT). Postoperative shoulder pain is a common complication following RCR and may be aggravated by activation of myofascial trigger points (MTrP) associated with the injury to the soft tissues surrounding the surgical incision. This study aimed to describe a preliminary, randomized, sham-controlled trial to evaluate the effectiveness of implementing 4 sessions of myofascial trigger point dry needling (MTrP-DN) as a muscle treatment approach along with 10 sessions of multimodal rehabilitat
