Evaluation and treatment of the dominant dorsal duct syndrome (pancreas divisum redefined).

The controversial association of pancreatitis and pancreas divisum was studied in 100 patients (77 women, 23 men, median age 35) with episodic acute pancreatitis (49%) or "pancreatic pain" (51%). Seventy-one had classic pancreas divisum (type 1); 23 had only a dorsal duct with an absent Wirsung's duct (type 2); and 6 had a filamentous connection between the two duct systems (type 3). Accessory papilla sphincteroplasty was performed in 88 patients, with a mean follow-up of 53 months. The orifice was stenotic at the mucosal level in 66 patients. Seventy percent of patients have shown improvement

[Transduodenal papillosphincterotomy in the treatment of nontumorous lesions of the terminal portion of the common bile duct].

For benign stricture, obstruction of the major duodenal papilla (MDP) and terminal common bile duct in 130 patients with acute and chronic cholecystitis and its complications, the papillosphincterotomy was performed. Postoperative lethality was 3.8%. A good long-term result was noted in 84% of the patients, satisfactory--in 12%, bad--in 4%. There were no restenosis of MDP and recurrent choledocholithiasis.

Fo … , during which membranous hydatid material was observed emerging from the major papilla. Endoscopic sphincterotomy and balloon sweeping were performed to remove the obstructing material, followed by placement of a self-expandable metal stent for biliary drainage. Lemmel syndrome: the role of prudent radiological evaluation - a case report.

INTRODUCTION AND IMPORTANCE: Lemmel syndrome (LS) is a rare cause of obstructive jaundice resulting from compression of the distal common bi … phy (ERCP) confirmed LS caused by food debris within the diverticulum. Endoscopic balloon sweep and sphincterotomy achieved complete resolution, and the patient remained symptom-free at 6 months. CLINICAL DISCUSSION: The case highlights a diagnostic pitfall, which is an over-reliance on radiologists' interpretations and printed MRCP images. Review of thin-slice …

Intra-Abdominal Hemorrhage Following Endoscopic Retrograde Cholangiopancreatography in a Patient with a Cascade Stomach: A Case Report.

Intra-abdominal hemorrhage is an extremely rare complication of endoscopic retrograde cholangiopancreatography (ERCP). We report a case of i … eved using a large-diameter sliding tube (ST-CB1; Olympus), followed by biliary stent placement. No sphincterotomy or other invasive procedure was performed. Two hours after the procedure, the patient developed severe epigastric pain, hypotension, and progressive anemia. Contrast-enhanced computed tomography (CT) revealed a large hematoma centered on the gastros …

Endoscopic management of biliary stricture in primary sclerosing cholangitis.

Primary sclerosing cholangitis (PSC) is a progressive fibroinflammatory disease characterized by multifocal biliary strictures, recurrent ch … uding the use of periprocedural antibiotic prophylaxis, technique modifications, and individualized sphincterotomy decisions. As innovative diagnostic and therapeutic strategies for PSC continue to evolve, rigorous multicenter, prospective studies are needed to assess the efficacy, safety, and cost-effectiveness of these strategies before widespread adoption.

Temporary sphincter-preserving covered biliary stent with a frontal umbrella-shaped occlusive mechanism for common bile duct stone removal.

Currently, endoscopic retrograde cholangiopancreatography (ERCP) has been established as standard treatment for common bile duct (CBD) stones. However, this procedure requires endoscopic sphincterotomy (EST), which compromises function of the sphincter of Oddi and may lead to adverse events (AEs) such as bleeding, perforation, biliary reflux of intestinal contents, and stone recurrence. It is particularly unsuitable for patients requiring long-term anticoagulation therapy. In light of this, several researchers, including our team, have attempted to place a self-expandable metal stent (SEMS) in…

Endoscopic Use of N-Butyl-2-Cyanoacrylate in Refractory Pancreatic Duct Leak and Cystic Duct Leak: Is It Really a Last Resort?

Background/Objectives: The management of refractory pancreatic duct (PD) and cystic duct leaks may represent an endoscopic challenge. Standard endoscopic therapy involves pancreatic/biliary sphincterotomy and stenting during endoscopic retrograde cholangiopancreatography (ERCP). After conservative (fasting, parenteral nutrition, and use of somatostatin analogs) or conventional endoscopic treatments fail, a surgical approach is usually required, leading to higher mortality due to several technical complications. Previous evidence of the endoscopic use of N-butyl-2-cyanoacylate (NBCA) for treati…

All MS were placed after sphincterotomy. Pancreatitis occurred in four patients (11.4%) and no early AEs were observed. Short-Term Efficacy of 2% Topical Diltiazem in Fissure in Ano: A Prospective Observational Study From a Tertiary Care Centre in a Sub-Himalayan Region in India.

INTRODUCTION: An anal fissure is a common anorectal condition characterised by a linear tear in the lower anal canal, causing severe pain and rectal bleeding. Topical chemical sphincterotomy using agents such as diltiazem has become a popular non-surgical treatment option. METHODS: This prospective observational study was carried out over 12 months at the Department of Surgery, Himalayan Institute of Medical Sciences, Dehradun, India. Six hundred patients aged 18-60 years with acute, acute-on-chronic, or chronic anal fissure and no previous treatment history were enrolled. Patients received 2…

Challenges in the management of acute lithiasic cholangitis due to a long-retained plastic biliary stent: A case report.

BACKGROUND AND IMPORTANCE: This case report focuses on a rare cause of acute lithiasis cholangitis, which is residual choledocholithiasis on a plastic biliary stent that was placed nine years prior. CASE PRESENTATION: An 87-year-old male, with a history of hypertension and previous surgery for gallstone disease including cholecystectomy and placement of a Kehr drain in 2006, was diagnosed with residual stones in 2008 and received a plastic biliary stent after endoscopic sphincterotomy.
