Abdominal and pericardial ultrasound in suspected extrapulmonary or disseminated tuberculosis.

OBJECTIVE: Tuberculosis (TB) in patients with or without advanced HIV infection may present as smear-negative, extrapulmonary and/or disseminated forms. We studied the role of pericardial and abdominal ultrasound examinations in the determination of extrapulmonary or disseminated TB. METHODS: A prospective descriptive and analytic cross-sectional study design was used to determine the ultrasound findings of value in patients with subsequently proven TB. Ultrasound examinations were performed on 300 patients admitted to G F Jooste Hospital with suspected extrapulmonary or disseminated TB. OUT

Development and validation of a diagnostic clinical prediction model for suspected pulmonary tuberculosis in adults: A retrospective analysis.

OBJECTIVE: Early and accurate diagnosis of pulmonary tuberculosis (PTB) is critical for disease control. This study aimed to develop and validate a clinical prediction model to differentiate adult inpatients with suspected PTB. METHODS: A retrospective cohort of 602 adults admitted to Changsha Central Hospital (January-October 2019) with suspected PTB was randomly divided into a modeling group (n = 421) and an internal validation group (n = 181). Univariate and multivariate logistic regression analyses in the modeling cohort identified independent clinical and radiological predictors. Model p

Impact of drainage strategies on recovery in Stage III tuberculous empyema: a retrospective study.

INTRODUCTION: Tuberculous empyema, a severe complication of pulmonary tuberculosis, often requires surgical intervention in stage III to remove fibrotic tissue and restore lung function. METHODOLOGY: This retrospective study enrolled 224 stage III tuberculous empyema patients undergoing single-port thoracoscopic decortication and closed chest drainage. Patients were divided into three groups: Single-Tube group (n = 42), Double-Tube group (n = 51), and Double-Tube with Negative Pressure (Double-NP) group (n = 131, with -8 to -10 cm H₂O negative pressure applied from postoperative day 2). Analysis of factors influencing high infectivity in patients with tuberculosis using clinical and imaging methods.

BACKGROUND: To optimise early screening for hospitalised patients with highly contagious tuberculosis (TB) to allow for effective isolation measures to reduce the risk of TB transmission. METHODS: Data were collected from 9458 newly diagnosed patients with TB from 1 January 2015 to 31 December 2023 at a designated TB hospital in Tianjin. The patients were divided into two groups based on their infection status: highly infectious and less infectious. Data, including clinical characteristics and computerised tomography (CT) examination results, were extracted from the hospital information system

Chest X-Ray Findings Comparison between Multi-drug-resistant Tuberculosis and Drug-sensitive Tuberculosis.

BACKGROUND: Imaging has a big role in tuberculosis (TB) diagnosis and chest X-ray is preferable because it is available in primary health care and can point out the location, area, and morphology of lesions, such as cavity, consolidation, pleural effusions, and fibrosis. We aimed to compare the chest X-ray findings in multi-drug resistant TB (MDR-TB) and in drug-sensitive TB (DS-TB) cases. METHODS: This is a retrospective cross-sectional study which compares chest X-ray findings of two groups of patients, involving 183 DS-TB patients and 183 MDR-TB patients. Radiologic findings that we analyz

Complete debridement for treatment of thoracolumbar spinal tuberculosis: a clinical curative effect observation.

BACKGROUND CONTEXT: Traditional focal debridement involves clearing of cold abscesses, caseous necrosis, residual intervertebral disc tissue, sinus tracts, bony sequestration, and inflammatory granulation. Reports have demonstrated that approximately 13% to 26% of patients were not better or relapsed after traditional focal debridement; these patients required a second surgery or prolonged antituberculous therapy. The presence of retained and diseased focal tissues requiring removal remains poorly understood. The contents of these retained tuberculous foci, improvement of surgical strategies,

Anaphylatoxins Enhance Recruitment of Nonclassical Monocytes via Chemokines Produced by Pleural Mesothelial Cells in Tuberculous Pleural Effusion.

In the present study, we sought to elucidate the mechanisms by which monocytes migrate into the pleural space in the presence of anaphylatoxins in tuberculous pleural effusion (TPE). Monocytes in both pleural effusion and blood were counted, and their phenotypic characteristics were analyzed. Activation of the complement system was detected in TPE. The effects of Mpt64 and anaphylatoxins on the production of chemokines in pleural mesothelial cells (PMCs) were measured. The chemoattractant activity of chemokines produced by PMCs for monocytes was observed. Levels of CD14+CD16+ monocytes were si

"Silent" post-menopausal genital tuberculosis with lethal outcome.

Female genital Tuberculosis (TB) is a rare disease, that usually affects female patients of reproductive age living in developing countries. The diagnosis of the disease is quite difficult since there is no consensus on the appropriate screening and management needed. Female genital TB can mimic many other pathologies including advanced ovarian cancer. Laboratory findings or blood serum tests usually are not suggestive of the disease. Imaging findings can be misleading. The incidence of the disease in Greece is rare. We present a rare case report of a post-menopausal patient with female genita

Development and validation of novel diagnostic nomogram for tuberculous pleurisy based on TB-IGRA results.

OBJECTIVE: To establish the diagnostic nomogram for tuberculous pleurisy (TP) based on TB-interferon-gamma release assays (TB-IGRA), as well as clinical and peripheral blood characteristics.MATERIAL AND METHODS: Patients who underwent TB-IGRA tests during hospitalisation and were finally diagnosed, were retrospectively and continuously enrolled. TP was divided into confirmed TP (cTP) and presumptive TP (pTP), and corresponding diagnostic nomograms were established.RESULTS: A total of 1283 patients were enrolled (median age 49 years, range 14-96; males: 63.1%). The area under the curve (AUC) of

"Luck's always to blame": silent wounds of a penetrating gunshot trauma sustained 20 years ago.

Gunshot tracheal injuries represent life-threatening events and usually necessitate emergent surgical intervention. We report a case of an exceptional finding of a patient with retained ballistic fragments in the soft tissues of the thorax, proximal to the right subclavian artery and the trachea, carrying silently his wounds for two decades without any medical or surgical intervention. The bullet pellet on the upper part of the trachea seen accidentally in the chest computed tomography, was also found during bronchoscopy. In short "luck's always to blame".

Dry pleurisy complicating solitary pulmonary nodules caused by Mycobacterium avium: a case report.

INTRODUCTION: Mycobacterium avium complex (MAC) lung disease presenting as a solitary pulmonary nodule (MAC-SPN) is often asymptomatic, is more common in middle to old age, and mimics lung cancer or tuberculoma. We report herein a case of MAC-SPN in an immunocompetent young adult patient, presenting with persistent chest pain and a subacutely progressive nodule with high intense (18)F-fluorodeoxyglucose uptake. Histological examination of resected specimens revealed pleurisy, which is a rare finding of MAC-SPN. CASE PRESENTATION: A 36-year-old Japanese male presented with chest pain and a sub

Detection of drug-resistant Mycobacterium tuberculosis in pericardial fluid culture and its correlation with cartridge based nucleic acid amplification test and adenosine deaminase activity.

BACKGROUND: Pericardial effusion is the accumulation of fluid in the pericardial cavity. In nations with high tuberculosis (TB) load, TB is the most common cause of pericardial effusion. 1-2% of patients with pulmonary TB develop Pericardial TB worldwide. Multi-drug-resistant (MDR) TB, including extrapulmonary TB (EPTB) cases, are rising in number. Adenosine Deaminase (ADA) is an enzyme in lymphocytes and myeloid cells, which has certain immune functions in the body. ADA levels are increased in inflammatory conditions, like pleural, pericardial, or joint effusions, of bacterial etiology, granu
