Medial elbow exposure for coronoid fractures: FCU-split versus over-the-top.

OBJECTIVE: The optimal exposure interval for anteromedial coronoid fractures is unknown. The purpose of this study was to quantitatively compare the osseous and ligamentous exposure of the medial elbow using the flexor carpi ulnaris (FCU)-Splitting and Hotchkiss Over-the-Top approaches. METHODS: Forty surgical approaches were performed on 20 fresh-frozen cadaveric elbows using a randomized crossover design. Access to key anatomic landmarks [anteromedial facet, coronoid tip, sublime tubercle/anterior bundle of the medial collateral ligament (MCL), posterior bundle of the MCL, and radial head]

Application of recombinant BMP-7 on persistent upper and lower limb non-unions.

The purpose of this study was to evaluate the efficacy and safety of recombinant bone morphogenetic protein 7 (rhBMP-7 or OP-1) as a bone-stimulating agent in the treatment of persistent fracture non-unions. Twenty-five consecutive patients [19 males, mean age 39.4 years (range: 18-79)] with 26 fracture non-unions were treated with rhBMP-7. There were 10 tibial non-unions, eight femoral, three humeral, three ulnar, one patellar, and one clavicular non-union. The mean follow-up was 15.3 months. The mean number of operations performed prior to rhBMP-7 application was 3.2, with autologous bone gr

The anatomical basis for anterior interosseous nerve palsy secondary to supracondylar humerus fractures in children.

INTRODUCTION: Various studies have found that 6.6 to 31% of supracondylar elbow fractures in children have nerve-related complications. One-third of these are cases of anterior interosseous nerve (AIN) palsy that usually result in a deficit of active thumb and index flexion. The goal of this cadaver study was to describe the course of the AIN to achieve a better understanding of how it may get injured. MATERIALS AND METHODS: On 35 cadaver specimens, the median nerve and its collateral branches destined to muscles were dissected at the elbow and forearm levels. The distance at which the variou

Intramedullary screw fixation for simple displaced olecranon fractures.

PURPOSE: Olecranon fractures are common and typically require surgical fixation due to displacement generated by the pull of the triceps muscle. The most common techniques for repairing olecranon fractures are tension-band wiring or plate fixation, but these methods are associated with high rates of implant-related soft-tissue irritation. Another treatment option is fixation with an intramedullary screw, but less is known about surgical results using this strategy. Thus, the purpose of this study was to report the clinical and functional outcomes of olecranon fractures treated with an intramed

A new way to treat forearm post-traumatic non-union in young patients with intramedullary nailing and platelet-rich plasma.

INTRODUCTION: Non-union rate in forearm fractures is generally less than 2% when a proper technique is used; this rate increases when ulnar lesions are involved. PATIENTS AND METHODS: We present a case series of seven young patients whose average age was 14 years (range 11-19 years) at the time of surgery and who presented with a forearm post-traumatic non-union that was previously treated in different ways (three isolated ulnar non-union, two isolated radial non-union and two combined). Average follow-up was 34 months (range 9-72 months). Surgical treatment began with the removal of the prev

Early range of motion exercise in pediatric patients with olecranon fractures treated with tension band suture with double loops and double knots.

BACKGROUND: Pediatric patients with olecranon fractures are uncommon. The tension band suture technique was introduced to reduce the burden of implant removal and other complications. However, to our knowledge, early range of motion (ROM) exercise has not been introduced in this population of patients. Double Vicryl loops and knots with 2 cross-pins are used to maintain the benefits of the tension band suture technique and to enhance fixation tensile strength. We believe that early ROM exercises could be achieved without nonunion or fixation failure. METHODS: Twelve pediatric patients with ol

Epidemiology of pediatric forearm fractures in Washington, DC.

BACKGROUND: Pediatric forearm fractures result in substantial morbidity and costs. Despite the success of public health efforts in the prevention of other injuries, the incidence of pediatric forearm fractures is increasing. Our objective is to characterize the epidemiology of forearm fractures in Washington, DC, children evaluated in an urban pediatric emergency department (ED). METHODS: This retrospective study includes Washington, DC, children, aged 0 years to 17 years, treated for an isolated forearm fracture in the Children's National Medical Center ED from 2003 to 2006. Patients with bo

Role of orthopedic implants and bone morphology in the identification of human remains.

When conventional methods of identification, such as visual recognition and dental comparison, cannot be used to identify a deceased person, it becomes necessary to consider alternative methods. The presence of an orthopedic implant in a body may assist identification if ante-mortem medical records are available for comparison. Another method of identification involves comparison of ante-mortem and postmortem radiographs. Eight cases are reported from Forensic Science SA where the presence of orthopedic implants and/or ante-mortem radiographs were used to try to establish identification. In si

Complication rate and factors affecting outcome of olecranon osteotomies repaired with pin and tension-band fixation in dogs.

The objective of this study was to determine the complication rate and evaluate factors affecting the outcome of olecranon osteotomy in dogs. Medical records were searched to identify dogs that had undergone olecranon osteotomy (stabilized with 2 Kirschner wires and a figure-of-8 wire) during internal fixation of a supracondylar or condylar humeral fracture. Signalment, description of the fracture, parameters regarding the osteotomy and its repair, and radiographic outcome were recorded. A logistic regression model compared patient and technical parameters with the osteotomy outcome. Of the 19

Fractures of the olecranon: an in vitro study of elbow joint stresses after tension-band wire fixation versus proximal fracture fragment excision.

BACKGROUND: Displaced fractures of the olecranon usually require operative treatment, by either open reduction with internal fixation (ORIF) or excision of the proximal fragment. However, the relative merits of these treatment options have not been fully delineated. One treatment outcome measure of joint function is residual intra-articular stress. The purpose of this study was to evaluate the effect of these two types of olecranon fracture treatment on humeroulnar joint stress. METHODS: Eight matched pairs of fresh frozen cadaveric upper extremities were thawed; stripped of skin, muscular, a

Proximal radial resection for posttraumatic radioulnar synostosis: a new technique to improve forearm rotation.

BACKGROUND: Heterotopic ossification about the elbow joint can lead to considerable functional disability, including the loss of forearm rotation. Many procedures have been described for the treatment of proximal radioulnar synostosis. Varying degrees of success have been achieved with regard to the improvement of the flexion arc, but less success has been reported in terms of the restoration of forearm rotation. The success of treatment is associated with the extent of heterotopic ossification, soft-tissue scarring, and anatomical distortion. A new and simple technique to address the unresect

Chronic radial head dislocation in children, Part 1: pathologic changes preventing stable reduction and surgical correction.

The pathologic changes in 15 elbows of 14 children (2 bilateral congenital, 12 unilateral posttraumatic, mean age 9.5 years) with radial head dislocation unreduced for at least 3 months (range 3-180 months) and their open treatment were reviewed. Common dysplastic changes observed in both congenital and posttraumatic groups included large deformed radial heads, slender radial necks, and ulnar bowing. Changes were bilateral and symmetrical in the congenital group. In traumatic dislocations, patients with more severe deformities had longer delays from time of injury to time of reduction. Persist

The Impact of COVID-19-Related Restrictions on the Incidence of Diaphyseal and Distal Forearm Fractures: A Retrospective Analysis.

Background and Objectives: Pediatric forearm fractures are among the most common childhood injuries. COVID-19-related societal restrictions, including school closures and suspension of sports activities, altered children's daily routines and may have influenced injury patterns. This study aimed to evaluate whether periods of stricter COVID-19 restrictions were associated with changes in the incidence of pediatric distal and diaphyseal forearm fractures after accounting for seasonal variation and long-term temporal trends. Materials and Methods: This retrospective observational time-series stud

What injury makes an isolated small coronoid fracture? Morphological analysis using three-dimensional computed tomography reconstruction.

BACKGROUND: Small, isolated coronoid fractures involving less than one-third of the coronoid height are often underestimated on plain radiographs due to minimal displacement and preserved joint congruity. However, these fractures can be associated with significant soft tissue injuries, leading to occult elbow instability. This study aimed to investigate the mechanisms and characteristics of isolated small coronoid fractures and to analyze their three-dimensional (3D) morphology using computed tomography (CT)-based modeling. METHODS: We retrospectively reviewed 27 cases of isolated, noncomminu

US-guided axillary nerve block vs. fracture hematoma block for forearm fracture analgesia in the ED: A randomised controlled trial.

AIM: Fracture hematoma blocks (FHB) are commonly used for analgesia in distal forearm fracture reduction. However, the effectiveness is suboptimal due to variability in technique among physicians and fracture type and the procedure is often perceived as painful by patients. This study investigated whether an ultrasound-guided axillary nerve block (ANB) is an effective alternative for analgesia during reduction of displaced distal forearm fractures in patients in the emergency department (ED). METHOD: In this randomised controlled trial, we compared pain scores on the numeric rating scale (NRS

Does the coronoid fracture in terrible triad injury always need to be fixed?

BACKGROUND: The ideal treatment of terrble triad injuries and whether fixation of coronoid process fractures is needed or not are still debated. Therefore, we aimed to investigate if terrible triad injuries necessitate coronoid fracture fixation and evaluate if non-fixation treatments have similar efficacies and outcomes as fixation-treatments in cases of terrible triad injuries. METHODS: From August 2011 to July 2020, 23 patients with acute terrible triad injuries without involvement of the anteromedial facet of the coronoid process were included to evaluate the postoperative clinical and ra

Three-dimensional quantitative study and functional outcome analysis of coronoid fracture in different elbow injury patterns.

BACKGROUND: Coronoid fractures are often part of a complex fracture-dislocation of the elbow. For an optimum prognosis, it is important to understand the characteristics of coronoid fractures in different elbow injury patterns. Significant differences in these characteristics have been observed in various studies, but these previous studies have suffered from limitations, such as insufficient quantification and unknown prognostic differences among different injury patterns. Therefore, we aimed to quantitatively analyze coronoid fracture characteristics and functional outcomes in different elbo

Management of Major Traumatic Upper Extremity Amputations.

Traumatic upper extremity amputation is a life-altering event, and recovery of function depends on proper surgical management and postoperative rehabilitation. Many injuries require revision amputation and postoperative prosthesis fitting. Care should be taken to preserve maximal length of the limb and motion of the remaining joints. Skin grafting or free tissue transfer may be necessary for coverage to allow preservation of length. Early prosthetic fitting within 30 days of surgery should be performed so the amputee can start rehabilitation while the wound is healing and the stump is maturing
