Ethical decision-making for extremely preterm deliveries: results of a qualitative survey among obstetricians and midwives.

OBJECTIVE: To explore the practices, attitudes and feelings of obstetricians and midwives in cases of extreme prematurity. METHODS: A qualitative study was conducted as part of a European Concerted Action (EUROBS) in three tertiary-care maternity units, located in three cities in the northern, southern and central areas of France. Semi-structured interviews lasted an average of 60 min and were tape-recorded. They were independently analyzed by two different researchers using a content analysis. All full-time obstetricians and half of the full-time midwives were eligible for the study. Overall

Hypothesis for endometrial carcinoma carcinogenesis. Preventive prospects.

Considering endometrial carcinoma as a natural experimental model for in vivo study of carcinogenesis, a hypothesis of endometrial type A carcinogenesis and some preventive prospects are advanced. Under the name of endometrial carcinoma two different types are considered: A) hormone dependent type, and B) autonomous type. Aging, obesity, hypertension and/or diabetes, persistent exposure to unopposed exogenous or endogenous estrogens are recognized epidemiological factors for endometrial carcinoma. Experimental and clinical studies have shown that in pregnancy associated with clinical condition

Generational Differences in Illness-Related Uncertainty Among Haemophilia Carriers: A Qualitative Study Using Mishel's Uncertainty in Illness Theory.

AIM: Haemophilia carriers (HCs) have historically been regarded as asymptomatic or only mildly affected, resulting in under-recognition of bleeding symptoms and unmet healthcare needs. Although advances in healthcare and access to information have changed the experience of haemophilia, little is known about how illness-related uncertainty differs across generations. Guided by Mishel's Uncertainty in Illness Theory, this study compared illness-related uncertainty between a younger generation of HCs and a previously studied maternal cohort. METHODS: Semi-structured interviews were conducted wit

The cost of preterm birth: the low cost versus high value of tocolysis.

The consequences of preterm birth (PTB), to the individual and society at large, remain a major financial and personal burden. Babies born at the limits of viability, who survive, often have major neurological impairments, such as cerebral palsy, developmental delay and blindness. The cost of initial hospitalisation is more than $200,000 for each birth but takes no account of future costs once they leave the hospital. The major morbidities associated with extreme prematurity are respiratory distress syndrome (RDS), intraventricular haemorrhage (IVH) and necrotising enterocolitis (NEC). With ad

Postpartum Virologic Failure Among Women with HIV Suppressed at Delivery in Spain: Incidence, Timing, and Associated Factors.

The postpartum period remains a vulnerable phase for maintaining virologic suppression among women with HIV (WWH); however, evidence on postpartum virologic failure (VF) among women virologically suppressed at delivery is limited. We analyzed data from the CoRIS cohort including WWH aged 18-50 years who had a live birth while receiving antiretroviral therapy (ART) and were virologically suppressed at delivery between 2004 and 2024. The primary outcome was time to VF within 12 months postpartum, defined as two consecutive HIV RNA measurements >50 copies/mL or a single measurement >1000 copies/m

Outcomes in Early Adulthood for Individuals Born Very Preterm and/or with Very Low Birth Weight: Evidence from Multinational Cohorts.

BACKGROUND: Advances in neonatal care have improved survival rates for infants born very preterm (VP) and/or with very low birth weight (VLBW), yet their long-term outcomes into adulthood remain understudied. OBJECTIVES: To assess the impact of VP/VLBW status on mortality, educational attainment, and labor market outcomes in early adulthood using data from the RECAP Preterm Project. METHODS: We used harmonized data from 5 nationally representative cohort studies in high-income countries (Canada, Germany, the Netherlands, New Zealand, and Norway) participating in the RECAP Preterm Project. Randomized control trials demonstrate that nutrition-sensitive social protection interventions increase the use of multiple-micronutrient powders and iron supplements in rural pre-school Bangladeshi children.

OBJECTIVE: To examine the impact of a nutrition-sensitive social protection intervention on mothers' knowledge of Fe deficiency, awareness of multiple-micronutrient powders (MMP) and the consumption of MMP and other Fe supplements by their children aged 6-59 months. DESIGN: Two randomized controlled trials with treatment arms including cash transfers, food transfers, cash and food transfers, cash and nutrition behaviour change communication (BCC), and food and nutrition BCC were implemented over two years. Both included a control group that received no transfer or BCC. Transfer recipients wer

Intrauterine hyperglycemia-induced inflammatory signalling via the receptor for advanced glycation end products in the cardiac muscle of the infants of diabetic mother rats.

PURPOSE: Gestational diabetes is associated with increased risk to the health of the mother and her offspring. In particular, the infants of diabetic mothers (IDMs) exhibit elevated levels of preterm birth, macrosomia, hypoglycemia, hypocalcemia, and cardiomyopathy. We have previously reported that IDMs showed abnormalities in cardiac Akt-related insulin signalling, and that these deficiencies in Akt-related signalling were attenuated by supplementing the maternal diet with fish-oil. Herein, we investigated whether the eicosapentaenoic acid (EPA) found in fish oil can be used to attenuate diab

Decision-to-Delivery Time and Perinatal Complications in Emergency Cesarean Section.

BACKGROUND: A decision-to-delivery interval (DDI) of no more than 20 minutes has long been considered a requirement for cesarean sections, even though there have hardly been any studies on this topic. We retrospectively investigated data relevant to DDI for emergency cesarean sections performed for the most common indications, namely, suspected and documented fetal asphyxia. METHODS: We analyzed data on emergency in-hospital cesarean sections in the period 2008-2015. Low 5- and 10-minute Apgar scores (a scheme with points awarded for breathing, heart rate, muscle tone, skin coloration, and th

Review of cytomegalovirus shedding in bodily fluids and relevance to congenital cytomegalovirus infection.

Congenital cytomegalovirus (CMV) infections are a leading cause of sensorineural hearing loss (SNHL) and neurological impairment. Congenital transmission of CMV can occur with maternal primary infection, reactivation, or reinfection during pregnancy. We reviewed studies of CMV shedding in bodily fluids (defined as CMV detected by culture or CMV DNA detected by polymerase chain reaction). Following diagnosis at birth, children with congenital CMV infection exhibited the highest prevalences of CMV shedding (median = 80%, number of sample population prevalences [N] = 6) and duration of shedding,

Aetiology of community-acquired neonatal sepsis in low and middle income countries.

BACKGROUND: 99% of the approximate 1 million annual neonatal deaths from life-threatening invasive bacterial infections occur in developing countries, at least 50% of which are from home births or community settings. Data concerning aetiology of sepsis in these settings are necessary to inform targeted therapy and devise management guidelines. This review describes and analyses the bacterial aetiology of community-acquired neonatal sepsis in developing countries. METHODS: A search of Medline, Embase, Global Health and Web of Knowledge, limited to post-1980, found 27 relevant studies. Data on

Endometrial Preparation for Frozen Embryo Transfer: Protocols and Pregnancy Outcomes.

Frozen embryo transfer (FET) has become widely used over the past decade, largely because of advances in vitrification techniques. To optimize pregnancy rates in FET cycles, accurate synchronization between embryo development and endometrial receptivity is essential. However, the optimal individualized approach to endo-metrial preparation remains a matter of ongoing debate. This review summarizes the current literature on endo-metrial preparation protocols and their impact on preg-nancy and perinatal outcomes, with a particular focus on artificial, natural, and stimulated cycles. Although many

A comparison of long-term maternal mortality associated with pathologic placental separation: Highlighting possible trends and mechanisms.

Abruption and retention, two types of abnormal placental separation, are associated with significant morbidity and mortality. Though advancements in obstetric management have improved peripartum injury, patients who experience abnormal placental separation may be at risk for long-term complications. This study evaluates long-term maternal mortality in patients with abruption or retention compared to those with normal placental separation. In our cohort of 638,911 vaginal deliveries (625,890 normal, 5,435 abruption, 7,586 retention), the mortality rate was 6.4 per 1,000 in normal deliveries, 9.

Transamniotic stem cell therapy (TRASCET).

Transamniotic stem cell therapy (TRASCET) constitutes the least invasive approach to prenatal stem cell delivery described to date. This strategy is largely based on the recently discovered active transport of donor cells from the amniotic fluid to the placenta directly through the amnio-placental interface. From a simple amniocentesis, via the placenta, donor cells of different phenotypes can reach the fetal circulation and virtually any fetal anatomical site, including, and particularly, the fetal bone marrow. As a result, TRASCET has shown broad therapeutic potential in an assortment of ani

How nonhuman primate infant and mother-infant models can inform pediatric HIV-1 cure research.

PURPOSE OF REVIEW: An estimated 1.4 million children live with HIV-1, yet major gaps remain in understanding neonatal pathogenesis and curative strategies. Nonhuman primate (NHP) infant and mother-infant models provide a critical platform to investigate immune development, elucidate age-specific mechanisms of infection, and evaluate treatment responses, aspects that cannot be directly studied in humans. RECENT FINDINGS: Newborn macaques exhibit exaggerated gut and lymph nodes seeding, absent interferon-I responses, and accelerated progression compared to juveniles, while ultra-early antiretro

Iron Deficiency and Iron Deficiency Anemia: A Comprehensive Overview of Established and Emerging Concepts.

Iron deficiency (ID) is the most prevalent micronutrient deficiency globally, affecting approximately one in four individuals, with a particularly high burden among children, women of reproductive age, and populations in low- and middle-income countries. It contributes significantly to the global burden of disease, with consequences ranging from impaired cognitive and motor development in children to increased risks during pregnancy, including low birth weight, preterm delivery, and maternal mortality, as well as reduced physical performance and quality of life in adults. ID often precedes iro

Predictive models for live birth outcomes following fresh embryo transfer in assisted reproductive technologies using machine learning.

BACKGROUND: Infertility affects approximately 15% of couples globally, with assisted reproductive technologies (ARTs) becoming the primary interventions. Despite the growing use of ARTs, success rates have plateaued at around 30%, highlighting the need for improved predictive models to enhance outcomes. This study aimed to develop a machine learning-based predictive model for live birth outcomes following fresh embryo transfer. METHODS: A total of 51,047 ART records were collected from 2016 to 2023 at the Shanghai First Maternity and Infant Hospital. After data preprocessing, 11,728 records a
