BACKGROUND: Patients' characteristics and antidepressants are discussed to be relevant in the context of phobic exposure.
AIMS: To identify patients characteristics associated with a differential course of fear during disorder-specific symptom provocation as well as to elucidate the effect of selective serotonin-(noradrenalin-) reuptake inhibitors [SS(N)RI] on development of fear in the context of re-exposure to the phobic stimuli.
METHODS: Twenty-eight clinically well-characterized patients with panic disorder and agoraphobia (PD/AG) were classified into subjects who show a reduction of fea…

A growing body of literature suggests that virtual reality is a successful tool for exposure therapy in the treatment of anxiety disorders. Virtual reality (VR) researchers posit the construct of presence, defined as the interpretation of an artificial stimulus as if it were real, to be a presumed factor that enables anxiety to be felt during virtual reality exposure therapy (VRE). However, a handful of empirical studies on the relation between presence and anxiety in VRE have yielded mixed findings. The current study tested the following hypotheses about the relation between presence and anxi

OBJECTIVE: Previous studies have shown that neuroactive steroids modulate anxiety and stress reactivity. However, no data on the possible role of these gamma-aminobutyric acid(A) (GABA(A)) receptor-modulating neuroactive steroids in patients with anxiety disorders are available.
METHOD: The concentrations of 3alpha,5alpha-tetrahydroprogesterone (3alpha,5alpha-THP), 3alpha,5beta-THP, 3beta,5alpha-THP, and their precursors were studied in the plasma of 10 patients with panic disorder and 10 matched healthy comparison subjects. In addition, the effects of paroxetine treatment on neuroactive ster

OBJECTIVE: To compare the relative efficacy and safety of lesopitron 4-80 mg/d versus lorazepam 2-4 mg/d and placebo in a subgroup of patients with anxiety history taken from a larger study of patients with a primary diagnosis of generalized anxiety disorder (GAD).
DESIGN: Six-week, randomized, double-blind, parallel, placebo and lorazepam-controlled, Phase II, single-center, outpatient study.
SETTING: Outpatient clinic.
PATIENTS: One hundred sixty-one patients with GAD were randomized in the main study; 68 with a documented history of GAD or anxiety disorder not otherwise specified were in

Thirty panic disorder patients were assigned randomly to four weekly sessions of either cognitive-behavioral therapy or nondirective, supportive therapy. Dependent variables included clinician ratings, standardized self-report questionnaires, and self-monitoring. Cognitive-behavioral therapy led to significant reductions in worry about the recurrence of panic, and in overall ratings of phobic distress. Nondirective supportive therapy did not produce significant effects. In addition, proportionately more patients who were actively panicking at preassessment and underwent cognitive-behavioral tr

Cholecystokinin (CCK) may mediate human anxiety and animal data suggest that cholecystokinin antagonists could provide an important advance in the treatment of anxiety disorders. The study of CCK receptor systems in psychiatric patients has, however, been severely limited by the lack of available probes. We utilized intravenous infusions of pentagastrin, a selective CCK-B receptor agonist, and studied behavioral and cardiovascular responses in 10 patients with panic disorder and 10 normal controls. Pentagastrin produced substantial symptomatology, including anxiety, and increases in heart rate

30 patients with "Anxiety disorder" were investigated with regard to their psychosocial experiences, their family history, the course of their illness, and their interpersonal relationships using a semi-structured interview. Patients with panic disorder significantly more frequently reported real fright of a parent than did patients with generalized anxiety disorder. Both groups showed a high degree of secondary depression and avoidance behavior in the course of their illness. Chronic anxiety was more frequent in patients with generalized anxiety disorder. The difference in the perception and

In order to evaluate adrenergic function in anxiety disorders, platelet alpha 2-adrenergic binding parameters and supine and standing blood pressure, pulse, and venous plasma epinephrine and norepinephrine were determined in patients with panic attacks or generalized anxiety disorder and in normal subjects. The maximum number of binding sites (Bmax) for the partial agonist tritiated clonidine was significantly lower for both patient groups than for normal subjects, and the Bmax for the antagonist tritiated yohimbine was significantly lower for panic patients. There were no other substantive di

Abnormalities of the hypothalamic-pituitary-adrenal (HPA) axis and noradrenergic function have been reported in patients with panic disorder. Mean urinary free cortisol and plasma MHPG were measured in 12 medication-free panic disorder patients and 12 normal controls. No significant difference in urinary free cortisol and plasma MHPG was observed between the patients and controls. There was no relationship between plasma MHPG and urinary free cortisol in the panic patients or normal controls. These findings are described within the context of current concepts of stress and noradrenergic dysfun

Outcome at discharge and during a follow-up averaging 5 years clearly distinguished 116 panic disorder inpatients from 123 age- and sex-matched inpatient controls with primary unipolar depression - 60.2% of the primary depression patients recovered at some time during follow-up compared to only 15.5% of the panic disorder patients. Differences in recovery rates grew larger with increasing follow-up length and were undiminished by the exclusion of patients who received antidepressants or convulsive therapy. Furthermore, these two groups had no predictors of outcome in common. These findings acc
