How does angiogenesis develop in pediatric moyamoya disease after surgery? A prospective study with MR angiography.

OBJECT: The dynamic process of neovascularization seen in patients with moyamoya disease who have received bypass surgery has not been sufficiently elucidated. Magnetic resonance angiography (MRA)is expected to disclose this process since it can be conducted frequently because of its non-invasive nature. In this study, we prospectively investigated postoperative neovascularization using repeated MRA. METHODS: We studied 14 pediatric patients (23 sides) who have received both direct bypass surgery of the superficial temporal artery (STA) to middle cerebral artery and indirect surgery of enceph

Carotid angioplasty in the management of external carotid artery stenosis associated with an occluded internal carotid artery.

The author describes the surgical treatment of 14 patients in whom angiography demonstrated total occlusion of the internal carotid artery (ICA) associated with stenosis at the origin of the ipsilateral external carotid artery (ECA). Each patient underwent ECA endarterectomy followed by an angioplasty to enlarge the ECA outflow tract by incorporating the ICA stump into the proximal ECA. Nine patients subsequently underwent superficial temporal-middle cerebral artery anastomosis. There was no mortality or permanent neurological morbidity. This method of treating ECA stenosis is recommended beca

Diagnostic Values of the "To and Fro" Conflict Sign on Intraoperative Indocyanine Green Video Angiography as a Warning Sign of the Focal Cerebral Hyperperfusion and Watershed Shift Phenomenon after Superficial Temporal Artery-Middle Cerebral Artery Bypass for Adult Patients with Moyamoya Disease.

UNLABELLED: <p>Introduction: The focal cerebral hyperperfusion (CHP) is a potential complication after superficial temporal artery-middle cerebral artery (STA-MCA) bypass for moyamoya disease (MMD) that can result in delayed intracerebral hemorrhage and/or neurological deterioration. The focal CHP could accompany hemodynamic ischemia due to the "watershed shift (WS) phenomenon." Preoperative prediction of the focal CHP and WS phenomenon remains challenging. Here, we aimed to assess the diagnostic value of the "to and fro" conflict sign, conflicting blood flow around the vascular territory of t

Extracranial-Intracranial Bypass and Risk of Stroke and Death in Patients With Symptomatic Artery Occlusion: The CMOSS Randomized Clinical Trial.

IMPORTANCE: Prior trials of extracranial-intracranial (EC-IC) bypass surgery showed no benefit for stroke prevention in patients with atherosclerotic occlusion of the internal carotid artery (ICA) or middle cerebral artery (MCA), but there have been subsequent improvements in surgical techniques and patient selection. OBJECTIVE: To evaluate EC-IC bypass surgery in symptomatic patients with atherosclerotic occlusion of the ICA or MCA, using refined patient and operator selection. DESIGN, SETTING, AND PARTICIPANTS: This was a randomized, open-label, outcome assessor-blinded trial conducted at

Novel Software-Derived Workflow in Extracranial-Intracranial Bypass Surgery Validated by Transdural Indocyanine Green Videoangiography.

BACKGROUND: The introduction of image-guided methods to bypass surgery has resulted in optimized preoperative identification of the recipients and excellent patency rates. However, the recently presented methods have also been resource-consuming. In the present study, we have reported a cost-efficient planning workflow for extracranial-intracranial (EC-IC) revascularization combined with transdural indocyanine green videoangiography (tICG-VA). METHODS: We performed a retrospective review at a single tertiary referral center from 2011 to 2018. A novel software-derived workflow was applied for

Clinical implications of the cortical hyperintensity belt sign in fluid-attenuated inversion recovery images after bypass surgery for moyamoya disease.

OBJECTIVE Transient neurological symptoms are frequently observed during the early postoperative period after direct bypass surgery for moyamoya disease. Abnormal signal changes in the cerebral cortex can be seen in postoperative MR images. The purpose of this study was to reveal the radiological features of the "cortical hyperintensity belt (CHB) sign" in postoperative FLAIR images and to verify its relationship to transient neurological events (TNEs) and regional cerebral blood flow (rCBF). METHODS A total of 141 hemispheres in 107 consecutive patients with moyamoya disease who had undergone

Good clinical outcome after ischemic stroke with successful revascularization is time-dependent.

BACKGROUND: Trials of IV recombinant tissue plasminogen activator (rt-PA) have demonstrated that longer times from ischemic stroke symptom onset to initiation of treatment are associated with progressively lower likelihoods of clinical benefit, and likely no benefit beyond 4.5 hours. How the timing of IV rt-PA initiation relates to timing of restoration of blood flow has been unclear. An understanding of the relationship between timing of angiographic reperfusion and clinical outcome is needed to establish time parameters for intraarterial (IA) therapies. METHODS: The Interventional Managemen

The clinical utility of the Kopitnik arteriovenous malformation microclip during STA-MCA bypass surgery.

PURPOSE: Yasagil temporary clips have been widely used in extracranial-intracranial (EC-IC) arterial bypass surgery. However, the extremely delicate vessels involved often require the application of finer clips. We report on the use of the Kopitnik arteriovenous malformation (AVM) microclip system for superficial temporal artery-middle cerebral artery (STA-MCA) bypass. METHODS: Kopitnik AVM microclips are new mechanical devices that are used during AVM surgery. They exert a pre-defined closing force of 50-70 g, and also feature a special, pyramid-shaped structure stamped on inner surfaces of

Ultrasonographically predicting the extent of collateral flow through superficial temporal artery-to-middle cerebral artery anastomosis.

BACKGROUND AND PURPOSE: This study was performed to elucidate whether the extent of bypass flow through superficial temporal artery-to-middle cerebral artery (STA-MCA) anastomosis could be indirectly estimated by measuring the blood flow velocity in the superficial temporal artery (STA) by using duplex ultrasonography. METHODS: We analyzed 29 patients (31 sides) who underwent STA-MCA bypass surgery for occlusive cerebrovascular disease (28 sides) or unclippable cerebral aneurysm that required therapeutic occlusion of the internal carotid artery (three sides). The flow velocities of the STA we

Rapid resolution of newly formed carotid free-floating thrombus through anticoagulation therapy.

BACKGROUND: Carotid free-floating thrombi (CFFT) are uncommon but pose a high risk of embolic stroke. While vascular intervention is often favored, this case study demonstrates the potential of short-term anticoagulation, guided by thorough imaging, as an alternative treatment strategy. CASE PRESENTATION: We present a patient with an acute ischemic stroke (NIHSS 15) due to middle cerebral artery (MCA) occlusion. Intravenous alteplase successfully revascularized the MCA. However, subsequent imaging revealed a CFFT as the likely source of the occlusion. Rather than immediate surgical interventi

Diabetes, stroke severity and hemodynamic cerebral ischemia.

BACKGROUND: Diabetes is an independent risk factor for ischemic stroke. This study was undertaken to determine whether hemodynamic cerebral ischemia (HCI), which increases stroke severity, is more frequent in diabetic patients. METHODS: Between 01/01/1990 and 30/06/2019, we revascularized 4117 carotid bifurcations in 3745 patients (2687 men and 1058 women; mean age: 71 (range: 28–97) years; 940 (25.1%) diabetics and 2805 (74.9%) nondiabetics). HCI was diagnosed clinically, under regional anesthesia, when a shunt needed to be inserted. All data were included in a prospective database. RESULTS:

Near-Infrared Spectroscopy Might Help Prevent Onset of Cerebral Hyperperfusion Syndrome.

Cerebral hyperperfusion syndrome (CHS) is a rare but fatal perioperative complication after surgical correction of carotid stenosis. Despite numerous treatment options for preventing CHS, it does occur in some patients. We developed the outlet gate technique (OGT), in which the embolic balloon was deflated gradually in accordance with the ratio of oxygen saturation measured by a brain oximeter of the ipsilateral brain region to that in the contralateral region. Between June 2017 and May 2018, 39 patients with carotid stenosis underwent endovascular carotid revascularization procedures; of thes

Is General Anesthesia for Endovascular Thrombectomy Helpful or Harmful?

Endovascular thrombectomy (EVT) has significantly improved outcomes for patients with acute ischemic stroke due to large vessel occlusion. However, despite advances, more than half of patients remain functionally dependent 3 months after their initial stroke. Anesthetic strategy may influence both the technical success of the procedure and overall outcomes. Conventionally, general anesthesia (GA) has been widely used for neuroendovascular procedures, particularly for the distal intracranial circulation, because the complete absence of movement has been considered imperative for procedural succ

Treatment of chronic internal carotid artery occlusion by ipsilateral external carotid endarterectomy.

Chronic complete occlusion of the internal carotid artery (ICA) is a common and important cause of ischemic cerebrovascular disease, which can be treated in many ways: antithrombotic and anticoagulant agents, direct or indirect procedure for revascularization. However, there is still no evidence to prove which method is the most effective. Here we reported a case showed the successful endarterectomy of the external carotid artery (ECA) for a chronic complete occlusion of ICA patient. A 50-year-old male patient presented with progressively aggravated left-sided limb weakness and limb cold feeli

Posterior Cerebral Artery Insufficiency in Pediatric Moyamoya Disease.

The majority of clinical studies on moyamoya disease (MMD) have focused on anterior circulation. The disease involvement of posterior circulation in MMD, mainly in the posterior cerebral artery (PCA), has been mentioned since the early 1980s, and it has been repeatedly emphasized as one of the most important factors related to poor prognosis in MMD. However, its clinical features and outcome have only been elucidated during the last few years. In this review, the angiographic definition of PCA stenosis is summarized. The clinical features are elucidated as being either early-onset or delayed-o

Use of distal embolic protection devices as a bailout strategy for suboptimal flow reversal during transcarotid artery revascularization: illustrative case.

BACKGROUND: Transcarotid artery revascularization (TCAR) has become an increasingly utilized surgical option for carotid disease, relying on flow reversal for distal embolic protection (DEP). However, anatomical variations, such as an isolated middle cerebral artery hemisphere, and severe carotid stenosis may compromise the safety of flow reversal intraoperatively. The authors describe the use of an adjunctive DEP device during TCAR as a "bailout" strategy for high-risk patients. OBSERVATIONS: The authors present the case of an adult patient with high-grade symptomatic carotid stenosis who wa

Carotid Artery Stenting by Direct Puncture of the Aortic Arch Graft during Total Arch Replacement.

OBJECTIVE: We report a unique case of carotid artery stenting (CAS) conducted via direct puncture of an aortic arch graft in a patient who underwent total arch replacement (TAR), complicated by carotid artery dissection. CASE PRESENTATION: A 72-year-old woman underwent TAR for an ascending aortic aneurysm. Aortic dissection extending to the right carotid artery was detected immediately after the aortic arch graft anastomosis, which resulted in severe stenosis and decreased cerebral perfusion. Therefore, urgent revascularization was deemed necessary. The aortic arch graft was directly puncture
