Treatment of open periarticular shoulder fractures sustained in combat-related injuries.

Open periarticular shoulder fractures present a tremendous challenge for orthopedic surgeons. These injuries, albeit rare, are typically caused by high-energy mechanisms and are associated with insult to multiple organ systems resulting in high morbidity and mortality. Although the civilian trauma literature includes several articles on outcomes of closed periarticular shoulder fractures, only 1 peer-reviewed article has focused on this specific open injury pattern. No standard management technique has been adopted for these injuries, and treatment patterns have anecdotally evolved from war to

The characteristics of midfacial fractures and the association with ocular injury: a prospective study.

Ocular injuries commonly occur in patients with facial fractures. This prospective study was set up to determine the incidence of ocular injuries, as assessed by an ophthalmologist, in patients who had sustained midfacial fractures. Over a 2-year period, a study of 363 patients who had sustained midfacial trauma sufficient to lead to a facial bone fracture (438 fractures) was undertaken and patients received a comprehensive examination by an ophthalmologist and an orthoptist within 1 week of injury. The characteristics of the eye injuries sustained were related to the aetiology of the fracture

Effects of Schanz screw location on reduction efficiency in distal femur fractures: A finite element analysis.

OBJECTIVES: The aim of this study was to evaluate the biomechanical efficiency of different Schanz screw positions for indirect reduction of distal femur fractures using finite element analysis. MATERIALS AND METHODS: A three-dimensional finite element model of a comminuted distal femur fracture was constructed, incorporating relevant anatomical structures including ligaments, menisci, and the gastrocnemius muscle. A 30 N posterior force simulated gastrocnemius-induced deformity, followed by a 15 N horizontal traction force applied through Schanz screws inserted at six positions (hole 1 to 6)

Management of civilian ballistic fractures.

INTRODUCTION: The management of ballistic fractures, which are open fractures, has often been studied in wartime and has benefited from the principles of military surgery with debridement and lavage, and the use of external fixation for bone stabilization. HYPOTHESIS: In civilian practice, bone stabilization of these fractures is different and is not performed by external fixation. PATIENTS AND METHODS: Fifteen civilian ballistic fractures, Gustilo II or IIIa, two associated with nerve damage and none with vascular damage, were reviewed. After debridement and lavage, ten internal fixations a

Treatment of multifragmentary fractures of the femur by indirect reduction (biological) and plate fixation.

We present a retrospective review of the results of 43 multifragmentary femur fractures treated under the principle of indirect reduction (biological) and plate fixation. Fractures were caused by high-energy trauma in all patients. Sixteen were subtrochanteric, 14 diaphyseal and 13 supracondylar. There were 13 open fractures. In six of the patients with diaphyseal fractures, a plate was inserted through isolated proximal and distal incisions only, deep to the vastus lateralis. None of the fractures was treated with bone graft. The mean follow-up time was 28.3 months. Union was achieved in 41 p

Principles of external fixation and supplementary techniques in distal radius fractures.

External fixation for fractures of the distal radius has been used for almost 80 years. The main objective is to gain reduction and maintain the reduction throughout the treatment period. Several fixator concepts are available and selection is based on the complexity of the injury to be treated as well as the surgeon's experience. Periarticular application of the fixator with immediate use of the wrist joint is recommended whenever possible. For intra-articular fractures, transarticular application is advisable. External fixtion in complex fractures has to be supplemented by bone grafting, fix

Hybrid external fixation in tibial plafond fractures.

Fourteen patients with Ruedi/Allgower types II and III distal tibia fractures received indirect reduction followed by application of a Monticelli-Spinelli hybrid external fixation system. All 14 patients were available for followup. A stability similar to that of an open reduction and internal Fixation was noted. The mean time to union was 13 weeks. There were no nonunions. One patient experienced malunion in 7 degrees varus. Superficial pin tract infections occurred in 8 pins (3 patients), and all resolved with aggressive pin care and a short course of orally administered antibiotics. Through

External fixation in severe open tibial shaft fractures: indications, orthoplastic strategy, and infection-aware decision-making.

Complex fractures of the femoral shaft rise problems due to high energy trauma with major soft-tissues injuries and bone comminution so increasing operative difficulties, risk of infection and delayed union. Our aim was to appraise the outcome of these problems when using a closed intramedullary locked nailing. There were 52 men and 16 women. Aged 16 to 83 years. 52 patients had multiple-injuries. There were 17 open fractures. The 68 cases were subdivided according to a classification in 3 types. Stable fractures type A and B1 were excluded. The shaft was divided into 5 zones. Patients were in

All-in-one trifocal ipsilateral tibial fractures: A case report on comprehensive surgical fixation of tibial plateau, shaft and bimalleolar fractures.

INTRODUCTION: Trifocal, non-contiguous, ipsilateral tibial fractures involving the plateau, diaphysis and ankle are extremely rare, with limited literature to guide their optimal surgical management. These complex injuries demand a strategic approach to achieve stable reduction and fixation, maintain implant compatibility and enable early rehabilitation. CASE REPORT: We report a case of a 60-year-old male who sustained a high energy trifocal fracture of the left tibia following blunt trauma from a falling gate. Imaging revealed a tibial plateau fracture (AO 41C2.2), a long spiral diaphyseal f

[Bicoronal approach in the management of frontal sinus fractures].

The frontal sinus is located at the level of the junction between the naso-ethmoido-orbital region, the cranial vault and the skull base and plays a particularly important role in the biomechanics of the centro-facial region. It represents a locus minoris resistentiae placed between the fronto-orbital frame, whose resistance to trauma is good, and the thin posterior fronto-ethmoidal structures. Fractures of the frontal sinus more frequently involve the anterior wall of the frontal sinus and, occasionally, also the posterior wall and the base of the sinus. Where the fracture involves the anteri
