Subject-Specific Sparse Dictionary Learning for Atlas-Based Brain MRI Segmentation.

Quantitative measurements from segmentations of human brain magnetic resonance (MR) images provide important biomarkers for normal aging and disease progression. In this paper, we propose a patch-based tissue classification method from MR images that uses a sparse dictionary learning approach and atlas priors. Training data for the method consists of an atlas MR image, prior information maps depicting where different tissues are expected to be located, and a hard segmentation. Unlike most atlas-based classification methods that require deformable registration of the atlas priors to the subject

α-Synuclein in human cerebrospinal fluid is principally derived from neurons of the central nervous system.

The source of Parkinson disease-linked α-synuclein (aSyn) in human cerebrospinal fluid (CSF) remains unknown. We decided to measure the concentration of aSyn and its gradient in human CSF specimens and compared it with serum to explore its origin. We correlated aSyn concentrations in CSF versus serum (Q(aSyn)) to the albumin quotient (Q(albumin)) to evaluate its relation to blood-CSF barrier function. We also compared aSyn with several other CSF constituents of either central or peripheral sources (or both) including albumin, neuron-specific enolase, β-trace protein and total protein content.

Artifact on electromyography produced by an implanted sacral nerve stimulator.

A patient with muscle cramps was referred for nerve conduction studies and electromyography. Her study demonstrated bursts of spontaneous electromyography activity, which waxed and waned in amplitude with a "dive bomber"-like sound. The abnormal finding was found to be an artifact from an implanted sacral nerve stimulator for bladder incontinence.

Interleukin 4 and interleukin 10 levels are elevated in the cerebrospinal fluid of patients with Creutzfeldt-Jakob disease.

BACKGROUND: In neurodegenerative diseases, increasing attention has been focused on inflammatory mediators such as pro-inflammatory and anti-inflammatory cytokines and their potential influence in the process of neurodegeneration. In prion diseases, much data has been gained on the cell culture and animal disease models level, but only limited information is available on humans affected by Creutzfeldt-Jakob disease (CJD). OBJECTIVE: To obtain data on anti-inflammatory cytokines interleukin 4 and interleukin 10 in the cerebrospinal fluid of patients with CJD, patients with other dementia, and

The Top 100 Most Cited Journal Articles on Hydrocephalus.

Hydrocephalus represents a significant burden of disease, with more than 383,000 new cases annually worldwide. When the magnitude of this condition is considered, a centralized archive of pertinent literature is of great clinical value. From a neurosurgical standpoint, hydrocephalus is one of the most frequently treated conditions in the field. The focus of this study was to identify the top 100 journal articles specific to hydrocephalus using bibliometric analysis. Using the Journal of Citation Report database, 10 journals were identified. The Web of Science Core Collection was then searched

Visual environmental rotation: a novel disorder of visiospatial integration.

A 70-year-old man experienced an unusual disorder of visual perception after undergoing a ventriculoperitoneal shunt for normal-pressure hydrocephalus. The disorder was characterized by transient episodes of 90 degrees rotation of the visual environment, rather than the retinotopic visual field. This phenomenon is different from standard visual allesthesia and may have been caused by disordered integration of vestibular and visual inputs to the posterior parietal cortex or perseveration of a pre-existing environmental memory trace.

Third Ventriculocisternostomy for Shunt Failure.

BACKGROUND: Our objective was to analyze the relevance, potential prognostic factors, and complications of endoscopic third ventriculostomy (ETV) in patients with shunt failures. METHODS: Among 721 ETVs performed between 1999 and 2013, we studied 53 patients with shunts (31 men, 21 less than 18 years of age) who had an ETV performed for shunt failures as the result of various causes. We included all initial causes of hydrocephalus except adult chronic (i.e., "normal pressure") and pediatric communicant hydrocephalus. The mean duration between initial shunting for hydrocephalus and the ETV pro

Comparison of ventricular steady-state infusion with bolus infusion and pressure recording for differentiating between arrested and non-arrested hydrocephalus.

In 6 years 26 adult patients with chronic communicating or non-communicating hydrocephalus underwent ventricular fluid pressure (VFP) recording, including intraventricular steady-state and bolus infusion tests. Patients were treated with a shunt when steady-state infusion yielded a csf outflow resistance (Rcsfs) greater than 10 mm Hg/ml/min; the success rate was 83%. The main purpose of the study was to compare Rcsfs with outflow resistance obtained by bolus infusions (Rcsfb), pressure-volume index (PVI) and VFP. Rcsfs was higher than Rcsfb, particularly when resistance was high and the degree

Proteomic differences between focal and diffuse traumatic brain injury in human brain tissue.

The early molecular response to severe traumatic brain injury (TBI) was evaluated using biopsies of structurally normal-appearing cortex, obtained at location for intracranial pressure (ICP) monitoring, from 16 severe TBI patients. Mass spectrometry (MS; label free and stable isotope dimethyl labeling) quantitation proteomics showed a strikingly different molecular pattern in TBI in comparison to cortical biopsies from 11 idiopathic normal pressure hydrocephalus patients. Diffuse TBI showed increased expression of peptides related to neurodegeneration (Tau and Fascin, p < 0.05), reduced expres

Arterial-related craniospinal physiological compliance assessed by phase-contrast MRI and lumbar infusion test.

Compartmental compliance is classically defined as the ratio between volume change and resulting pressure change. Nevertheless, the concept of craniospinal compliance is inherently complex, as it involves different physiological mechanisms that vary according to temporal and volumetric scales. During lumbar infusion tests, the craniospinal system is stressed artificially, whereas during a single cardiac cycle, it operates under dynamic physiological conditions. In this study, we propose a novel approach for estimating arterial-related craniospinal compliance under physiological conditions by c

Kidins220-deficient hydrocephalus mice exhibit altered glial phenotypes and AQP4 differential regulation in the retina and optic nerve, with preserved retinal ganglion cell survival.

Hydrocephalus, characterized by ventriculomegaly due to cerebrospinal fluid accumulation in the cerebral ventricles, is a co-morbidity factor in several neurodevelopmental, psychiatric and neurodegenerative diseases. Aquaporin-4 (AQP4) is crucial for brain water homeostasis, with Aqp4 knockout mice showing sporadic ventriculomegaly and increased brain water content. Kinase D interacting substrate of 220 kDa (Kidins220), a transmembrane protein involved in neuronal survival, synaptic activity and neurogenesis, controls AQP4 levels in ependymocytes and brain astrocytes. Indeed, Kidins220 deficie

Neuroamine related compounds in the CSF of hydrocephalic rabbits.

The effects of neonatal hydrocephalus on the levels of tyrosine, tryptophan, 5-hydroxyindoleacetic acid (5-HIAA), and homovanillic acid (HVA) in CSF were determined by high-performance liquid chromatography (HPLC) with fluorometric detection in normal and chronically hydrocephalic rabbits. The hydrocephalic rabbits showed a highly significant increase in both the serotonin metabolite 5-HIAA and the dopamine metabolite HVA. There were no significant effects of the hydrocephalus on either tyrosine or tryptophan levels. There was a significant positive correlation between the intracranial pressur

The dynamics of brain and cerebrospinal fluid growth in normal versus hydrocephalic mice.

OBJECT: Hydrocephalus has traditionally been quantified by linear measures of ventricular size, with adjunct use of cortical mantle thickness. However, clinical outcome depends on cognitive function, which is more directly related to brain volume than these previous measures. The authors sought to quantify the dynamics of brain and ventricular volume growth in normal compared with hydrocephalic mice. METHODS: Hydrocephalus was induced in 14-day-old C57BL/6 mice by percutaneous injection of kaolin into the cisterna magna. Nine hydrocephalic and 6 normal mice were serially imaged from age 2-12

Optimizing shunt integrity during acute subdural hematoma evacuation.

BACKGROUND: Even mild head trauma can cause severe intracranial hemorrhage in patients with cerebrospinal fluid (CSF) shunts for hydrocephalus. CSF shunts are considered a risk factor for subdural hematoma (SDH). The management of acute SDH (ASDH) in shunted patients with normal pressure hydrocephalus can be challenging. Addressing the hematoma and the draining function of the shunt is important. To preserve the shunt, we set the shunt valve pressure to the highest and perform hematoma evacuation for ASDH. In this study, we report the surgical cases of ASDH in patients with shunts. METHODS: B

Reversible cognitive and gait impairment by a giant quadrigeminal cistern arachnoid cyst: A case report of marked reversal following neuroendoscopic fenestration.

BACKGROUND: Quadrigeminal cistern arachnoid cysts are rare in adults, but can cause reversible gait and cognitive impairment secondary to obstructive hydrocephalus. Differentiating these structural lesions from idiopathic normal-pressure hydrocephalus (iNPH) or neurodegenerative diseases is essential because they are surgically treatable. CASE DESCRIPTION: A 71-year-old woman presented with progressive gait disturbance and mild cognitive impairment. Neuropsychological tests showed a Montreal Cognitive Assessment-Japanese version (MoCA-J) score of 21/30 and a frontal assessment battery (FAB) s

Spinal cord astrocytoma presenting as "idiopathic" intracranial hypertension.

Increased intracranial pressure is rarely seen in association with spinal tumors. We describe a young, non-obese man who presented with increased intracranial pressure, papilledema and visual obscuration. Multiple cerebrospinal fluid (CSF) examinations with normal or minimally elevated CSF protein lead to the initial diagnosis of idiopathic intracranial hypertension. After a lumboperitoneal shunt placement a progressive thoracic myelopathy developed 7 months after onset of symptoms. The spinal MRI showed a low cervical-upper thoracic intramedullary tumor. Open biopsy confirmed a grade 3 fibril

Seeking reversibility and treatability in dementia.

The search for "reversible causes" in the patient presenting with dementia has become a routine part of medical care. This article reviews evidence as to the prevalence of reversibility and concludes that actual reversal of dementia by treatment is rare. In this context, the utility of specific laboratory investigations is discussed, and commonly sought conditions, such as normal-pressure hydrocephalus and B12 deficiency, are reviewed. Overemphasis on reversibility risks neglect of the assistance physicians can provide to patients suffering from irreversible conditions and their families.

Role of endoscopic third ventriculostomy in tuberculous meningitis with hydrocephalus.

Hydrocephalus is one of the commonest complications of tuberculous meningitis (TBM). It can be purely obstructive, purely communicating, or due to combinations of obstruction in addition to defective absorption of cerebrospinal fluid (CSF). Endoscopic third ventriculostomy (ETV) as an alternative to shunt procedures is an established treatment for obstructive hydrocephalus in TBM. ETV in TBM hydrocephalus can be technically very difficult, especially in acute stage of disease due to inflamed, thick, and opaque third ventricle floor. Water jet dissection can be helpful in thick and opaque ventr

Acquired Chiari malformations: incidence, diagnosis, and management.

The acquired descent of the cerebellar tonsils radiographically indistinguishable from Chiari I malformations has been previously reported. The relationship between lumbar shunting procedures for hydrocephalus and symptomatic Chiari malformations has been established. We report the cases of 10 children with lumboperitoneal shunts in whom previous radiographic studies had confirmed a normal hindbrain configuration. Seven of the 10 patients acquired tonsillar descent into the foramen magnum, detected by magnetic resonance imaging, whereas the others remained normal. Four of seven patients were s

Neuropathological aspects of dementias resulting from abnormal blood and cerebrospinal fluid dynamics.

In a large autopsy series of elderly individuals, organic dementia was attributed to (pre-) senile atrophy in 52.8%, to cerebrovascular disease in 22.5%, while 13.6% were of mixed senile and vascular origin, and 1.3% showed communicating hydrocephalus with meningopathies or were of undetermined origin. A survey is given of the morphological criteria of dementia resulting from disorders of cerebral blood supply and CSF circulation. The anatomic basis of vascular dementias are: atherosclerotic encephalopathy with lacunar state or multiple infarcts; granular cortical atrophy resulting from local
