Bipolar II and unipolar comorbidity in 153 outpatients with social phobia.

Previous studies on social phobia (SP) have focused largely on comorbidity between SP and major depression. Less attention has been devoted … depression and 68 (44.4%) had no lifetime history of major mood disorders. Comorbid panic disorder/agoraphobia (PDA), obsessive-compulsive disorder (OCD), and alcohol abuse were reported more frequently in the bipolar group than in the other two subgroups. Unipolar patients showed higher rates of comordid PDA and OCD compared with SP patients without mood di …

The bergen 4-day treatment for panic disorder: a longer-term follow-up.

INTRODUCTION: Bergen 4-Day treatment (B4DT) is a form of concentrated exposure-based cognitive behavioral therapy (CBT) in which patients receive treatment over four consecutive days. Previous studies have shown B4DT to be a promising treatment format for panic disorder (PD), although the long-term stability of treatment gains requires additional study. AIM: The aim of the current study was to evaluate the longer-term effectiveness of B4DT for patients with panic disorder with or without agoraphobia. This study extends a previously published study by providing a long-term follow-up of the sam…

Quality of life impairment in generalized anxiety disorder, social phobia, and panic disorder.

Interest in the assessment of quality of life in the anxiety disorders is growing. The present study examined quality of life impairments in individuals with generalized anxiety disorder (GAD), social phobia, and panic disorder. Results showed that individuals with these disorders reported less satisfaction with their quality of life than non-anxious adults in the community. However, the degree of quality of life impairment is similar across these three disorders. Additionally, comorbid depression, but not anxiety, was found to negatively impact quality of life in these individuals. Finally, d

How prevalent are anxiety disorders in schizophrenia? A meta-analysis and critical review on a significant association.

OBJECTIVE: The presence of anxiety disorders (AD) in schizophrenia (SZ) is attracting increasing interest. However, published studies have yielded very broad variations in prevalence rates across studies. The current meta-analysis sought to (1) investigate the prevalence of co-occurring AD in SZ by reporting pooled prevalence rates and (2) identify potential sources of variations in reported rates that could guide our efforts to identify and treat these co-occurring disorders in patients with SZ. METHODS: We performed a systematic search of studies reporting prevalence of AD in SZ and related

[Psychotherapy and pharmacotherapy of anxiety disorders].

Anxiety disorders are highly prevalent in the general population, their course of illness is often chronic with many relapses, and is associated with a pronounced psychiatric comorbidity and psychosocial disability. Therefore, anxiety disorders are a major challenge to any mental health delivery system. From a perspective of treatment both differential psychopharmacological and psychotherapeutic interventions can be outlined which are well based in empirical terms. In psychiatric care biological and psychological approaches are often combined although the empirical basement of combined treatme

Sixteen-item Anxiety Sensitivity Index: confirmatory factor analytic evidence, internal consistency, and construct validity in a young adult sample from the Netherlands.

The present investigation examined the factor structure, internal consistency, and construct validity of the 16-item Anxiety Sensitivity Index (ASI; Reiss Peterson, Gursky, & McNally 1986) in a young adult sample (n = 420) from the Netherlands. Confirmatory factor analysis was used to comparatively evaluate two-factor, three-factor, and four-factor models of the anxiety sensitivity construct. Support was found for a hierarchical structure of anxiety sensitivity, with one global higher-order factor and four lower-order factors. Internal consistency for the global and lower-order factors of the

Nerve growth factor serum concentrations rise after successful cognitive-behavioural therapy of generalized anxiety disorder.

Generalized anxiety disorder (GAD) is a chronic stress disease with permanent physical tension and cognitive strain. Raised nerve growth factor (NGF) serum levels were reported as an acute stress reaction in soldiers before their first parachute jump even before the rise in cortisol. Taking GAD as a clinical model of chronic stress, we measured NGF in the serum of 22 patients with GAD before and after cognitive-behavioural therapy (CBT) and compared them to those of healthy normal controls. Treatment response was tested by the values of the State and Trait of Anxiety Inventory (STAI) and the H

Emotion recognition deficits in pediatric anxiety disorders: implications for amygdala research.

INTRODUCTION: Anxiety disorders in adults involve aberrant processing of emotional information that is hypothesized to reflect perturbations in the amygdala. This study examines the relationship between face-emotion recognition and anxiety in a sample of children and adolescents participating in a brain-imaging study of amygdala structure and function. METHODS: This study recruited 15 children and adolescents with ongoing anxiety disorders and 11 psychiatrically healthy comparisons group-matched on age, gender, and IQ. Face-emotion recognition was assessed using the Diagnostic Analysis of Non

The efficacy of reboxetine in the treatment-refractory patients with panic disorder: an open label study.

BACKGROUND AND OBJECTIVE: Selective serotonin reuptake inhibitors (SSRIs) are currently the first-line treatment for panic disorder, although up to 30% of patients either do not respond to SSRIs or withdraw due to adverse events. Reboxetine, a selective norepinephrine reuptake inhibitor (selective NRI), is effective in treating depression and may alleviate depression-related anxiety. This study aimed to investigate the efficacy of reboxetine in the treatment of patients with panic disorder who did not respond to SSRIs. METHOD: In this 6-week, open-label study, 29 adult outpatients with panic

Assessing older adults with generalized anxiety: a replication and extension.

Anxiety is a major health problem for older adults. The cornerstone for further work in this area is research that establishes the psychometric utility of standardized measurement strategies to characterize anxiety in older adults. The goals of the current study were to replicate and extend prior research addressing the psychometric properties of five self-report measures of anxiety in a sample of 57 older adults with generalized anxiety disorder (GAD). Data addressed the descriptive characteristics, internal consistency, test-retest reliability, interrelation of subscales, and convergent and

[Is there an interpretational bias for childhood events in anxious adult patients?].

During anamnesis, we attempted often to include the events reported by the patients in our own functional explanation of the disorder. We forget on one hand that the patient reports events intrinsically linked to his pathology, and on the other hand, that an event could not be understood without its context. Concerning the links between the pathology and what is narrated, it is known for a long time that the anxious disorders modify the perception of world by some processes, like the fast detection of the threat in the environment. Questions are: are the anxious encoding bias active to the res

Relaxation-induced EEG alterations in panic disorder patients.

Based on previous reports of relaxation-induced panic attacks in panic disorder patients, quantitative electroencephalographic (EEG) profiles and subjective anxiety ratings were assessed in panic disorder patients and normal controls listening to neutral and relaxation audiotapes. Regardless of tape condition, patients exhibited a greater frequency and severity of panic-related symptoms. Relaxation failed to alter panic-related symptom ratings or anxiety ratings in patients and controls. Theta and alpha increments were observed during relaxation, but only in normal controls. High frequency bet

Epinephrine infusions in panic disorder: a double-blind placebo-controlled study.

Twenty-four subjects with Panic Disorder were infused with epinephrine in physiological doses or placebo according to a double-blind design. The panic rate in the epinephrine group (67%) was higher than in the placebo group (25%). Placebo panic occurred early during the procedure and was apparently associated with anticipation anxiety and stress-provoking situational factors. Panickers were characterized by a greater increase in heart rate as well as a drop in pCO2. Fear of bodily sensations was only weakly associated with state anxiety levels at point of panic. It is concluded that cognitive

Panic disorder and suicide attempt. A reanalysis of data from the Epidemiologic Catchment Area study.

BACKGROUND: Analysing data from the Epidemiologic Catchment Area (ECA) study, Weissman and colleagues reported that panic disorder was strongly associated with suicide attempt. However, they did not control optimally for comorbid disorders known to increase suicide risk. METHOD: Reanalysing the ECA data, we controlled for comorbid disorders in the aggregate rather than one at a time when we estimated the association between panic disorder and suicide attempt. RESULTS: Panic disorder was not associated with an increased risk of suicide attempt. CONCLUSIONS: Comorbid conditions strongly influ

Mood swings in patients with panic disorder.

Depression is frequently found in patients with panic disorder. It is known that patients suffering from panic disorder and depression have more severe symptoms of anxiety and avoidance and respond less well to treatment with antidepressants. The General Behaviour Inventory, an instrument that identifies subjects with subsyndromal and syndromal recurrent mood disorders was completed by 108 patients in a panic disorder program. Thirty-four percent (n = 37) were classified as suffering from a mood disorder. Of the mood disordered patients, 25 of 37 (68%) (23% of the total) were classified as suf

Relationship schema formulation: role-relationship models and intrapsychic conflict.

Recurrent maladaptive interpersonal relationship patterns are a central characteristic of the personality disorders and the main focus in dynamic, cognitive, and family psychotherapies. In dynamic formulations the emphasis is on examining conflict in terms of opposing wishes, fears, and defenses. Such formulations can take place at various levels of information, involving ideas, emotions, self and objects. This paper presents a system for formulating intrapsychic conflicts at the level of person schemas (Horowitz 1988a). It uses units called Role-Relationship Models (RRM).

Management of specific anxiety disorders in the elderly.

Advanced age is not, per se, an indication for a different treatment approach, although certain modifications in the treatment plan are required. Recognition and management of anxiety disorders in the elderly must be prompt to interrupt the vicious cycle of anxiety-depression, physical illness, and other stresses.

Behavior therapy, supportive psychotherapy, imipramine, and phobias.

In a controlled outcome study of phobias, 111 adult patients (69% women, 31% men) received a course of 26 weekly treatment sessions consisting of (1) behavior therapy and imipramine hydrochloride (2) behavior therapy and placebo, or (3) supportive psychotherapy and imipramine. Patients were classified as agoraphobic, mixed phobic, or simple phobic. The great majority of patients in all groups showed moderate to marked global improvement (70% to 86%, depending on rater). In agoraphobics and mixed phobics (both groups experiencing spontaneous panic attacks), imipramine was significantly superior
