The role of brain MRI in autoimmune encephalitis.

PURPOSE OF REVIEW: Brain MRI is central to the diagnostic work-up of autoimmune encephalitis (AE). This review summarizes established patterns on routine MRI sequences across antibody-defined AE types and highlights emerging quantitative and network-based approaches aimed at improving sensitivity, phenotypic stratification, and prognostication. RECENT FINDINGS: Conventional MRI reveals syndrome-associated patterns that can support diagnosis in several AE types, including medial temporal involvement in LGI1, CASPR2, and GABA(B) receptor encephalitis; multifocal cortico-subcortical lesions in G

Rocky Mountain Spotted Fever Presenting as an Ischemic Stroke: A Rare Manifestation.

Rocky Mountain spotted fever (RMSF) is among the most severe tick-borne rickettsial infections encountered in the United States. Its presentation ranges from the classic triad of fever, headache, and rash to central nervous system (CNS) conditions, including meningitis, encephalitis, and seizures. Notably, RMSF presenting as an ischemic stroke due to CNS vasculitis is rare, which can prove fatal if recognition is delayed. We present the case of a woman in her 50s with a history of well-controlled diabetes mellitus type 2 and hypertension, who presented with fever, chills, body aches, and rash.

Clinical Challenges in Managing Shoulder Osteoarthritis and Suprascapular Neuropathy.

Chronic shoulder pain is a common and multifactorial condition. Suprascapular nerve neuropathy, although relatively uncommon, may lead to pain, weakness, and muscle atrophy as a result of extrinsic compression, trauma, or inflammation. When it occurs in the presence of glenohumeral osteoarthritis, overall shoulder function may be further compromised, complicating both diagnostic assessment and therapeutic decision-making. We report the case of a 56-year-old man with a three-year history of chronic left shoulder pain and progressive proximal weakness, despite multiple courses of physiotherapy.

Diabetic ketoacidosis and cerebral edema: a rare case of infarct-like MRI findings in a pediatric patient.

INTRODUCTION AND IMPORTANCE: Diabetic ketoacidosis (DKA) is a serious and potentially life-threatening complication of type 1 diabetes mellitus, particularly in pediatric patients. Cerebral edema (CE) is a rare but devastating consequence of DKA. Although neuroimaging findings in CE are typically nonspecific, infarct-like changes are exceedingly rare and may complicate clinical management. CASE PRESENTATION: We report the case of an 11-year-old boy with a history of type 1 diabetes mellitus who presented with epigastric pain, vomiting, polydipsia, and polyuria. Despite timely initiation of st

Histopathological evidence that diethylene glycol produces kidney and nervous system damage in rats.

Diethylene glycol (DEG) is an organic compound that has been found as an adulterant in consumer products as a counterfeit glycerin. Diethylene glycol is metabolized to two primary metabolites: 2-hydroxyethoxyacetic acid (2-HEAA) and diglycolic acid (DGA), the latter shown to accumulate in the kidney and cause dose-dependent cell necrosis. DEG poisonings are characterized predominately by acute kidney injury (AKI) but have also produced delayed neurological sequelae such as sensorimotor neuropathy. To better understand these effects, Wistar-Han rats were orally administered a water control or d

Role of the SphK-S1P-S1PRs pathway in invasion of the nervous system by SARS-CoV-2 infection.

Global spread of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is still ongoing. Before an effective vaccine is available, the development of potential treatments for resultant coronavirus disease 2019 (COVID-19) is crucial. One of the disease hallmarks is hyper-inflammatory responses, which usually leads to a severe lung disease. Patients with COVID-19 also frequently suffer from neurological symptoms such as acute diffuse encephalomyelitis, brain injury and psychiatric complications. The metabolic pathway of sphingosine-1-phosphate (S1P) is a dynamic regulator of various c

Intranasal delivery of interleukin-4 attenuates chronic cognitive deficits via beneficial microglial responses in experimental traumatic brain injury.

Traumatic brain injury (TBI) is commonly followed by long-term cognitive deficits that severely impact the quality of life in survivors. Recent studies suggest that microglial/macrophage (Mi/MΦ) polarization could have multidimensional impacts on post-TBI neurological outcomes. Here, we report that repetitive intranasal delivery of interleukin-4 (IL-4) nanoparticles for 4 weeks after controlled cortical impact improved hippocampus-dependent spatial and non-spatial cognitive functions in adult C57BL6 mice, as assessed by a battery of neurobehavioral tests for up to 5 weeks after TBI. The Inferior Colliculus in Alcoholism and Beyond.

Post-mortem neuropathological and in vivo neuroimaging methods have demonstrated the vulnerability of the inferior colliculus to the sequelae of thiamine deficiency as occurs in Wernicke-Korsakoff Syndrome (WKS). A rich literature in animal models ranging from mice to monkeys-including our neuroimaging studies in rats-has shown involvement of the inferior colliculi in the neural response to thiamine depletion, frequently accomplished with pyrithiamine, an inhibitor of thiamine metabolism. In uncomplicated alcoholism (i.e., absent diagnosable neurological concomitants), the literature citing in

CT and MR in non-neonatal hypoxic-ischemic encephalopathy: radiological findings with pathophysiological correlations.

Non-neonatal hypoxic-ischemic encephalopathy is a clinical condition often related to cardiopulmonary arrest that demands critical management and treatment decisions. Management depends mainly on the degree of neurological impairment and prognostic considerations. Computed tomography (CT) is often used to exclude associated or mimicking pathology. If any, only nonspecific signs such as cerebral edema, sulci effacement, and decreased gray matter (GM)/white matter (WM) differentiation are evident. Pseudosubarachnoid hemorrhage, a GM/WM attenuation ratio <1.18, and inverted GM attenuation are ass

Correlation of endothelial nitric oxide synthase and vascular endothelial growth factor expression with malignancy in patients with astrocytic tumors.

PURPOSE: Many characteristics of malignant brain tumors (increased vascular permeability, vasodilatation, neovascularisation and free radical injury to the tumor and adjacent normal tissues) are believed to be mediated by nitric oxide (NO) synthetized by endothelial NO synthase (eNOS). Overexpression of vascular endothelial growth factor (VEGF) is associated with several central nervous system (CNS) diseases and tumors. Our aim was to study immunohistochemically the coexpression of eNOS and VEGF in astrocytic tumors and to analyse their possible correlation with tumor grade, angiogenesis and p

White Matter Hyperintensities of Presumed Vascular Origin: Current Understanding of Development and Progression.

BACKGROUND AND PURPOSE: White matter hyperintensities (WMH) of presumed vascular origin are among the most common MRI manifestations of cerebral small vessel disease and are a major contributor to vascular contributions to cognitive impairment and dementia. Although WMH have traditionally been attributed to chronic ischemia resulting from small vessel pathology, the biological mechanisms underlying their development and, more importantly, their progression remain incompletely understood. Recent advances in longitudinal neuroimaging, quantitative MRI, pathology, genetics, and physiological imag

Total Magnetic Resonance Imaging Burden of Small Vessel Disease in Cerebral Amyloid Angiopathy: An Imaging-Pathologic Study of Concept Validation.

IMPORTANCE: Cerebral amyloid angiopathy (CAA) is characteristically associated with magnetic resonance imaging (MRI) biomarkers of small vessel brain injury, including strictly lobar cerebral microbleeds, cortical superficial siderosis, centrum semiovale perivascular spaces, and white matter hyperintensities. Although these neuroimaging markers reflect distinct pathophysiologic aspects in CAA, no studies to date have combined these structural imaging features to gauge total brain small vessel disease burden in CAA. OBJECTIVES: To investigate whether a composite score can be developed to captu

The state of head injury biomechanics: past, present, and future: part 1.

This article is the first of two parts of a comprehensive survey of the biomechanics of head injury since its inception in 1939 in the United States, the separation being made for temporal and spatial reasons. The second portion of this material will be published at a later time in this journal. The discussion will be almost exclusively limited to nonpenetrating events. The topics presented in the following sections include an introduction that discusses the magnitude of the problem, the basic tools of biomechanics, and significant major reference sources covering this subject. This is succeed
