A case of a common bile duct stone that formed around a fish bone as a nidus after distal gastrectomy with Roux-en-Y reconstruction.

BACKGROUND: The presence of a foreign body in the common bile duct (CBD) is a rare phenomenon. Thus, the route and mechanism of its migration remain difficult to fully clarify, especially for cases that occur after gastrectomy with Roux-en-Y reconstruction. Herein, we present a case of a CBD stone that formed around a fish bone as a nidus subsequent to distal gastrectomy with Roux-en-Y reconstruction. CASE PRESENTATION: A 70-year-old man was admitted to our hospital due to repeated episodes of epigastralgia. He had undergone distal gastrectomy with Roux-en-Y reconstruction for gastric cancer

Nonsurgical Management of a Caudal Mediastinal Granuloma.

A 3 yr old female springer spaniel presented with a 3-day history of pyrexia, lethargy, and a mild cough. A year prior, the dog had undergone surgical removal of a grass seed from her sublumbar muscles. Computed tomography of the thorax revealed the presence of a caudal mediastinal mass closely associated with the esophagus and diaphragm. A median sternotomy was performed, but the mass proved nonresectable. Biopsy results confirmed a chronic granuloma, and the culture isolated Escherichia coli spp. The patient was treated with long-term antibiotics and the response was followed with computed t

A multifunctional scaffold that promotes the scaffold-tissue interface integration and rescues the ROS microenvironment for repair of annulus fibrosus defects.

Due to the limited self-repair ability of the annulus fibrosus (AF), current tissue engineering strategies tend to use structurally biomimetic scaffolds for AF defect repair. However, the poor integration between implanted scaffolds and tissue severely affects their therapeutic effects. To solve this issue, we prepared a multifunctional scaffold containing loaded lysyl oxidase (LOX) plasmid DNA exosomes and manganese dioxide nanoparticles (MnO2 NPs). LOX facilitates extracellular matrix (ECM) cross-linking, while MnO2 NPs inhibit excessive reactive oxygen species (ROS)-induced ECM degradation

Mesh Migration and Bowel Perforation as a Late Complication of Transabdominal Preperitoneal Laparoscopic Hernia Repair.

Minimally invasive surgery is increasingly used in the treatment of inguinal hernias, with two main techniques described: transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP). In both techniques, a prosthetic mesh is placed in a preperitoneal position. However, in TAPP, the peritoneum flap must be completely closed. The TAPP technique is associated with more intra-abdominal complications. This article describes a case of bowel occlusion due to migration and erosion of a mesh after a TAPP repair in a 57-year-old patient with a history of colonic diverticular disease. The patien

Catastrophic vertebral artery and subclavian artery pseudoaneurysms caused by a fishbone: A case report.

BACKGROUND: Fishbone is the most common esophageal foreign body and tends to migrate after piercing the esophagus to nearby structures. Vascular injury around the esophagus is a serious complication and has a high mortality rate, especially in the case of multiple vascular injuries. CASE SUMMARY: We report an extremely rare case of successive vertebral artery and subclavian artery pseudoaneurysms caused by swallowing a fishbone in a previously healthy 29-year-old female. She was transferred to the emergency department of our hospital because of hemorrhagic shock due to a vertebral artery pseu

Embolization of a fractured central venous catheter placed using the internal jugular approach.

INTRODUCTION: Fracture and embolization of central venous catheters placed via the subclavian approach is well recognized, but fractured catheters placed via the internal jugular vein are extremely rare. PRESENTATION OF CASE: A 65-year-old man presented with a catheter embolus after placement of a central venous port using the internal jugular approach undertaken to administer adjuvant chemotherapy for colon cancer with lung metastases. Goose neck and conformational loop snares were successfully used to percutaneously retrieve the severed catheter, which had migrated to the right ventricle.

Endometrial morphological changes in IUD users: a review.

The introduction of an intrauterine device into the uterine cavity induces a foreign body reaction in the surrounding endometrium which is characterized by the infiltration of polymorphonuclear leucocytes and macrophages into the endometrial stroma and subsequently through the surface epithelium. Leucocyte migration is greater with copper IUDs than with inert IUDs. Ulceration of the surface epithelium, haemorrhage of erythrocytes and microthrombosis of stomach capillaries occur in the functional endometrium in contact with inert and copper IUDs. In endometrium adjacent to, but not in contact w

Minimally Invasive Approach after Lumbar Abscess Secondary to Retained Gallstones in Abdominal Cavity after Laparoscopic Cholecystectomy.

INTRODUCTION: Laparoscopic cholecystectomy is widely performed and safe, but may present rare complications such as retained gallstones ("dropped gallstones"), which can act as a nidus for infection and cause late intra-abdominal or retroperitoneal abscesses. METHODS: A 40-year-old male with prior laparoscopic cholecystectomy presented with fever and right lumbar pain. Examination showed an inflammatory lumbar mass with leukocytosis and elevated C-reactive protein. Imaging revealed a right paravertebral collection with a fistulous tract to the abdominal cavity. Despite antibiotics and percuta

Endovascular management of a fractured dialysis catheter: a case report and review of retrieval techniques.

BACKGROUND: With the advancements in endovascular techniques, reports of rare vascular complications have increased. This case describes the accidental fracture of a non-cuffed hemodialysis catheter and its subsequent endovascular management, offering valuable insights for the treatment of similar intravascular foreign bodies (IFBs). CASE REPORT: An 83-year-old male with a non-cuffed catheter presented after a catheter fracture. Radiographic imaging confirmed migration of the fragment to the right atrium. After a multispecialistic collaboration assessment, endovascular retrieval was performed

[Supportive drugs for improved implant healing].

BACKGROUND: Improvement of implant healing in orthopedic and trauma surgery serves to improve the life expectancy of the implant. Good primary stability by clamping is a prerequisite for secondary stability and for the actual integration and healing of the implant. RESULTS: Possible causes of implant loosening are abrasive particles, which arrive at non-integrated implants at the unsealed prosthesis-bone interface and provoke a macrophage-mediated foreign body reaction, resulting in periprosthetic osteolysis. Numerous animal studies have already described the use of bisphosphonates to inhibit

Ultrasonographic features of grass awns in the urinary bladder.

Grass awns are a common cause of foreign body disease in animals, but little is known about their presence in the lower urinary tract. The ultrasonographic features of grass awns in vivo and in vitro have been described in detail. The purpose of this report is to describe the clinical and sonographic features of grass awns in the urinary bladder of dogs and cats. Three male Yorkshire terriers (one of which was examined twice) and one female domestic short-haired cat were evaluated for signs of lower urinary tract disease, and an intravesicular grass awn was suspected based on ultrasound examin

From Childhood Ingestion to Adult Obstruction: A Long-Retained Synthetic Gastric Foreign Body.

Foreign body ingestion occurs predominantly in children, and most objects pass spontaneously; only a minority require endoscopic or surgical intervention. Prolonged gastric retention of synthetic material with presentation in adulthood is exceptionally uncommon. A 26-year-old man, who recalled accidental ingestion of expanding construction foam at four to five years of age, was first evaluated by gastroenterology on initial presentation for long-standing epigastric pain, nocturnal discomfort, heartburn, acid regurgitation, reduced appetite, and intermittent vomiting. He had no significant rece

Tibial implant migration of cemented and cementless total and unicompartmental knee arthroplasties in patients with low and normal bone mineral density: a 5-year RSA cohort study of 397 cases.

BACKGROUND AND PURPOSE: Periprosthetic bone may influence the longevity of joint implants. We aimed to investigate the association between bone mineral density (BMD) and tibial implant migration of cemented and cementless knee arthroplasty as a surrogate marker of aseptic loosening. METHODS: In a prospective cohort study, patients were operated on between 2014 and 2018 with a unicompartmental knee arthroplasty (UKA) or a total knee arthroplasty (TKA). Preoperative BMD was measured using dual-energy X-ray absorptiometry of the lumbar spine and hips. Patients were categorized into low or normal

Minimally invasive abdominal catheter placement in ventriculoperitoneal shunt patients is associated with lower abdominal catheter complications: a single center experience.

PURPOSE: Ventriculoperitoneal (VP) shunt placement represents one of the modalities for hydrocephalus (HC) requiring cerebrospinal fluid (CSF) flow diversion and is considered a procedure with a high complication rate. Reducing VP-shunt procedure time and optimizing abdominal catheter placement could help surgical VP-shunt complication rates. METHODS: A 7-year, single-center retrospective analysis was performed on adult patients who underwent minimally invasive (MI) trocar-guided direct peritoneal puncture (through the outer and inner fascia) for abdominal catheter insertion during VP-shunt im

A Novel Technique for Retrieval of Dislodged Coronary Stent from Renal Artery: A Case Report.

Coronary artery stent (CAS) dislodgment is a rare complication in contemporary percutaneous coronary intervention (PCI), with migration to the renal artery being exceedingly uncommon. Herein, we present a case report of a 78-year-old female patient who was transferred to a PCI hub center after an unsuccessful PCI of the left anterior descending artery at a spoke center, leading to CAS migration to the left coronary cusp of the aortic valve. During retrieval, the stent further dislodged into the left inferior segmental renal artery. Multiple standard retrieval techniques failed, but the SpiderF

Rectal Perforation due to Interstim® Sacral Neuromodulation Device Lead Migration.

BACKGROUND: Sacral neuromodulation (SNM) implantable devices such as the InterStim® device by Medtronic carry a small risk of stimulatory lead migration. In very rare cases migrated leads can injure the rectal wall resulting in a full-thickness perforation. The sequela of rectal perforation by SNM leads may have varied clinical presentation, and can remain undiagnosed without appropriate suspicion, increasing the risk for further complications. CASE REPORT: A 43-year-old woman with a history of multiple SNM device revisions presented to our emergency department (ED) with rectal pain and gastr

Pulmonary Arteriovenous Malformations Requiring Surgical Resection Due to the Migration of Two Coils into a Single Lesion with Mycobacterium avium Complex Infection after Coil Embolization.

A 21-year-old male with hereditary hemorrhagic telangiectasia underwent coil embolization for three pulmonary arteriovenous malformations (right S6c, right S10a, and left S6b). Three years after coil embolization, coil migration was observed in the right S6c and S10a, and a cavitary lesion developed between the two coils. Four years after embolization, the two coil lesions were found to have merged within the cavity. Right lower lobectomy was performed for severe recurrent hemoptysis. A histological examination revealed a granuloma with caseous necrosis related to Mycobacterium avium complex (
