Arthroplasty with a metal radial head for unreconstructible fractures of the radial head.

BACKGROUND: Treatment of unreconstructible comminuted fractures of the radial head remains controversial. There is limited information on the outcome of management of these injuries with arthroplasty with a metal radial head implant. METHODS: The functional outcomes of arthroplasties with a metal radial head implant for the treatment of twenty-five displaced, unreconstructible fractures of the radial head in twenty-four consecutive patients (mean age, fifty-four years) were evaluated at a mean of thirty-nine months (minimum, two years). There were ten Mason type-III and fifteen Mason-Johnston

A study of function after tibial cast bracing.

103 patients with fresh unilateral fractures of the tibial shaft were reviewed clinically and radiologically to assess the results of treatment by functional bracing. At review, 84 had returned to their previous employment and all but 22 could run. All had full knee movements, 85 per cent had a normal range or a mild loss of subtalar movement, and only 11 patients had greater than 25 per cent reduction of measured ankle movement. The average shortening measured radiologically was 0.5 cm. Angulation greater than 10 degrees was present in 1 patient. Factors leading to delayed union include commi

Spontaneous correction of angulational deformity of the proximal femoral epiphysis after cervical and trochanteric fracture.

This case report of a 5-year-old girl describes a severely comminuted fracture of the cervical and trochanteric areas of the left proximal femur which required open reduction and internal fixation for stable realignment. At 7 months follow-up, angulational deformity was recognized due to partial growth arrest leading to valgus tilt of the proximal femoral epiphysis. After persisting 1 year, gradual spontaneous improvement occurred leading to total realignment with a normally wide growth plate at 36 months following injury. It is hypothesized that the valgus deviation of the femoral head, by ca

Antibiotic prophylaxis in oral and maxillofacial surgery: a systematic review.

Surgical site infections are a complication of oral and maxillofacial procedures, with the potential for significant morbidity and mortality. Use of preoperative, perioperative, and postoperative antibiotic prophylaxis to reduce the incidence of surgical site infections must be balanced with considerations of a patients' risk of antibiotic-related adverse events. This review aimed to provide evidence-based recommendations for antibiotic prophylaxis. Searches were conducted using MEDLINE, the Cochrane Library, EMBASE, and PUBMED for maxillofacial procedures including: treatment of dental absces

Hinged elbow fixation for recurrent instability following fracture dislocation.

The purpose of this study was to evaluate the use of an articulated external fixator of the elbow in the management of instability after fracture dislocation. We retrospectively reviewed results of eight patients treated with an articulated external fixator between 1998 and 1999. Study inclusion criteria included recurrent/chronic dislocation following fracture dislocation. Patients were divided into two groups based on the onset of instability. Group I (n=3) included patients with acute instability. The indication for use of the articulated external fixator in this subset of patients was the

Treatment of type C pilon fractures by external fixator combined with limited open reduction and absorbable internal fixation.

INTRODUCTION: This study was conducted to evaluate the clinical outcome of the treatment of type C pilon fractures by using an external fixator combined with limited open reduction and absorbable internal fixation. PATIENTS AND METHODS: Twenty-five type C pilon fractures, including 4 open fractures, were included in this retrospective study. The procedure of first-stage manipulation and external fixation spanning the ankle joint was conducted as early as possible. For the second stage, the tibial and fibular fractures were reduced and fixed with absorbable screws or rods through a limited inc

Characteristics and prediction of cranial crush injuries in children.

This study documents four clinical cases of fatal crush injuries to children between 1.5 and 6 years of age with correlations between modeled stress and clinically observed fracture patterns. The clinical case fractures were concentrated in the basicranium, bridged the impact sites, and traversed the middle cranial fossa in the area of the spheno-occipital synchondrosis. The crushing forces from these cases were recreated on a simplified finite element model of a cranium by applying bilateral pressures to corresponding regions. Numerous trials were run to develop a representative pattern of pr

Devastating massive knee defect reconstruction using the cornucopian chimera flap from the subscapular axis: two case reports.

Two cases of successful reconstruction of massive defects around the knee with multiple-island combined flaps based on the subscapular axis are reported. Both defects resulted from aggressive debridement for acute osteomyelitis after open fixation of high-energy fractures around the knee. In Case 1, a four-island combined flap consisting of the scapular flap, the lattissimus dorsi muscle flap, the serratus anterior muscle flap, and the scapular osseous flap, based on the same subscapular axis, was used. Partial bone transport using the Ilizarov apparatus was added as reinforcement of arthrodes

Creating high-resolution 3D cranial implant geometry using deep learning techniques.

Creating a personalized implant for cranioplasty can be costly and aesthetically challenging, particularly for comminuted fractures that affect a wide area. Despite significant advances in deep learning techniques for 2D image completion, generating a 3D shape inpainting remains challenging due to the higher dimensionality and computational demands for 3D skull models. Here, we present a practical deep-learning approach to generate implant geometry from defective 3D skull models created from CT scans. Our proposed 3D reconstruction system comprises two neural networks that produce high-quality

Biomechanical design prognosis of two extramedullary fixation devices for subtrochanteric femur fracture: a finite element study.

The design rationale of extramedullary fixation for femur fracture has remained a matter of debate in the orthopaedic community. The present work provides a comparative preclinical assessment between two standard fracture fixation techniques: dynamic hip screw (DHS) and proximal femoral locking plate (PFLP), by employing finite element (FE)-based in silico models. The study attempts to evaluate and compare the two implants on following biomechanical behaviours: (1) stress variation on the femur and implant, (2) axial displacement of the fixated femur constructs, (3) postoperative stress shield
