Duodenal large-cell neuroendocrine carcinoma as unusual cause of acute pancreatitis.

Early identification of acute pancreatitis etiology is essential for choosing the best therapeutic management. The main causes are cholelithiasis and alcohol consumption. Tumors that obstruct the main pancreatic duct are uncommon causes of acute pancreatitis. Duodenal neuroendocrine tumors are rare entities and may be exceptional causes of acute pancreatitis. A 57-year-old male, with associated severe cardiovascular pathology, was admitted with clinical and biological picture of acute pancreatitis. Biliary and alcoholic causes were excluded. Abdominal contrast-enhanced computed tomography scan

Improving rehabilitation and quality of life after percutaneous transhepatic cholangiography drainage with a rapid rehabilitation model.

BACKGROUND: Percutaneous transhepatic cholangiography drainage (PTCD) effectively treats biliary obstruction. However, patients must maintain the drainage tube after hospital discharge, which may interfere with daily life and work, potentially causing psychological distress. Postoperative rehabilitation is crucial, and strengthened nursing interventions can shorten recovery time. AIM: The aim was to evaluate an inpatient model to shorten rehabilitation duration and improve quality of life after PTCD. METHODS: A total of 118 patients with malignant obstructive jaundice who were admitted to ou

Acute cholangitis: Diagnosis and management.

Acute cholangitis is an infection of the bile and biliary tract which in most cases is the consequence of biliary tract obstruction. The two main causes are choledocholithiasis and neoplasia. Clinical diagnosis relies on Charcot's triad (pain, fever, jaundice) but the insufficient sensitivity of the latter led to the introduction in 2007 of a new score validated by the Tokyo Guidelines, which includes biological and radiological data. In case of clinical suspicion, abdominal ultrasound quickly explores the biliary tract, but its diagnostic capacities are poor, especially in case of non-gallsto

Predictors of curative resection and long term survival of gallbladder cancer - A retrospective analysis.

BACKGROUND: Gallbladder cancer (GBC) is an aggressive malignancy. We analysed factors predicting resectability and survival of patients with GBC and the impact of surgical obstructive jaundice (SOJ). METHODS: Four hundred and thirty-seven patients with suspected GBC were analysed (52 excluded: benign pathology n = 35, missed GBC n = 17). The remaining 385 patients were divided into non-SOJ (n = 234) and SOJ (n = 151) groups. Predictors of resectability and long term survival were analysed and compared with their subgroups. RESULTS: Patients with gastric outlet obstruction, abdominal lump, we

A novel biliary stent coated with silver nanoparticles prolongs the unobstructed period and survival via anti-bacterial activity.

Symptomatic biliary stricture causes life-threatening complications, such as jaundice, recurrent cholangitis and secondary biliary cirrhosis. Fully covered self-expanding metal stents (FCSEMSs) are gaining acceptance for treatments of benign biliary stricture and palliative management of malignant biliary obstructions. However, the high rate of FCSEMS obstruction limits their clinic use. In this study, we developed a novel biliary stent coated with silver nanoparticles (AgNPs) and investigated its efficacy both in vitro and in vivo. We first identified properties of the AgNP complex using ultr

Biliary strictures: diagnostic considerations and approach.

Biliary strictures present a diagnostic challenge, especially when no etiology can be ascertained after laboratory evaluation, abdominal imaging and endoscopic retrograde cholangiopancreatography (ERCP) sampling. These strictures were traditionally classified as indeterminate strictures, although with advances in endoscopic techniques and better understanding of hepato-biliary pathology, more are being correctly diagnosed. The implications of missing a malignancy in patients with biliary strictures-and hence delaying surgery-are grave but a significant number of patients (up to 20%) undergoing

The choice of surgical timing for biliary duct reconstruction after obstructive bile duct injury: an experimental study.

BACKGROUND/AIMS: The surgical time of obstructive bile duct injury when biliary-enteric reconstruction is needed remains controversial till now. METHODOLOGY: Sixty dogs models of bile duct injury were established and divided into six groups according to the days (0, 5, 10, 15, 20, 30 days) of injury. At the time of surgery, biopsy was taken and Roux-en-Y hepaticojejunostomy was performed. The morphology and local histopathology of bile duct, the liver function in different periods were observed. Surgical complications and survival were also evaluated. RESULTS: In the early time (5-10 days),

Biliobronchial fistula after liver surgery for giant hydatid cyst.

Background. Biliobronchial fistula (BBF) is a rare complication in the natural history of liver hydatid disease by Echinococcus granulosus. We present a case of BBF after resection of a giant liver hydatid cyst in a 72-year-old woman. Case Report. A total cystpericystectomy was done, leaving the left lateral section of the liver that was fixed to the diaphragm. Postoperatively, the patient developed obstructive jaundice. An ERCP showed an obstruction at the junction of the left biliary duct and the main biliary duct and contrast leak. At reoperation, the main duct was ischemic, likely due to t

Neutrophils in biliary atresia. A study on their morphologic distribution and expression of CAP37.

Biliary atresia is a rare pediatric disorder that results in obstructive jaundice in early infancy. Liver biopsies are characterized histologically by cholestasis, portal inflammatory infiltrate, and marked bile ductular proliferation. We studied the distribution of neutrophils in biliary atresia in patients who have undergone the Kasai procedure. Thirteen different liver biopsies were accompanied by hilar section in eight cases. Control liver specimens were from two patients with choledochal cysts, three patients with neonatal hepatitis, and three patients with alpha-1 antitrypsin deficiency.

[Oncologic emergencies associated with pancreatobiliary cancer].

Clinically common oncologic emergencies associated with pancreatobiliary cancer are gastrointestinal bleeding caused by duodenal invasion of pancreatic carcinoma, severe duodenal obstruction due to pancreatic carcinoma, and acute cholangitis accompanied by obstructive jaundice in patients with biliary tract carcinoma. When a patient with gallbladder cancer presents with acute cholecysitis, emergency surgery is sometimes performed on the basis of the latter diagnosis. Emergency procedures can also be required in the perioperative management of pancreatobiliary cancer, for example, in biliary pe

Magnetic resonance cholangiopancreatography: evaluation in 150 patients.

BACKGROUND AND AIMS: Magnetic resonance cholangiopancreatography (MRCP) is a non-invasive imaging technique for evaluating the biliary and pancreatic ducts. MRCP has reached a level of resolution and reliability where it may replace diagnostic endoscopic retrograde cholangio-pancreatography (ERCP). We analyzed the results of MRCP in adult patients with biliary or pancreatic disease, and compared the findings with those at surgery or on ERCP. METHODS: Data of 150 patients who underwent MRCP with both single slab and multislice rapid acquisition with relaxation enhancement sequences with half-f

Helicobacter species are not detectable by 16S rDNA PCR in bile from Dutch patients with common bile duct stones.

BACKGROUND: Some Helicobacter species colonize the intestinal tract. To explore the possible relation between Helicobacter spp. and gallbladder disorders, we have investigated their presence in bile of patients with biliary obstruction and dilatation of the bile ducts. METHODS: Bile was sampled from 31 Dutch patients with biliary obstruction identified by jaundice and dilatation of the bile ducts on ultrasound. Samples (n = 31) were obtained immediately following cannulation of the common bile duct (CBD) by endoscopic retrograde cholangiopancreatography (ERCP) (n = 29) or by peri-operative pu

Surgical bypass versus metallic stent for unresectable pancreatic cancer.

With the development of interventional radiology and endoscopy, the practice of inserting expandable metallic stents for malignant jaundice has become widespread. Many studies have compared surgical bypass with polyethylene stents, or metallic stents with polyethylene stents. However, few data are available on the comparison of surgical bypass and metallic stents. The aim of this study was to compare the patient's postprocedure course and the cost performance of surgical bypass and metallic stents in patients with unresectable pancreatic cancer. The parameters analyzed were the rates of proced

[Mirizzi syndrome--a contraindication for laparoscopic surgery].

The Mirizzi syndrome refers to a benign obstruction of the common hepatic duct by a stone impacted within the cystic duct or the neck of the gallbladder causing an obstructive jaundice. According to McSherry's subclassification based on endoscopic retrograde cholangio-pancreatography we distinguish a type I, involving an external compression of the common hepatic duct by a large stone impacted in the cystic duct or Hartmann's pouch without lesion of the gallbladder or the common hepatic duct wall. In type II a cholecysto-choledochal fistula is present, caused by a calculous which has already e

Intrabiliary rupture of hydatid cyst of the liver.

Infestation with Echinococcus granulosus is common in Iraq, where a close relationship exists between dogs, the carnivorous definitive hosts, and sheep, the herbivorous hosts of the parasite. Plants contaminated by eggs of the tapeworm passed in the dog's faeces may be ingested by man, giving rise to hydatid disease. Of 136 cases of hydatid disease affecting various tissues and organs studied and treated during a 3-year period, the liver was involved in 94; intrabiliary rupture occurred in 15. Pain, hectic fever, and obstructive jaundice were invariable in these 15 cases but biliary obstructio

Impact of Anatomical Variations of Right Hepatic Artery on Biliary Obstruction and Liver Fibrosis in Pediatric Congenital Biliary Dilatation Patients: A Retrospective Analysis of 1464 Cases.

BACKGROUND: The role of the ventrally positioned right hepatic artery (RHA) in congenital biliary dilatation (CBD) remains unclear. METHODS: We retrospectively analyzed 1464 pediatric CBD patients undergoing surgery from 2015 to 2023, comparing those with ventrally positioned versus normal RHA. RESULTS: Ventrally positioned RHA was identified in 10.31% of patients and was associated with more frequent symptoms (e.g., abdominal pain, vomiting, jaundice), elevated liver enzymes, and a higher incidence of liver fibrosis (23.40% vs. 14.85%, p = 0.008). It was an independent risk factor for fibro

Pancreatitis Manifestation and Mimicking Gallstone Symptoms Caused by Fasciola Hepatica: A Case Report.

Fasciola hepatica is a parasitic trematode that infects humans, typically through the consumption of contaminated water or raw vegetables. The infection progresses through acute and chronic phases, with the latter often presenting as jaundice, cholangitis, pancreatitis, and cholecystitis due to bile duct obstruction. We report the case of a 53-year-old Iranian woman who experienced persistent eosinophilia, nausea, right upper quadrant and epigastric pain, and occasional chills for one year. Imaging revealed a dilated common bile duct (CBD) with sludge, and endoscopic retrograde cholangiopancre
