Impact of JC virus antibody testing in patients with Crohn's disease with loss of response to infliximab: a Markov model.

BACKGROUND: The optimal treatment strategy for patients with Crohn's disease who have loss of response to the anti-tumor necrosis factor α medication infliximab is uncertain. Natalizumab has an alternative mechanism of action, but its use has been limited by the risk of progressive multifocal leukoencephalopathy. In this study, we performed a decision analysis assessing the impact of JC virus (JCV) antibody testing and natalizumab utilization for loss of response to infliximab. METHODS: We constructed a Markov model to assess the difference between unscreened natalizumab use (option 1), JCV a

Assessment of JC virus DNA in blood and urine from natalizumab-treated patients.

OBJECTIVE: Analyses were conducted to determine the clinical utility of measuring JC virus (JCV) DNA in blood or urine of natalizumab-treated multiple sclerosis (MS) patients to predict the risk of progressive multifocal leukoencephalopathy (PML). METHODS: A total of 12,850 blood and urine samples from nearly 1,400 patients participating in natalizumab clinical trials were tested for JCV DNA using a commercially available quantitative polymerase chain reaction (qPCR) assay. A subset of these samples was also tested using a more sensitive qPCR assay developed at the National Institutes of Heal

Differential diagnosis of multiple sclerosis: contribution of magnetic resonance techniques.

It is widely accepted that magnetic resonance imaging (MRI) findings are not totally specific for the diagnosis of multiple sclerosis. White matter lesions that mimic those of multiple sclerosis may be detected in both normal volunteers and patients harbouring different diseases. Virtually all the characteristic features of multiple sclerosis are sometimes encountered in other conditions affecting predominantly the white matter. Different conditions such as vasculitis, subcortical atherosclerotic leukoencephalopathy, Lyme disease, or acute disseminated encephalomyelitis can be virtually indist

[Diagnosis of diffuse encephalopathies in adults with HIV infection. 2].

A variety of the central nervous system lesions may cause a diffuse encephalopathy in AIDS patients. Apart from viral encephalitides already described in part one, metabolic encephalopathies are a classical cause of diffuse brain dysfunction and are frequent at the terminal stage of the disease. Atypical forms of some infectious, vascular or tumoural processes which usually determine focal lesions, may cause diffuse encephalopathies. These forms are not exceptional in AIDS. The association in the same patient of lesions due to different agents is the rule. Whereas most of the neurological comp

What's new in cerebral pathology in acquired immune deficiencies?

Over the last few decades, a new pathology has appeared, directly related to the modified immune status of the hosts. It presents several distinctive points. The central nervous system is particularly affected. The opportunistic pathogenic agents do not usually injure the brain parenchyma and are not known for their aggressiveness in normal adults (papovavirus). Diagnosis of these different diseases is often difficult, some biological tests being irrelevant because of alterations of the immune system (toxoplasmosis). The lesions may be exclusively located in the brain (tuberculosis, lymphomas)

A study on the influence of papovavirus upon encephalitis caused by SSPE agent.

To investigate a possible interaction between the SSPE agent and papovaviruses, histopathological and statistical evaluations were made of encephalitis induced in hamsters by intracerberal inoculation with isolates from SSPE and PML patients, and with BK virus, alone or in combination. With SSPE virus alone, hamster brains showed a reduction of susceptibility to encephalitis with increasing age. A modification of the pathology of SSPE encephalitis was observed after administration of the PML isolate or BK virus: encephalitis was either enhanced or inhibited depending on the time the papovaviru

HIV-associated neurological infections in a Brazilian tertiary care center: clinical-epidemiological features and predictors of in-hospital mortality.

Neurological manifestations remain a significant cause of hospitalization and in-hospital mortality among people living with HIV (PLWH), even in the era of antiretroviral therapy (ART). This study aims to describe the clinical and epidemiological profile of PLWH with neurological opportunistic infections (nOIs) and to identify factors associated with in-hospital mortality. We conducted a retrospective cohort study with PLWH aged >18 years hospitalized due to nOIs between November 2017 and December 2021 at a tertiary hospital in Brazil. Demographic, clinical, and laboratory data were extracted

Retinal atrophy in multiple sclerosis is similar with extended versus standard interval natalizumab therapy.

BACKGROUND: Retinal ganglion cell + inner plexiform layer (GCIPL) atrophy reflects global neurodegeneration and disability in multiple sclerosis. Extended interval dosing (EID; every 5-8 weeks) natalizumab reduces progressive multifocal leukoencephalopathy risk in people with relapsing-remitting multiple sclerosis (PwRRMS) versus standard interval dosing (SID; every 4 weeks). OBJECTIVES: To assess if EID versus SID natalizumab differentially impacts GCIPL atrophy, other retinal layer (peripapillary retinal nerve fiber layer (pRNFL), inner nuclear layer (INL), and outer nuclear layer (ONL)) at

Selective IgM Hypogammaglobulinemia and Multiple Sclerosis Treated with Natalizumab and Ofatumumab: A Case Report.

Background: B-cell-depleting drugs targeting the CD20 antigen have been increasingly implemented as an "exit strategy" from natalizumab for relapsing-remitting multiple sclerosis (RRMS) patients due to the increased risk of progressive multifocal leukoencephalopathy. Data on recently approved anti-CD20 drugs, such as ofatumumab serving as a natalizumab "exit strategy", are lacking. Furthermore, due to their immunosuppressive mechanism of action, prolonged use of these "highly effective" drugs is associated with the development of hypogammaglobulinemia and, consequently, a higher risk of infect

Identification of JC polyomavirus in upper respiratory samples from Portuguese children.

BACKGROUND: JC polyomavirus (JCPyV) is ubiquitous in the human population and the causative agent of a rare, fatal and demyelinating disease of the central nervous system named Progressive Multifocal Leukoencephalopathy (PML). The route of JCPyV transmission remains unclear, but high values of seroprevalence suggest an easy and frequent mode, such as respiratory route. OBJECTIVES: The present study aims to investigate the presence of JCPyV in upper respiratory samples and contribute to the elucidation of the JCPyV transmission pathway. STUDY DESIGN: Nasopharyngeal swabs from 587 Portuguese i

Infections in the Era of Targeted Therapies: Mapping the Road Ahead.

Immunosuppressive treatment strategies for autoimmune diseases have changed drastically with the development of targeted therapies. While targeted therapies have changed the way we manage immune mediated diseases, their use has been attended by a variety of infectious complications-some expected, others unexpected. This perspective examines lessons learned from the use of different targeted therapies over the past several decades, and reviews existing strategies to minimize infectious risk. Several of these infectious complications were predictable in the light of preclinical models and early

COS-7 cells are a cellular model to monitor polyomavirus JC miR-J1-5p expression.

Polyomavirus JC (JCPyV) is a ubiquitous human neurotropic virus that can cause progressive multifocal leukoencephalopathy (PML), sometimes as a consequence of drug treatment for disabling diseases, including Multiple Sclerosis. JCPyV expresses microRNAs (miRNAs), and in particular miR-J1-5p, but at now we have limited knowledge regarding this aspect. In the present study the expression of JCPyV miR-J1-5p was measured in infected COS-7, to verify if and when this miRNA is expressed in a cell model of JCPyV-MAD-4 strain infection. Results showed that miR-J1-5p expression was relatively constant

Practical approach to monitoring and prevention of infectious complications associated with systemic corticosteroids, antimetabolites, cyclosporine, and cyclophosphamide in nonmalignant hematologic diseases.

Corticosteroids constitute a first-line therapy for adults and children suffering from nonmalignant immune-mediated hematologic diseases. However, high disease relapse rates during the tapering period or upon drug discontinuation result in long-term corticosteroid use that increases the risk of infection. This same concept applies to other immunosuppressive agents, such as antimetabolites, calcineurin inhibitors, and cyclophosphamide. Corticosteroids are associated with a length-of-treatment and dose-dependent risk for infection. Screening and antimicrobial prophylaxis against tuberculosis, he
