Malignant hyperthermia is a seemingly rare genetic myopathy. Hypermetabolic crisis accompanied by a rise in body temperature to as high as 44 degrees C is its hallmark. Malignant hyperthermia is usually triggered by potent inhalated anesthetics and/or depolarizing muscle relaxants. Because of the extraordinary risk of death in patients who are at risk, plastic surgeons may be reluctant to operate on these patients. Five such patients were referred to the Plastic Surgery Service and the UCLA Malignant Hyperthermia Center for anesthetic and surgical management following plastic surgical procedur

Foot and ankle deformity in the face of neuromuscular involvement will challenge the podiatric surgeon's ingenuity. The basic principles of management of the various major conditions encountered in spastic and flaccid muscle paralysis-paresis has been discussed. The use of a combined approach of hard and soft tissue surgery is emphasized as well as an intimate understanding of the specific neuromuscular process itself.

Chronic dislocation of the distal radio-ulnar joint is usually treated conservatively or by resection of the ulnar head (the Darrach procedure). Recently there has emerged a trend towards reconstructive stabilization procedures, based on modifications of old methods. This article describes a stabilization procedure for correction of chronic subluxation of the distal radio-ulnar joint (ulnar-dorsal) using the flexor carpi ulnaris tendon. A series of five patients is presented together with a historical review of the development of reconstructive approaches in this area.

A retrospective survey of fifty shoulders in thirty-five patients with recurrent posterior instability revealed generally poor results of treatment, particularly of operative treatment. Only eleven of the shoulders had a primary traumatic insult as the event initiating the instability. Although forty-one of the affected shoulders demonstrated voluntary instability, it was the associated involuntary, unintentional instability of these shoulders that prompted the patients to seek medical attention. Pain and functional impairment relating to work and to the activities of daily living were not sig

Electrical injury results in serious damage or amputation of the upper extremity in a significant percentage of survivors. Fourteen of sixteen patients with electrical injuries followed over a two and a half year period had major hand wounds. Careful wound assessment, management, and closure combined with appropriate reconstructive technics now available provide the greatest potential for restoration of satisfactory quality of life. Because these injuries are often of an industrial nature, occupational vigilance and prevention remain essential.

AIM: The objective of this study was to describe the outcome of reverse shoulder arthroplasty (RSA) combined with modified L'Episcopo procedure at long-term follow-up (5 to 12 years).
METHODS: A retrospective review of 17 RSAs (mean age 67.2 years) with the modified L'Episcopo procedure conducted between 2006 and 2016 was performed. All patients had a combined loss of active elevation and external rotation with an irreparable posterosuperior rotator cuff tear. Clinical assessment was performed with a minimum follow-up of five years (mean 97.3 months). Outcome measures included range of motion

Forearm supination contractures can occur as a result of neurological derangement of the upper extremity. Primarily, this is observed in patients with neonatal brachial plexus birth palsy. The contractures develop slowly over time and become problematic in childhood as the patients begin requiring forearm pronation for activities of daily living including typing on a keyboard and writing. Although supination contracture is a well-described sequelae of neonatal brachial plexus birth palsy, there is a paucity of literature describing techniques and outcomes for reconstruction and restoration of

Treatment of advanced parotid or cutaneous malignancies often requires sacrifice of the facial nerve as well as resection of the parotid gla … s, nerve grafting utilizing motor nerves to the vastus lateralis muscle, and orthodromic temporalis tendon transfer. Further considerations relative to the eye, forehead, and long-term facial refinement are also discussed.

Fifteen non-opposable triphalangeal thumbs (eight patients) were treated with a new two-staged surgical technique. The first stage included excision of the proximal interphalangeal joint and osteotomy of the thumb metacarpal, and aimed to restore a correct thumb length, position and alignment and to create a wide thumb web space. The second stage consisted of an opponensplasty with refining of any of the first stage components, as necessary. The mean age at surgery was 3.2 years (range, 2.5-6 years). After a mean follow-up period of 42 months (range, 26 months to 7 years), the targets of treat

BACKGROUND AND PURPOSE: Double elevator palsy (DEP) is a monocular elevation deficiency in abduction and adduction characterized by hypofunction of the superior rectus (SR) and inferior oblique muscles. Only a limited number of studies are published on the management of this problem. Therefore, we studied and report and add our experience with emphasis on the indications and types of surgery for DEP.
PATIENTS AND METHODS: The records of 18 patients with DEP out of 3612 strabismic cases (0.5%) were reviewed. Fourteen underwent surgery. Inferior rectus (IR) recession was performed in cases with

ACL reconstruction with patellar tendon graft has become a standard procedure. The graft can be inserted either using two tunnels and a lateral femoral incision or with a femoral half tunnel drilled from the joint, thus avoiding the lateral incision. Advantages of the single-incision technique in the early rehabilitation period have been claimed. 40 patients with ACL deficiency were included in a prospective randomized trial comparing single and two-incision technique with a follow-up period of 4 years. Preoperative data did not show any significant difference between the two groups. At follow

Tibial tunnel placement during endoscopic anterior cruciate ligament (ACL) reconstruction has received increased emphasis in the recent literature. Appropriate tunnel length is a critical technical consideration. A tunnel that is too short results in graft extrusion, necessitating supplemental fixation techniques. A tunnel that is too long may make distal fixation and femoral tunnel placement difficult. A simple rule is proposed that allows for correct tunnel length and allows placement of the bone plug consistently within the tibial tunnel, allowing interference screw fixation.

For correction of instability of the carpometacarpal joint (CMC joint) of the thumb in combined paralysis of ulnar and median nerves in leprosy bone fusing procedures have been used, but they are not desirable and can often be avoided. A procedure analogous to the "Extensor pollicis brevis deviation graft operation" for the correction of instability of the metacarpophalangeal joint of the thumb is described here. The new procedure appears to be useful to correct and stabilize the subluxated carpometacarpal joint of the thumb actively during the use of the hand. When thumb web contracture has o

Thirty-three patients with cerebral palsy had iliopsoas transfers as part of their surgery for hip instability. All had total-body involvement, spastic cerebral palsy and none could walk. At the time of surgery, eight hips were subluxated and 39 were dislocated or severely subluxated. Mean follow-up was 8 years in patients between 8-25 years old. Forty-five of the 47 hips were located. Thirty patients had an accompanying scoliosis, and, in 10 patients, the rib cage impinged on the pelvis. Hip flexion had decreased in most patients. Sitting ability had not improved in any patient, and had in fa

Out of the multiple methods for treating Lunatum-malacia the following were used in our hospital: shortening of the radius, pisiform-transposition, cancellous bone graft, tendon interposition arthroplasty, intercarpal arthrodesis, prothetic replacement, wrist arthrodesis and denervation of the wrist. Because of non satisfactory results the conservative treatment was abandoned. The indication for one of the different operative methods depended on the radiological Decoulx-Stage, the profession and the age of the patient and his willingness to cooperate. From 1972-1988 36 patients with Kienböck's
