Surgical treatment of wrist deformity in hereditary multiple exostosis.

Wrist deformity represents a unique problematic entity in patients with Hereditary Multiple Exostosis (HME). We report our experience in the treatment of wrist deformities due to HME using three surgical procedures and the outcome of 12 wrist surgical corrections by comparing preoperative, postoperative and last follow-up radiographic values of the carpal slip, radial articular angle and ulnar shortening. Eight out of 12 forearms did show a postoperative improvement of the radiographic parameters, although recurrence of the deformity occurred frequently. Two forearms showed no change in postop

Bilateral Floating Knee Injuries Managed by Intramedullary Nailing - An Unusual Injury.

INTRODUCTION: Congenital pseudoarthrosis is a rare disorder, and mostly occurs in the radius, ulna, tibia, and fibula, but is not reported in the femur. Congenital cases that were reported in the literature, most were associated with neurofibromatosis, cleidocranial dysostosis, congenital constriction band syndrome and fibrous dysplasia and the acquired cases followed fractures of bones or tuberculosis. Since the life span of the patient is affected rarely, clinical examination of these patients and serial radiographs can give us plenty of information about the limb abnormality, its clinical c

Outcome of diaphyseal forearm fracture-nonunions treated by autologous bone grafting and compression plating.

BACKGROUND: The treatment of forearm fracture-nonunions continues to represent a therapeutic challenge, and reported outcomes are moderate at best. Limiting aspects of this particular anatomic location include the relation between restoration of shaft length with the anatomy and long-term functional outcome of adjacent joints, as well as the risk of elbow and wrist stiffness related to prolonged immobilization. The present study was designed to assess the outcome of autologous bone grafting with compression plating and early functional rehabilitation in patients with forearm fracture non-union

[Arthroscopic management of recent or chronic lesions of Triangular Fibrocartilage Complex of the wrist.].

Lesions of the triangular fibrocartilage complex of the wrist (TFCC) have perfectly been dismembered by Andrew Palmer and have largely benefited from progress of arthroscopy of the wrist. One distinguishes thus traumatic lesions (class 1) individualized according to their localization, central (1-A), ulnar (1-B), radial (1-D) or distal (1-C). Central lesions are classically associated to a positive ulnar variance. The clinical symptomatology evokes a meniscal like syndrome of the wrist. In case of failure of the medical treatment, it will be necessary to propose a surgical procedure guided by

[Analysis of the effectiveness of sequential plate internal fixation in correction of Madelung deformity after ulnar osteotomy and shortening].

OBJECTIVE: To investigate the effectiveness of sequential plate internal fixation in the correction of Madelung deformity after ulnar osteotomy and shortening. METHODS: The clinical data of 13 patients with Madelung deformity admitted between September 2015 and July 2021 were retrospectively analyzed. There were 5 males and 8 females with an average age of 18.3 years ranging from 17 to 23 years. The disease duration ranged from 12 to 24 months, with an average of 17 months. Three cases had a clear history of trauma. All patients had external radial deviation deformity and limited movement of

Vascularized Small-Bone Transfers for Fracture Nonunion and Bony Defects.

Vascularized small-bone grafting is an efficient and often necessary surgical approach for nonunion or necrosis of several bones in particular sites of the body, including scaphoid, lunate, distal ulna, and clavicle. The medial femoral condyle is an excellent graft source that can be used in treating scaphoid, ulna, clavicle, or lower-extremity bone defects, including nonunion. Vascularized bone grafting to the small bones, particularly involving reconstruction of damaged cartilage surfaces, should enhance subchondral vascular supply and help prevent cartilage regeneration. Vascularized osteop

Successful Strategies for Managing Monteggia Injuries.

Monteggia fracture-dislocations refer to traumatic ulnar fractures associated with proximal radioulnar and radiocapitellar joint instability. Careful clinical and radiographic evaluation of the entire limb in the acutely injured child is critical for timely diagnosis and appropriate treatment. Treatment principles include restoring and maintaining both ulnar length and alignment as well as radiocapitellar joint reduction. Recent information suggests that surgical treatment of acute injuries associated with complete ulnar fractures is safe and effective in maintaining bone and joint alignment.

[SURGICAL TECHNIQUE OF BLOCKING INTRAMEDULLARY OSTEOSYNTHESIS OF FOREARM FRACTURES].

An introduction of blocking osteosynthesis was recently included into practice of treatment of forearm fractures. The forearm has a very difficult biomechanics and extremely important functional value. There are really high requirements to accuracy of reposition and stability of forearm fracture fixation. The blocking osteosynthesis successfully combines fixation stability with functionality, although its performance technique isn't so simple, that forces experts to use other methods of fixation. It is possible successfully use advantages of the method by studying and keeping variety of operat

Changes in forearm bone mass and bone size after menopause--a mean 24-year prospective study.

OBJECTIVE: Bone loss and periosteal expansion is found after menopause. The accelerated early postmenopausal bone loss is not permanent but if the same accounts for the periosteal expansion is unknown. METHODS: Bone mineral density (BMD) and skeletal structure of the distal forearm were followed from menopause and on average 24 years (range 18-28) by single-photon absorptiometry at 12 occasions in a population-based sample of 81 Caucasian women with no medication or disease affecting bone metabolism. A Strength Index based on areal BMD and bone structure was calculated. Postmenopausal serum-e

Double type III epiphyseal separation of distal ulna associated with ulnar nerve palsy.

This article presents a previously unreported double type III epiphyseal separation of the ulnar head associated with ulnar nerve palsy. The ulnar epiphysis burst into volar and dorsal segments, the latter giving rise to the ulnar nerve palsy. The patient was treated by open reduction of the translocated epiphysis and neurolysis of the ulnar nerve. Although good clinical results were obtained after surgery, forearm rotation remained limited, confirming that open reduction immediately after injury is mandatory in treating type III epiphyseal separations.

Real time ultrasonography in the assessment of movement at the site of a scaphoid fracture non-union.

We investigated the use of real time linear ultrasonography in determining movement at a scaphoid fracture site in 27 patients with non-united scaphoid fractures. 24 of these patients had surgical treatment. Fracture movement was observed at the time of surgery and this was compared with the ultrasonographic findings. The technique proved to be 100% specific for visualization of movement at the fracture site and it was non-invasive. However, it was of no benefit in assessing proximal pole non-union.

The distal radioulnar joint.

A phylogenic review shows the DRUJ to be a highly evolved and specialized structure. The diarthrodial joint, in combination with the TFCC, ligament complex, and oblique fibers of the distal interosseous membrane, permit rotation while preserving stability and transmitting the load borne by the radiocarpal joint to both bones of the forearm. Our improved, yet imperfect, understanding of the anatomy, biomechanics, and pathophysiology of the region, abetted by new imaging modalities and wrist arthroscopy, permit more precise diagnosis and thus more rational treatment. The persistent inconsistency

Post-traumatic cubitus varus. Long-term follow-up of corrective supracondylar humeral osteotomy in children.

The results in twenty-four patients who had had a supracondylar osteotomy of the humerus to correct post-traumatic cubitus varus were reviewed after the patients had completed skeletal growth. The average age of the patients was 7.9 years at operation and thirty-one years at the time of follow-up. The average duration of follow-up was twenty-three years. According to our grading system, seven patients had a good; six, a fair; and eleven, a poor result. All but two of the nineteen patients in whom the humero-ulnar angle had been measured preoperatively lost correction that had been obtained at

Arthrosis in the elbow joint of young rapidly growing dogs. VII. Occurrence in the Rottweiler breed.

The author has previously shown that lesions of the coronoid process are the most common reason for elbow lameness and osteophyte formation of the elbow joint in the Rottweiler and other larger breeds. To get more information regarding the elbow status of the Rottweiler breed, 207 dogs were radiographically investigated. Of these, 66 dogs were admitted because of elbow lameness, while 141 dogs more than 12 months of age, had never revealed foreleg lameness. Osteophyte formation was found in one or both elbow joints in 89.4% of the lame dogs and in 68.1% of the dogs free from lameness. When try
