The current understanding of varicocoele pathophysiology does not completely explain the clinical manifestation of infertility. The present treatment options such as antioxidant supplementation and varicocoelectomy only help ≈35% of men to achieve spontaneous pregnancy.

This article reviews the current diagnostic work-up of subfertile men and the evidence based treatment options. Epididymal ultrasonography has proved effective in finding anatomic obstructive alterations affecting the epididymis. The authors investigated two groups: 1. Bilateral vas agenesia (20 epididymis)--(sensitivity of EUS--100%) 2. Normozoospermic noninfertile patients (before different surgical exploration)--(45 epididymis) EUS resulted "ideal" epididymis in 36 out of 45. EUS was "altered" in 9 cases. The ability of US to diagnose the pathogenesis of the acute scrotum is unsurpassed by any other imaging modality. It is the first imaging performed in patients with acute scrotum. Knowledge of the normal and pathologic sonographic appearance of the scrotum and proper sonographic technique is essential for accurate diagnosis of acute scrotum.
